The aim of the present study was to compare the immunofluorescence technique (IF) with the immunoenzymatic (IE) alkaline phosphatase-antialkaline phosphatase method for the evaluation of the presence of lymphoid antigens (Ag) in 46 cases of acute myeloid leukemia (AML). The first technique allows detection of Ag expressed on the cytoplasmic membrane of living cells, whilst the second shows the presence of intracytoplasmic Ag on fixed cells. In general, the percentages of lymphoid Ag expression on AML cells are relatively low with both IE (15.2%) and IF (17.4%). We found a good correlation between the two methods for CD2 (4/4), CD7 (4/5), CD20 (1/1) and CD4 (2/2). The Ag CD19, CD21 and CD8 were negative in all cases, both with IE and with IF. CD3 (2 cases) and CD22 (1 case) were only evident with IE. CD10 was seen in 1 case with IF, whilst it was found more frequently with IE. For this reason, demonstration of CD10 with IF is more specific for the classification of acute leukemia.

Friend erythroleukemia stem cells are strongly growth inhibited by 60 mM ethanol. The expression of this inhibition requires several days to develop fully, and is not relieved by washing into new ethanol-containing medium even in the presence of excess folic acid. Removal of the fully inhibited cells from ethanol results in rapid growth recovery, with the onset of recovery occurring within a few hours. Cell cycle analysis reveals a Gl delay which is evident within a few hours after initial ethanol exposure. Bivariate RNA-DNA analysis indicates that this Gl delay results from restriction in late Gl. It is unclear at present whether this delay can account for all the observed growth inhibition.

The relationship between chromosome breakpoints associated with T-cell antigen receptor (TCR) genes and TCR-α/β//τ//δ rearrangements of peripheral leukaemic cells in 8 Japanese patients with the acute type of adult T-cell leukaemia (ATL) was examined. Break of the 14qll region with the assigned locus of TCR-α//δ was revealed in 6 patients, interstitial deletion of the 7q32–36 region with assigned locus of TCR-β in 1 patient, and break of the 7pl5 region with assigned locus of TCR-τ in 2 patients. Molecular analysis revealed TCR-α rearrangement in 7 patients, TCR-β rearrangement in all patients, and TCR-τ rearrangement in 5 patients. TCR-δ was deleted in all patients. Leukocyte endothelial interactions are essential for a normal immune response. It is known that this response is influenced by stress and that the latter induces demargination. We examined the question of whether stress demargination results from a decreased state of leukocyte adhesiveness. Included were various volunteers and patients under different degrees of stress. 66 young athletes before beginning their daily exercises, 67 middle-aged healthy volunteers, 25 patients before ergometry for evaluation of chest pain, 75 patients who were referred to the emergency room with chest pain without ischemia/infarction, 78 patients with ischemia/infarction, 65 patients with minor trauma, 25 with a fracture and 12 with polytrauma. The leukocyte adhesiveness/aggregation (LAA) values were measured with a direct slide test. The respective LAA values were 7.4 ± 4.7, 6.3 ± 4.4, 5.8 ± 3.6, 5.2 ± 3.5, 10.8 ± 8.5, 9.1 ± 5.8, 12.2 ± 6.6 and 19 ± 12.6% of aggregated leukocytes. We conclude that an increase in aggregated white blood cells can be detected in the circulating pool during major stress. We analyzed the immunological characteristics of patients with idiopathic thrombocytopenic purpura (ITP) and antiglycoprotein (GP) IIb/IIIa or GPIb autoantibodies. Among 101 ITP patients, 32 had anti-GPIIb/IIIa and 19 had anti-GPIb autoantibodies. Thrombocytopenia was more severe in patients with anti-GPIb autoantibodies than in patients without these autoantibodies, whereas ITP patients with anti-GPIIb/IIIa autoantibodies did not develop severe thrombocytopenia. Patients with anti-GPIb autoantibodies showed significant increases of platelet-associated IgM and platelet-associated C3 in comparison with patients without the autoantibodies, despite there being no significant difference in the platelet-associated IgG levels. The lymphocyte subsets and the blastogenic response in patients with anti-GPIb autoantibodies were also significantly different from those in the patients without these autoantibodies. Furthermore, severe purpura and a poor response to prednisolone were far more common in the patients with anti-GPIb autoantibodies. Activation of the complement system and/or functional abnormalities of lymphocytes thus appear to be involved in the development of thrombocytopenia in ITP patients with anti-GPIb autoantibodies, and such antibodies may be associated with a particularly severe form of ITP.

Thirty-seven patients with adult T-cell leukemia had serum β2-microglobulin (β2M) estimated at presentation. A significant association was found between β2M and the subtypes of the disease: β2M in the acute type is significantly higher than in the chronic and smoldering types (p < 0.01), and β2M in the lymphoma type is apparently higher than in the chronic and smoldering types, although the difference between the lymphoma and chronic types was not statistically significant. An apparent reduction in β2M levels after chemotherapy was observed in 7 patients who reached partial or complete remission. One patient who was resistant to chemotherapy showed elevated β2M values even after intensive chemotherapy. In another patient, who attained partial remission with a concordant reduction in β2M, renewed elevation of β2M was observed in parallel with exacerbation of other parameters such as lactate dehydrogenase (LDH), WBC, calcium and clinical manifestations. Correlation analysis was carried out between β2M, LDH, calcium, lymphoid cell count and the grade of clinical severity. The β2M level was moderately correlated with LDH, calcium, the grade of clinical severity but not with the lymphoid cell count. The level of granulocyte-macrophage colony-stimulating factor (GM-CSF) in the cerebrospinal fluid from 14 infants and children with meningitis and 6 patients who suffered other diseases besides meningitis was measured by our sensitive enzyme linked immunosorbent assay for GM-CSF. The minimal detection level of GM-CSF was 40 pg/ml. Six of 9 patients (67%) with aseptic meningitis had detectable GM-CSF in cerebrospinal fluid and the concentrations of GM-CSF ranged from 49 to 114 pg/ml (mean 72 pg/ml), whereas none of 5 patients with bacterial meningitis or 6 patients with other diseases besides meningitis had detectable GM-CSF levels. There was no clear correlation between the GM-CSF levels in cerebrospinal fluid and the leukocyte count in either peripheral blood or cerebrospinal fluid, or the concentration of protein or glucose in cerebrospinal fluid.

We present an unusual case of light chain myeloma complicated by systemic amyloidosis. There was massive infiltration of the left iliopsoas and quadratus femoris muscles with amyloid, manifesting itself clinically as a progressive, painless limp. We point out that accurate diagnosis by muscle biopsy in such cases has important prognostic implications and may avoid the unnecessary use of potentially toxic and ineffective treatment.

A case of acute eosinophilic leukemia (EoL) that occurred in a patient with preexistent myelodysplastic syndrome is reported. The patient was initially diagnosed as having refractory anemia (RA) on the basis of pancytopenia with dysplasia and chromosomal abnormalities. Two years later, he was readmitted because of progression of pancytopenia, and bone marrow and peripheral blood showed immature dysplastic eosinophils. Clonal assay of peripheral blood mononuclear cells revealed autonomous growth of colony-forming unit eosinophils. Cytotoxic chemotherapy did not induce remission, and extensive myelofibrosis developed. Cytogenetic analysis in the RA state showed +1p- and -7 whereas complicated abnormalities including +1p-, 3q- and 7p- dominated in the EoL state.

A 23-year-old man with severe sickle cell disease and a chronic non-healing left leg ulcer was entered into a study in which he received hydroxyurea (HU) and recombinant human erythropoietin (r-HuEPO) in order to modify the severity of his disease. HU 20 mg/kg/day effectively prevented vaso-occlusive crises and improved hematological parameters, but the chronic leg ulcer and chronic mild bone pains remained. Six months later, r-HuEPO 600–800 IU/kg subcutaneously was added at weekly intervals. This led to a rapid and complete healing of the chronic leg ulcer, further improvement in hematological parameters and relief from chronic pains. We conclude that treatment with r-HuEPO may provide an effective therapy for chronic leg ulcers in patients with sickle cell diseas
