The uptake of fluoride in enamel and dentin after application of an experimental fluoride varnish has been studied and compared with the uptake from a commercially available varnish (Duraphat®). Each varnish was applied to 10 extracted human cuspid teeth. Successive etchings with perchloric acid were carried out separately in enamel and root dentin to obtain samples at three different depths. The concentrations of fluoride and calcium in the samples were determined, and the fluoride uptake was calculated. The experimental varnish gave a significantly greater fluoride uptake at all three sample depths in both enamel and dentin.

The longevity of amalgam restorations in premolars and first and second molars in long-term recall patients has been calculated by means of survival analysis. The present retrospective longitudinal study includes information from 32 patients and their records over a time period of 17 years. The data were recorded by six dentists. The estimated time of 90% survival was 7 years. After 17 years the survival estimate decreased to 78%, indicating a favorable longevity of amalgam restorations in patients taken care of in an established recall system.

The purpose of this study was to evaluate a comprehensive, on-site, oral health care program for nursing home residents. Before and after a 1-year evaluation period the elderly residents in the nursing home (NH-1) were interviewed and examined clinically. A comparable control group comprising the elderly residents in another nursing home (NH-2) were also interviewed and clinically examined before and after a 1-year period. The main objectives of the program were to increase the utilization of the dental care system, to meet the need for immediate attention, to improve oral hygiene, to reduce the prevalence of oral mucosal lesions, gingivitis, and untreated decay, and to meet the realistic prosthodontic treatment need. All the elderly residents accepted a yearly examination by a dentist, and 50% were treated in NH-1 during the evaluation period, compared with 16% in NH-2. The need for immediate attention was reduced by 56% in NH-1 and had increased by 20% in NH-2. Whether the activities in NH-2 qualify as an oral health program with on-site examinations and referral for treatment outside the institution is discussed. The annual cost per resident during the evaluation period was a little more in NH-2 than in NH-1. The conclusion is that oral health improved during the observation period in both nursing homes. The rate of participation and the improvement in oral health was most evident in NH-1.

Mandibular incisors were extracted and allowed to dry in air for 45 min. They were then root-planed extensively, leaving a cervical collar and the apical region uninstrumented, to prevent direct contact between root surface and alveolar bone after replantation while, at the same time, enabling precise and stable reseating of the tooth. Control teeth were replanted either without further treatment or after treatment with citric acid for 3 min. Experimental teeth were treated before replantation with 1% SnF2for 5 min, 1% tetracycline HCl for 5 min, or 1% SnF2followed by 1% tetracycline. Histometric analysis of healing in the root-planed areas showed minimal amounts of inflammatory resorption and ankylosis after 21 days in experimental teeth as compared with the control teeth. A persisting inflammatory reaction in the periodontal ligament without root resorption was, however, frequently seen. The observations confirm that the frequency of adverse healing reactions after delayed replantation of teeth from which nonvital soft tissue remnants have been removed can be reduced by demineralizing the root surface and preventing mechanical trauma to the root surface in the postoperative period. Root surface treatment with SnF2followed by tetracycline resulted in complete absence of inflammatory resorption and ankylosis in this short-term experiment.

In 1986, 149 patients were provided with titanium crowns. The method used for fabrication of the titanium copings involved two principles: machine duplication of models and electric discharge machining. For the veneering of the copings, Isosit was used for the first 90% of the crowns in the series, and the Dentacolor-Silicoater technique for the last 10%. Of 205 individual crowns cemented in 1986, 167 could be examined after 2 years. The crowns were rated by four independent examiners using the CDA quality evaluation system. Bleeding Index and Margin Index were also used. The Margin Integrity score was recorded as satisfactory for all crowns examined over the period studied. A vast majority of the margins were rated as excellent. Isosit (n= 145) disclosed shortcomings including fractures and substantial deteriorations of Surface and Color and of Anatomic Form. With Dentacolor as veneering material (n= 18) the results with the factors Surface and Color and Anatomic Form were still rated satisfactory after 2 years, and no fractures of the veneering material were registered. Bleeding Index and Margin Index showed comparatively small changes after 2 years.

In a general adult Swedish population recurrent herpes labialis (RHL) was found in 3.1% of 20,333 people aged 15 years and more. When a 2-year history of RHL was considered, the total prevalence was 17.4%. There was a slight predominance among women. Only slight differences were found between various age groups. In a fraction of the population studied, constituting 2771 people, interviews concerning cases of RHL were carried out. Thus, for example, most individuals referred to only one herpes episode a year; the most prevalent site of eruption was an area comprising both the vermilion border and the skin, and most of the lesions showed healing times of 5–8 days. The prevalence of RHL was influenced by some tobacco habits. Thus, the prevalence was significantly lower among smokers, and especially among pipe smokers, than among people with no tobacco habit at all. Snuff dipping did not seem to influence the prevalence values.

Hyperplasia of the palatal epithelium was quantified in two groups of rats exposed thrice weekly to the carcinogen 4NQO for 2 weeks and 2 months, respectively. The lengths and areas of the epithelial layers were measured with a computerized line-following device. In the group treated for 2 weeks the maximum area of the nuclear layer was nearly three times and the maximum length of the epithelial/connective tissue interface almost twice the normal at the end of the carcinogen application period. The maximum area of the cornified layer was three times and the maximum lengths of the epithelial surface and the keratin/nuclear layer interface almost one and a half times the normal 1 week after painting with 4NQO. Thereafter the lengths and areas decreased gradually in both experimental groups. The area of the cornified layer and the length of the epithelial/connective tissue interface in the group treated for 2 months were significantly larger than those in animals treated for 2 weeks. These variables may be two of several indicators of prognostic significance in the assessment of dose-related premalignant epithelial hyperplasia.
