The frequency of chronic lymphocytic leukemia (CLL) varies a great deal from one population to another. We have undertaken to compare the aspects of CLL in Maghreb and Europe through two series of comparable importance, studied during the same period and under similar conditions in Algiers and Amiens. This comparison shows, in Algeria, a lower frequency, a very marked male predominance, a high proportion of patients with a normal or high rate of gammaglobulins and an almost complete lack of hypogammaglobulinemia. Demographic (compared structure of populations), technical (medical density, availability of laboratory examinations), and ethnic (rarity of CLL in some populations) factors are to be taken into consideration in explaining these differences.

Lymphocyte subpopulations were studied in 13 Sicilian glucose-6-phosphate dehydrogenase (G6PD)-deficient children during and after the hemolytic crisis due to fava bean ingestion. A statistically significant reduction of sheep-red-cell (SRC)-rosetting lymphocytes occurred in all patients during the hemolytic crisis, whereas B lymphocytes were not affected. In order to establish the possible relationship between serum changes occurring during hemolysis and reduction of SRC-rosetting lymphocytes, the effect in vitro of varying concentrations of bilirubin and hemoglobin on the rosette formation capacity of lymphocytes was studied. Both substances produced a statistically significant reduction of SRC-rosetting lymphocytes both in normal and deficient subjects. This effect showed a direct relationship with the concentrations of bilirubin and hemoglobin used and was more pronounced on lymphocytes from G6PD-deficient children. These data suggested that the reduction of SRC-rosetting lymphocytes observed during acute hemolysis of favism can be related to a modifying effect of bilirubin and hemoglobin.

In the diagnosis of multiple myeloma (MM), the radiological skeletal survey (RSS) was proven to be most useful for the detection of bone lesions. Since 1961, a new technique radioisotopic bone scan (RIBS), for the detection of such lesions, using 85Sr and 99mTc, has been shown to be highly sensitive for the detection of skeletal metastases of epithelial tumors. We have studied 30 patients with plasma cell dyscrasia (PCD) by both methods and concluded that RSS is clearly superior to RIBS in PCD. As RIBS detected less than 50% of the lesions demonstrated by RSS there seems to be no indication for a routine RIBS in the initial work-up of patients suspected to have PCD. However, due to the fact that RIBS is useful for the detection of new bone formation it has a certain value in the localization of pathological fractures in MM, mainly in vertebral compression.

Technetium-99m scintigraphy of the spleen was performed in 32 patients with various haematologic disorders prior to splenectomy. From the gamma camera image of the spleen, the weight of the spleen was estimated using two different mathematical formulas, and the results obtained were compared to the postoperative spleen weight. Both formulas revealed spleen weights showing highly significant correlations to the postoperative spleen weight and with similar standard deviations. Visual evaluation of the scintigram by an experienced examiner with classification of the spleens into four groups according to spleen weight revealed results as good as the mathematical calculations in predicting the postoperative spleen weight, both techniques giving diagnostic specificities of 0.70–0.80 for all groups and diagnostic sensitivities of 0.80–1.00. Physical examination of the abdomen showed that about 50% of the spleens weighing between 250 and 1,000 g were not palpable. Therefore, it is concluded that physical examination of the abdomen alone is unreliable. Successive chromatographic procedures made it possible to isolate thrombocytopoietin from the serum of thrombocytopenic rats. The following steps were taken: DEAE-cellulose phosphate chromatography, Sephadex chromatography, exclusion chromatography on DEAE-Sephadex A-50 gel. The apparent molecular weight of thrombocytopoietin was about 48,000 daltons. Like erythropoietin, thrombocytopoietin is a glycoprotein, its molecular weight is similar. A single form is obtained only when the different purification steps last less than 10 days. Beyond that time, partial degradation may occur.

37 type 2 diabetic patients with no clinical evidence of retinopathy or vascular disease were studied at diagnosis and following control of hyperglycaemia for evidence of abnormalities of coagulation, fibrinolysis and platelet behaviour. 38% showed hyperactive platelets, demonstrating either in vitro hyperaggregability, circulating platelet aggregates, or raised plasma beta-thromboglobulin levels. 36% showed abnormally raised factor VIII coagulant activity (FVIIIc) levels, though this was mainly in female patients. The mean level of FVIIIc decreased with treatment. Anti-thrombin III (AT-III) levels were decreased, and 33% of the patients had levels < 80%. In this group AT-III increased following treatment. No abnormalities of fibrinolysis were demonstrated. These findings support the concept that diabetes can be associated with a hypercoagulable state, which is not necessarily dependent on the presence of overt vascular disease, or correlated with the degree of chronic hyperglycaemia (HbA1c levels).

The influence of the hemoglobin (Hgb) phenotype on mean erythrocyte volume was re-examined in the light of recent evidence indicating a poor correlation between manual and more precise automated methods of measurement. The present studies identified a 17.6% frequency of microcytosis in a population of nonanemic Black males, a value higher than previously appreciated. In addition, it was shown that the frequency of microcytosis was also a function of the Hgb phenotype. Thus, 9.4% of Hgb A A, 18.6% of AS and 26.5% of AC subjects had microcytic erythrocytes. The data presented are consistent with the hypothesis that both the α-gene complement and the β-chain phenotype contribute to the erythrocytic mean corpuscular volume and that the frequency of microcytosis in a patient population is a function of both of these variable

25 patients with mixed autoimmune haemolysis are described; in these cases the combined criteria for warm antibody autoimmune haemolysis and also the cold agglutinin syndrome were present. The direct antiglobulin tests were positive with IgG and complement coating of the red cells. The IgM and IgG components could be separated. The cold autoantibodies were active at 30°C or above and often showed blood group specificity within the Ii system; specificity was not usually evident in the warm component. Haemolysins were found in 18 patients. Serologically, the condition maintained its mixed nature throughout; only 2 cases later changed to a warm type picture. Mixed autoimmune haemolysis was found in all age groups but was more frequent in later life; the male:female ratio was 1:1.5. The cases were either idiopathic (44%) or secondary; in the latter, systemic lupus erythematosus or lymphoma were the most frequently associated disorders. The patients tended to have servere haemolysis which, although usually responding well to treatment, ran a chronic course with intermittent exacerbations, thus making overall management difficult; plasma exchange may be a useful adjunct to chemotherapy

A 7-year-old boy with juvenile erythroleukemia is described, whose red cells demonstrated a high content of Hb F with fetal structure and yet contained carbonic anhydrase at adult red cell level. The findings seem to exemplify the occurrence of uncoordinated expression of fetal markers and consequently the incomplete reversion to fetal-type erythropoiesis in a hematologic malignancy.
