Peri-implant soft tissue and bone from 12 patients undergoing removal of stainless steel miniplates and screws after healing of jaw fractures were studied with regard to histomorphology and metal content. Three patients with titanium plates were also included. Light microscopy and scanning electron microscopy with energy-dispersive X-ray microanalysis were used. Non-osseous tissue adjacent to devices of both materials showed fibrosis, including areas of mild chronic inflammation. The cellular picture was dominated by fibroblasts with small aggregates of lymphocytes and scattered macrophages. A connective tissue collar was found between the bone tissue and the screws of both stainless steel and titanium. Bone formation was also evident adjacent to screws of both materials. Stainless steel or titanium particles 5–50μm in diameter were found in both soft tissue and bone next to implants of their corresponding bulk material. The amount of metal impregnation varied between individual sections, and fewer particles were found in the bone specimens than in soft tissue. The mild inflammatory changes were not restricted to areas of metal impregnation.

The present investigation was designed to determine the individual variation of plaque levels in adults over a period of 3 months. The participants were 20 healthy subjects, 11 men and 9 women, with a mean age of 42.3±7.3(SD) years. Plaque area was determined with a computerized image analysis system and expressed as percentage of tooth area (P% Index). The scoring procedures were carried out three times (tests 1, 2, and 3) at an interval of 4 weeks. After each test all plaque was removed. All subjects were instructed to maintain their personal oral hygiene habits during the study. The results showed no significant differences for the means of P% Index between tests 1, 2, and 3 in the whole mouth, upper and lower jaws, right and left sides, or each tooth surface. The intraindividual plaque levels for individual tooth surfaces were closely correlated between tests 1, 2 and 3. The intraindividual plaque level was consistent in tests 1, 2, and 3 with standard deviations within subjects ranging from 1.1% to 6.2%. This study suggests that individual plaque levels seem to be consistent on the basis of tooth surface over a certain period of time. Individual plaque control should be more directed towards tooth surfaces with comparably high plaque accumulation within the mouth for significant reduction of the overall plaque levels.

Specimens made from denture base materials were leached in Ringer solution and in ethanol. The specimens comprised a heat-cured product processed in two different ways and two cold-cured materials. The organic compounds leaching from the specimens to the solutions were separated, identified, and quantified by a combined gas-chromatography and gas-chromatography/mass-spectrometry technique. Additives and degradation products, possibly made by free radical reactions, were released from the denture base materials. In Ringer solution only phthalates could be quantified. In ethanol solvent, biphenyl, dibutyl phthalate, dicyclohexyl phthalate, phenyl benzoate, and phenyl salicylate were quantified. In addition, copper was found in the ethanol solvent from one of the denture base materials. The amount of leachable organic compounds varies among different materials. Processing temperature influences the initial amount of leachable compounds.

The frequency of temporomandibular disorders (TMDs) and radiologically observed abnormalities in the condyles of the temporomandibular joints (TMJs) of professional violin and viola players was investigated in 26 orchestra violinists/violists (VP group) and in their sex-, age-, and dentition-matched controls (C group). A routine clinical stomatognathic examination, a standardized interview, and radiography of the condyles were carried out for all subjects. The VP group showed a higher frequency of subjective symptoms and clinical signs of TMD, such as palpatory tenderness of masticatory muscles, TMJ clicking, painful mandibular movements, and deviation on opening or closing. There was no difference between the groups in terms of radiologic findings in the condyles. Weekly playing hours correlated positively with some signs of TMD. It is concluded that professional violin or viola playing might be a predisposing factor for TMD.

Radiographic findings in mandibular condyles were studied from the pre- and post-treatment panoramic radiographs of 625 orthodontic patients. The subjects' mean age was 11 years at the start and 14 years at the end of active orthodontic treatment. Radiographic condylar findings were seen in 14 (2%) subjects before treatment and in 54 subjects (9%) after treatment (p<0.001). In age-related controls (n= 783) condylar findings were seen in 3% (p<0.001). The condylar finding was 'flattening only' in half of the patients and in one-third of the controls with condylar findings. Activator treatment was associated with condylar findings (p<0.05). Condylar findings increased with age in the orthodontically treated subjects (p<0.05) but not in the unselected population controls. This may mean that condyles become more sensitive with age in children. Increase with age may be partly due to the radiographic interpretation, since minor condylar findings are difficult to observe in young children, and partly due to differences in treatment modalities and the duration of treatment.

The aim of the present study was to compare the effect of transcutaneous electric nerve stimulation (TENS) with the flat occlusal splint in the treatment of temporomandibular joint (TMJ) disk displacement without reduction. Thirty-one patients were included and randomly selected to be treated 6 weeks with either TENS (90 Hz, 30 min, three times/day) or with a flat occlusal splint (24 h/day. Those selected for the TENS group had one electrode placed over the painful TMJ and another electrode over the anterior temporal muscle. The splint group used a conventional flat occlusal splint with cuspid guidance. Both treatment groups visited the clinic once a week. Symptoms and signs were registered before and after treatment. The intensity of pain was recorded with a visual analogue scale (VAS) and with an electronic pocket-sized recorder (Pain-Track) carried 1 week before and also the last week of treatment for continuous registration of pain. Measured with the VAS, half of the patients treated with splints became pain-free or their TMJ pain improved at least 50% both at rest and with jaw function compared with only 6% in the TENS-treated group. With regard to strictly chewing pain, the VAS-registered pain improved in two-thirds of the splint group, compared with 50% of the TENS group. With the Pain-Track device it was found that in most individuals pain was aggravated at mealtimes. The conclusion was that flat occlusal splints in several respects are better than TENS in the treatment of symptoms associated with TMJ disk displacement without reduction.

The objective of this study was to compare bonding of acrylic resin teeth treated in various ways to the polymethyl methacrylate (PMMA) denture base material. The joint surface of each acrylic tooth cured to heat-cured or autopolymerizing PMMA was either untreated, ground, or adjusted with mechanical retention. The bond to the PMMA was tested with a three-point loading test. To determine whether the bond failure was adhesive or cohesive, the fracture surfaces were analyzed visually and by scanning electron microscopy. The highest bond strength to both the heat-cured and the autopolymerizing PMMA was obtained by grinding grooves on the joint surface of an acrylic resin tooth before it was cured to the PMMA (p0.05).

The aim was to analyze the steering effects of 15 social factors on sugary products behavior at 4 years of age in 177 children. The parents had in most cases received information on dental health care at child health centers, with due respect to the level of caries among the children, who were classified as‘healthy’(no caries experience) (n= 83) or 'diseased' (caries experience) (n= 84). The parents filled in a mailed diet history form. Only sugary products unsuitable from a cariologic point of view were considered. The products were given scores reflecting the frequency of intake. The social factors were social background, family, information, and conceptual factors. By means of multiple regression analysis, explanatory values of the social variables for sugary products behavior were estimated. For the total material, 13% of the variance was explained by all variables combined. Among the social background factors, 'parents' age' was statistically significant (p<0.05). Among family factors, 'day-care mainly at home' and 'oldest child' were the most important. All information factors proved to be nonsignificant. Of the conceptual factors, only 'importance of genetic factors' was statistically significant. For the healthy group 26% of the variance was explained by all variables. 'Mother's age', 'importance of genetic factors', and 'oldest child' were statistically significant. For the diseased group all variables were nonsignificant. Explanatory values were negligible. The important conclusions were that very few of traditionally conceived social variables seem to influence 4-year-old children's sugar behavior. Parents of children with caries appear to be a heterogeneous group with an irrational behavior.

Utilization of dental services by 30 diabetic and 30 nondiabetic subjects was assessed by longitudinal monitoring over a period of 3 years. All subjects were examined clinically three times, and their treatment consisted mainly of cariologic and periodontal treatment. The treatment was delivered by a dentist and an expanded-duty dental hygienist. The study groups were similar with regard to the total number of dental visits needed. However, the treatment of diabetic subjects was more demanding in that more dentist's workload was needed for the diabetic group. They also missed more appointments without cancellation and therefore more office time had to be reserved for them.

The purpose of this study was to evaluate a method for computer-assisted image processing to assess quantitatively fractional bone volume in sections of bone biopsies (part I). A second purpose was to apply this method in an experimental study to determine bone loss by measuring fractional bone area in sections of calvarial bone of gastrectomized rats and to compare this with a control group. Stained paraffin sections of skull bone of young rabbits (part I) and rats (part II) were examined. The histologic sections were placed on a microscope. A video camera was connected to the microscope, and the image transferred to a display monitor connected to a PC with dedicated software, performing the measurements. In the first part of the study tests of the reproducibility of the method were performed. The influence of factors such as external illumination and light intensity in the microscope was evaluated by measuring the same area several times at different times (= different illumination) and at different microscope light intensity levels. The automatic method was compared with a manual method, using a Merz grid but automatically calculated. The difference between the means of the manual and automatic measurements was determined with the pairedttest. In the second part of the study the unpairedttest was used to determine the differences between the control group and experimental group, both for the manually and the automatically measured values. The data of this study indicate that the automatic image processing method is an efficient tool for determining bone loss by measuring bone area to estimate fractional bone volume in histologic sections. However, the quality of the histologic sections is of paramount importance for precise measurements.
