To evaluate the clinical quality of ceramic (Optec) inlays, the inlay production of 10 dentists, served by 3 dental laboratories, was examined independently by 2 calibrated examiners. Of the 57 patients with 205 inlays participating in the first examination, 47 patients with 145 remaining inlays participated in the 3-year follow-up reexamination. For 14 of the participants in the reexamination, altogether 21 inlays had fractured. The mean time in service before fracture was 26.4 months (range, 12-53 months). The inlays were rated using the California Dental Association (CDA) quality evaluation system. This examination showed that the mismatch of color had increased from the first examination to the reexamination and registered slightly roughened surfaces. As to anatomic form, reduced number of inlays did not receive an excellent CDA rating at the reexamination. Visible evidence of ditching along the margin had increased by almost 50%, and an apparent discoloration of the margin between the restoration and the tooth structure was seen in 73% of the reexamined inlays, compared with 7% at the first examination.

The aim of the present clinical double-blind crossover study was to investigate the effect of two different toothpaste detergents, sodium lauryl sulfate (SLS) and cocoamidopropyl betaine (CAPB), as compared with detergent-free paste, on 30 patients with frequent occurrences of recurrent aphthous ulcers. The study consisted of three 6-week periods during which die patients brushed twice daily with the different test toothpastes. The localization and number of new ulcers were assessed. A significantly higher frequency of aphthous ulcers was demonstrated when the patients brushed with an SLS- than with CAPB-containing or detergent-free placebo paste. An SLS-free toothpaste may thus be recommended for patients with recurrent aphthous ulcers.

In double-blind study, diclofenac sodium (Voltaren®), 50 mg two or three times day, was compared with placebo in 32 patients with pain localized to the temporomandibular joint (TMJ). The patients were allocated into two equally large groups. A visual analog scale was used to estimate the pretreatment degree of pain. The treatment effect was assessed as the frequency of joint and muscle pain and by the patients' own evaluation of improvement. The change in the clinical condition was assessed by tenderness to palpation of the TMJ and masticatory muscles and by mandibular mobility. The frequency of TMJ pain showed greater reduction in the diclofenac group than in the placebo group, and there was significant reduction of daily TMJ pain in the diclofenac group. The diclofenac group also showed significant decrease in tenderness to palpation of the masticatory muscles in comparison with the placebo group. The patients with short duration of pain showed the best response to diclofenac. There was no evidence in this study to prove that diclofenac should be used as primary treatment of TMJ pain, but it could be used as a complement to other treatments of acute TMJ pain.

No currently available bond test for dental metal—ceramic systems has yet gained general acceptance. Such bond test cannot be established without careful analysis of the stress distribution within the adherence region. The objective of the present study was to establish mechanical shear stress test and combine the results with finite element stress analysis of an idealized crown to enable comparisons of different metal—ceramic systems. The titanium—ceramic systems tested, both machined and cast titanium, showed higher shear strength values than the high gold—ceramic system used as reference. Since the latter system has been used successfully in the clinic for many years, it seems reasonable to assume that the bond strength of titanium—ceramic systems is quite satisfactory for dental crown-and-bridge work. In conclusion, the interfacial bond strength test proposed in the present study should be applicable to all of the currently known material combinations in which brittle fracture might be expected to occur.

The association between subsidized dental care and periodontal status was studied in male industrial workers in southern Finland in 1994. Clinical examinations and multiple-choice questionnaire were completed for 325 workers (age, 38-65 years) with access to subsidized dental care and 174 controls without access. The CPITN scores based on full-mouth recordings were analyzed, using both the individual and sextant as units of analysis. Overall, 6% of the subsidized group and 2% of the control group had no periodontal treatment need (p<0.05). Deep pockets≥6 mm were found in 5% of the subsidized workers and 11% of die controls (p<0.05). In the logistic regression analyses the probability of calculus was negatively associated with access to subsidized dental care. Smoking was the strongest independent factor affecting periodontal status. Our findings show positive relationship between access to subsidized dental care and periodontal status.

This study was planned and conducted by the Health Authorities of Karelia in the Russian Federation and the Faculty of Dentistry, University of Kuopio, Finland. The aim of die study was to analyze caries experience and treatment need among adults who responded to an invitation for dental examination and offered free dental treatment in two communities in Karelia. A total of 227 adults aged 30-72 years from 2 rural communities were examined clinically in accordancewithWHO criteria by team of Finnish dental specialists. The participants represented two major ethnic groups (Fenno-Ugrians, 68%; Slavs, 32%). The mean DMFT and DMFS were 15.8 and 58.8, respectively. Among dentate subjects (n= 225) 88% had at least one carious tooth and 80% had one or more filled or crowned teeth. Altogether, 34% of the subjects needed extraction(s), and 83% needed restorative treatment. The main finding of the study was that with regard to caries experience, oral health was quite good compared with that found in Finland and the UK.

Twenty patients complaining of symptoms deriving from their amalgam fillings and non-patient group were assessed by means of the perceptual projective Defense Mechanism Test (DMT). The test protocols were scored for 130 DMT variables and analyzed by means of the multivariate statistical method Partial Least Squares discriminant analysis. The objective was to try to distinguish die group with amalgam illness from the non-patient group by means of the DMT. The results showed that it was possible to distinguish the two groups significantly from each other. The most characteristic traits of the patient group were general lateness in perception and few emotional responses compared with the non-patient group and, especially, an inability to perceive die aggressive component in the stimulus picture. The DMT seems to be powerful method in the effort to understand the mechanisms underlying the problems of amalgam illness.

Dental education in the Nordic countries was founded in the late 1800s, but the doctor's degree in dentistry (Ph.D.) was established somewhat later. Since the first dissertation in Finland in 1891, total of 204 doctoral dentist candidates had defended their Ph.D. theses by 1991, 50% of them during the most recent 12 years. Over the 100-year period, 54% of die dentists' Ph.D. theses in Finland were defended at the University of Helsinki, 27% at Turku, 14% at Kuopio, and 5% at Oulu. Women constituted minority of the candidates (23%) during the first 90 years but 54% from 1982 to 1991. From 1984 to 1993 total of 374 dentist candidates in Finland, Norway, and Sweden defended their Ph.D. theses. The mean ages of die candidates ranged from 37.7 to 41.7 years for men and from 40.6 to 43.2 years for women. In the 10-year period on average 53 doctor's degrees were received per institute in Sweden, compared with 28 in Finland and 27 in Norway. In all three countries about 6 of 100 graduates in 1980s received doctor's degree in dentistry. Almost all of these Ph.D. theses were written in English and based on collections of articles. Female candidates dominated in Finland (56%), compared with 34% in Sweden and 26% in Norway, where, however, women's contribution increased most rapidly, being tripled from early 1980s to 1990s.

The test specimens were processed by autopolymerizing poly(methylmethacrylate) (PMMA), and their surfaces were untreated, polished in conventional manner with rag wheel, or coated with light-curing resin. The residual methylmethacrylate (MMA) content and its release into water from die specimens were measured with high-performance liquid chromatography. The lightcuring resin coating reduced most effectively the release of residual MMA into water during 2-day storage, but conventional polishing of the PMMA surface had similar effect when the mean values of groups were tested by means of one-way ANOVA (p<0.001). The residual MMA content was lowest in the test specimens coated with lightcuring resin, whereas only slight difference was seen when the untreated and polished test specimens were compared. This study suggests that not only light-curing resin coating but also the conventional polishing of the denture PMMA reduces residual MMA release into water in vitro.

As part of comprehensive investigation of 88-year-old people, caries prevalence, stimulated salivary secretion rate, buffer capacity, and the prevalence of lactobacilli and mutans streptococci were studied in subsample of 92 dentate subjects. The mean number of remaining teeth was 14.1±7.3. The DMFT was 25.2±3.0, and DF surfaces 38.3±22.7. Root caries experience was found in 85% of the subjects, with mean root caries index of 36.6 + 28.5%. The men had significantly higher mean values of salivary secretion rate (2.0±1.3 ml/min) than the women (1.1±0.6 ml/min) (p106) were found in 49% and 55% of the subjects, respectively, of whom most were denture wearers. The mean number of total mutans streptococci was higher in persons harboring bothStreptococcus mutansandS. sobrinusthan in subjects with onlyS. mutans.
