In 1988, 100 12- to 13-year-old children from two neighborhoods in Montevideo with different standards of living were examined. Twenty-three children with large numbers of mutans streptococci, lactobacilli, or decayed surfaces or a combination of these factors were considered to be at high risk of developing caries. Twelve of these 23 children were selected for special preventive measures. A second examination was made 18 months later, at which 81 of the originally 100 children were studied. The clinical and microbiologic variables were tested for their ability to predict caries by calculating their sensitivity, specificity, the positive and negative predictive values. The incidence of new DMFS was significantly higher in the children from the low than in those from the high socioeconomic area. It was also significantly higher in the children considered being at high risk than in those at low risk at the start of the study. Preventive measures reduced the caries risk. The highest sensitivity was obtained with the clinical test, whereas the highest specificity was obtained with the combined clinical and microbiologic tests. The findings indicate that methods for identifying children at risk which started to be used in. Scandinavia 20 years ago could be applied in the Uruguayan population of today. It is valuable to know this, as the resources for both treatment and prevention are very limited.

The personality characteristics in 32 patients with resistant burning mouth syndrome (BMS) after treatment of diagnosed medical and odontologic diseases were examined and compared with a sex- and age-matched control group. After evaluation of burning mouth symptoms, the personality, the psychologic functioning, and the quality of life were determined by using the Karolinska Scales of Personality (KSP), an additional Personality Scale (PS), a Psychological Functioning Scale (PFS), and a Quality of Life Scale (QLS). The result showed that, compared with a control group, the patients with resistant BMS had a significantly lower score in socialization scale and significantly higher scores in somatic anxiety, muscular tension, and psychasthenia scales. Furthermore, the patients with resistant BMS were significantly more easily fatigued and more sensitive and showed a tendency to be more concerned about their health. With regard to the psychologic functioning, the BMS patients had significantly more problems taking the initiative, more easily became dizzy, and had more sad thoughts. They also showed a tendency to report palpitations and/or indigestions more often. The observed significant differences in personality and psychologic functioning might suggest that the burning sensations are psychosomatic symptoms in these patients. We recommend that patients with resistant BMS should undergo psychologic investigation. If psychologic and/or psychosocial disturbances are diagnosed, adequate treatment should be offered.

Rat molars are indicators of exposure concentration and target organ content in chronic mercury vapor exposure. We wished to study the accumulation and persistence of organic and inorganic mercury in rat teeth and the effect of selenium on mercury retention. Male Wistar rats received either inorganic or organic mercury (with or without addition of selenite), selenite only, or no mercury or selenite (controls) in the drinking water for 4 weeks. Group A was killed after exposure. Group B was killed 20 weeks later. The mercury content was measured by cold-vapor atomic absorption spectrophotometry. The mercury content in the molars in group B was 66% and 77% less than in group A after inorganic and organic exposure, respectively. In the incisors the corresponding reductions were 90% and 97%. Selenite had limited effect on mercury retention in group A and none in group B. We suggest that rat molars and, by inference, human deciduous teeth may serve as indicators of organic and inorganic mercury exposure.

This study investigates the frequency of use of electric toothbrushes in relation to time of possession and social background variables. A questionnaire was sent to 129 patients who had acquired an electric toothbrush within 36 months. They were patients at the Department of Clinical Periodontology and at general dental clinics inÖrebro. Response rate was 96%. The brush was used daily by 62%, whereas 3% had ceased to use them. Frequency of use and encountered problems were related only for those who had had the brush for 3 years or more. Logistic regression on the frequency of use with the variables age, gender, marital status, education, time of possession, and received instruction showed no associations except in two cases. For patients at general dental clinics, only brush type, and for patients at the department, only encountered problems had explanatory value. The conclusion is that compliance level was high and unrelated to social factors.

The prevalence of clinical signs of temporomandibular disorders in 76-, 81-, and 86-year-old subjects living in Helsinki, Finland, were studied, using Helkimo's clinical dysfunction index. In percentage distribution, clinically symptom-free subjects (Di 0) constituted 20% of all those examined, and by percentage the largest group of subjects without signs was that of the oldest men (47%). Women had a higher prevalence of signs of temporomandibular disorders, and only 15% were clinically symptom-free. Four per cent of all those examined had severe symptoms. The commonest signs were impaired range of movement of the mandible and impaired function of the temporomandibular joint.

The marginal and internal fit of four different types of ceramic inlays—Cerec, Celay, Empress, and Vita In-Ceram Spinell—was determined after they had been luted on extracted premolars. For the Cerec inlays sharp and U-shaped proximal box shapings were also compared. There was no statistically significant difference either in the proximal fit or in the gingivoproximal fit between the four inlay systems studied, with the exception of the Cerec inlays made for preparations with sharp proximal boxes, which had wider marginal gaps. At the occlusal margins a significant-principle order of the gap width was established. The best internal fit was recorded for the Celay inlays, whereas there was no significant difference in the internal fit between the other systems. For the Cerec inlays the U-shaped proximal box shaping improved the marginal accuracy all around the restoration.

The present study measured in vitro occlusal wear, quantity of remaining double bonds, and volumetric filler content of eight dual-cure resin cements. Furthermore, the effect of light curing on wear and quantity of remaining double bonds was evaluated. The filler content varied between 31 vol% and 66 vol%. In vitro wear varied between 30μm and 65μm, and quantity of remaining double bonds between 19% and 38% when resin cements had been both chemically cured and light cured. When light curing had been omitted, wear varied between 36μm and 74μm, and quantity of remaining double bonds between 25% and 56%. One resin cement did not harden when not cured by light. Light curing improved the wear resistance of three resin cements by up to 44% and reduced the quantity of remaining double bonds of six cements by up to 36%. A three-dimensional regression analysis found wear to decrease with decreasing quantity of remaining double bonds and increasing volumetric filler content. A negative correlation was shown between compressive strength and wear.

The purpose of the present study was to describe dental health behavior and self-reported oral health problems among hospitalized psychiatric patients. Two hundred and forty dentate patients were interviewed with regard to participation in school dental care, dental visiting habits, self-assessment of oral health, dental anxiety, and oral hygiene habits. Dental visiting habits were associated with gender, psychiatric diagnosis, duration of mental disorder, and fear of dental care. Moreover, toothbrushing habits varied with gender, status as inpatient or day-hospital patient, number of admissions, and psychiatric diagnosis. Constant regular dental visits were observed among 31 % of the participants, whereas regular toothbrushing was reported by 55%. Compared with reference figures of the general population, these figures reflect a relatively poorer dental health behavior among the study participants. The study underlines the need for specific preventive dental programs, which aim at improving the poor dental health behavior among psychiatric patients.

Patients with chronic orofacial pain often undergo endodontic therapy or surgery in an attempt to eliminate or relieve their symptoms. This study examined the relationship among invasive dental treatment, pain reports, and disease conviction in a group of patients referred for pain management. Forty-eight patients referred to the Facial Pain Diagnostic Group at the Karolinska Institute, School of Dentistry, Stockholm, were investigated retrospectively. There were 43 women and 5 men (age range, 30–81 years) with orofacial pain of more than 6 months' duration, which the patients considered to be of dental or paradental origin. Despite dental treatment intended to relieve the pain, no permanent relief had been achieved. There was no statistical correlation among the different pain reports, the individual types of treatment, and the disease conviction. The reason for the high degree of disease conviction in this type of patient warrants further investigation.

Continuous measurements of mercury vapor in the breathing zone of the dentist were made under ordinary clinical conditions. Fifty old amalgam fillings were removed and replaced by new ones. The mercury vapor measurements were performed by means of atomic absorption spectrophotometry. On the basis of the type of suction device used, the measurements were divided into seven groups, each of which consisted of a series of measurements. During the cutting, filling, and polishing operations the mean mercury vapor levels in the breathing zone of the dentist were in the range of 1—2μg Hg m−3air when proper mercury hygiene measures were taken. This is far below the currently valid threshold limit value (30μg Hg m−3air) in Sweden. The saliva extractor and the dental mirror-evacuator did not influence the mercury vapor levels when used together with the high-volume evacuator. However, when only a saliva extractor was used, the cutting of amalgam fillings caused highly fluctuating mercury vapor levels, which were 2–15 times higher than the threshold limit value.
