Acta Odontol Scand 1994;52:65-71. Oslo. ISSN 0001-6357.Osteoarthrosis-like changes were induced by means of experimental disk perforation in the right temporomandibular joint of rabbits. The turnover of proteoglycans and glycosaminoglycans was studied 16 weeks later, using35SO4. Tissues were sampled 1 day and 7 days after injection of the sulfate. The corresponding tissues from the left untreated joint were used as controls. After isolation of the glycosaminoglycans the incorporation of35SO4was estimated by scintillating counting. The extracted proteoglycans were analyzed, using gel electrophoresis, and the distribution of radioactivity was determined by autoradiography, followed by densitometry. Both the synthesis and rate of degradation of the proteoglycans were increased in the experimental disk, compared with those of the control. The net result of these metabolic changes seemed to be losses of small proteoglycans, whereas a slow increase in the number of larger ones may have occurred. The turnover rates of 4- and 6-sulfate increased, although their ratio remained unchanged at this stage of the osteoarthrosis-like process. In the condylar cartilage the turnover of large and small proteoglycans was also increased. The increase was most marked among those containing 6-sulfated galactosamino-glycans. The results concerning the experimental condylar cartilage indicated a decrease in the largest proteoglycan population, whereas the proportion of small proteoglycans was increased.

ISSN 00014357Enamel specimens from unerupted third molars were treated with the supernatant of mono-fluorophosphate (MFP)-containing toothpaste slurries in water. Calcium fluoride-like particles were formed on the enamel surface during a 24-h exposure. The globules were soluble in alkali. More particles were seen on enamel treated with a toothpaste containing both MFP and NaF. The experiments showed that the calcium fluoride-like material formed on enamel after treatment with MFP-containing toothpaste probably originates from the free fluoride present in the pastes. The relatively moderate deposition of fluoride may be of clinical significance because the teeth are so frequently exposed to toothpaste. No firmly bound fluoride was found on treatment of enamel with MFP-containing toothpaste.

ISSN 0001-6357Utilization of dental services by Finnish seamen was surveyed late in 1990. Three hundred and sixty-five respondents on different vessels at sea for more than 5 days returned a pretested questionnaire. A randomly drawn control group of 230 workers at a ventilation-equipment-producing plant was also surveyed. Fifty-five per cent of the seamen studied and 73% of the control group stated they had visited a dentist within the year before the survey. One-third of the seamen stated that an acute dental condition had been the reason for their last dental visit. Almost one-third of the seamen and 5% of the control group reported having last visited a dentist at a health center. When questioned separately, 14% of the seamen stated that they had not received dental treatment fast enough when needed. One-third of the seamen surveyed offered ideas for improving their dental services, half of them suggesting an increase in facilities and personnel at health centers for seamen. Periodic oral examinations for and full use of oral health self-care procedures by seamen are recommended.

ISSN 0001-6357Twenty-nine 3-month-old female Wistar rats were labeled by means of a single intraperitoneal tetracycline injection. Nineteen animals were subsequently ovariectomized, whereas a control group of 10 animals underwent sham operations. All the animals received the basal diet, and 10 of the ovariectomized animals were given an additional dietary xylitol supplementation (5%). Three months later the animals were killed by decapitation, and dentinal apposition on the molars was measured. The results indicate that supplementation of the diet with 5% xylitol had an attenuating effect on the enhanced dentin formation caused by ovariectomy, but the mechanism remains unsolved.

ISSN 0001-6357To improve diagnostic procedures and facilitate clinical decision-making, computer-assisted image analysis was performed on color slides from 30 patients with histopathologically verified oral lichenoid reactions. Areas from white hyperkeratotic and adjacent red inflamed areas of the lesions were selected and subjected to image analysis. The digitization of the color slides was done by means of an image scanner, and the digital information was transmitted to a personal computer for subsequent feature extraction and analysis. The different oral lesions were characterized as the difference in mean values between white hyperkeratotic and red inflamed areas, respectively, compared with clinically normal tissue. Statistical analyses were made on three different color systems: Red-Green-Blue (RGB), normalized red-green-blue (rgb), and Intensity-Hue-Saturation (IHS). The results showed statistically significant differences in all color systems for both the hyperkeratotic areas and adjacent inflammatory reactions. A linear correlation was obtained when the results of the image analysis of color variations were compared with a clinical score system for hyperkeratosis and inflammation evaluated by two investigators independently.

ISSN 00016357.The microflora on 44 toothbrushes at 4 day-care centers in the city of Goteborg have been investigated as a presumptive risk factor for transmission of microorganisms by children. Non-supervised toothbrushing without the use of toothpaste was performed at the day-care centers twice a day. Streptococci, predominantly S. salivarius, S. sanguis, and S. mitis, were the most frequently recorded group of microorganisms and generally constituted the greatest part of the flora (on average, 50%). Beta-hemolytic streptococci were not found in any sample. Haemophilus species were noted in 82% of the samples, H. parainfluenzae being the most frequent, and H. influenzae being identified in only one sample. Anaerobes constituted on average a third of the microflora. Staphylococci were identified in 86% of the samples, S. epidermidis dominating. Fungi including molds were found in 50% of the samples, and from one day-care center large numbers of enteric organisms were identified. Thus this study shows that unsupervised toothbrushing at day-care centers can be questioned, more from a general hygienic point of view than from the risk of transmitting serious pathogens.

ISSN 0001-6357The prevalence of pain and dysfunction in the stomatognathic system was studied in a group of 19 long-term survivors after pediatric bone marrow transplantation (BMT), conditioned with total-body irradiation (TBI). Compared with the control group, the children and adolescents in the BMT group had a significantly reduced mouth opening capacity. A reduced translation movement of the condyles was diagnosed in 53% of children treated with TBI, compared with 5% in the control group. Signs of craniomandibular dysfunction were found in 84% of children in the BMT group, compared with 58% in the control group. Both irradiation and chemotherapy induce long-term alterations in connective and muscle tissues resulting in inflammation and eventually fibrosis. These changes in tissue homeostasis and concomitant growth retardation may lead to the observed malocclusion and reduced mobility of the temporomandibular joint, with subsequent muscle pain and headaches, which were found in this study.

ISSN 0001-6357Subjective orthodontic treatment need and associated factors were studied in a sample of 226 young Finnish adults. The subjects, applicants to enter the University of Helsinki, were interviewed immediately after their entrance examination. A need for treatment was felt by 14% of the subjects. For 47% of those expressing such a need their reason was appearance only, and for 25% it was both appearance and their ability to chew. Among subjects who reported a visible dental irregularity, 38% were dissatisfied with their dental appearance., and 25% felt the need for orthodontic treatment. Women were more often dissatisfied with their dentition than men. Subjective need for treatment was significantly associated with perceived visible dental irregularity and infrequent socializing but not with orthodontic treatment in childhood, pattern of dental attendance, or district of residence. Of various perceptions of anterior teeth, irregularly and wrongly positioned teeth were most often connected with treatment need.

ISSN 00014357The purpose of the present study was primarily to establish the oral health status of young adults in the area of Porto, Portugal. The assessment is based on a random sample of 30–to 39-year-olds with criteria identical to those of a Norwegian study of 35-year-olds. This makes it possible also to present a comparative analysis of the caries prevalence in Oslo, Norway, and Porto, Portugal. The results indicate lower DMF scores among the Portuguese (DMFS = 46.2) than the Norwegian (DMFS = 85.0) adults. The difference is primarily due to a greater number of filled surfaces among the Norwegians (FS = 59.7) than the Portuguese (FS = 4.4). However, carious surfaces are more prevalent among Portuguese than Norwegian adults (DS = 9.2 versus DS = 3.3). Both among Portuguese and Norwegian adults, oral hygiene and dental visits seem to play an important role with regard to the prevalence of decayed surfaces. Decayed surfaces were more prevalent among men than women, and a correlation between social status and prevalence of decayed surfaces was present in both societies.

ISSN 00016357Several studies indicate that xylitol is not metabolized to acids either in pure cultures of oral microorganisms in vitro or in dental plaque in vivo. Chronic consumption of xylitol-sweetened chewing gum resulted in reduction of dental plaque, suppression of mutans streptococci. and reduced adhesiveness of plaque. So far, four field studies with regimens including chewing gum and other xylitol-containing products and four clinical trials have been carried out. All of the latter studies showed that a daily intake of two to three pieces of xylitol gum resulted in a defined reduction of caries. There are indications that regular and prolonged use of xylitol chewing gum may have a caries-preventive effect.
