In 34 patients with chronic lymphatic leukaemia (CLL) the lymphocytes have been separated and sized using a C1000 Channelyzer. The modal volume and the volume range of the populations have been obtained and related to clinical stage and mouse red blood cell (MRBC) rosetting capacity. Over 1 year’s observation with several estimations per patient there was no convincing drift towards increase in modal volume with deteriorating clinical status. The cell size of the CLL populations could vary from time to time in the same patient. The MRBC rosetting capacity varied greatly between estimations. Natural killer (NK) activity was investigated in 16 patients with acute leukemia in complete remission and in 19 normal subjects. Peripheral blood lymphocytes from the great majority of the patients failed to kill the K562 target cell in a short-term chromium release assay. Nevertheless, the number of cells with NK membrane features (i.e. low-affinity receptors for sheep red blood cells) was close to the normal value. Good levels of NK activity were found in only 2 patients, namely those who had the longest complete remission in their cytological groups. NK activity was also tested in two pre-leukemic individuals and found normal.

CFU-c suppressor T cells were generated in vitro by culturing overnight peripheral blood T cells from healthy donors with pokeweed mitogen (PWM), or T cells from patients with severe aplastic anemia (SAA) in remission, with culture medium (RPMI). The supernatants were removed the next morning, the cells harvested and washed, and both tested for CFU-c suppression on normal marrow cells. Cyclosporin A (CyA) was added to this system to test whether it could abrogate or prevent the generation of suppressor cells. CyA was incubated with T cells, at a concentration of 0.1 μg/ ml, for 30 min at 37 °C and then washed away, in two different assays: (a) before T cells were incubated overnight in culture medium with or without PWM, or (b) after T cells had been kept overnight in culture. The results of this study indicate that CyA can prevent the generation of CFU-c suppressor T cells if preincubated with both normal or SAA T cells prior to in vitro priming, whereas it cannot abrogate the suppressor activity of primed T cell

Benign neutropenia is often found among healthy Yemenite Jews. An assessment was made of the magnitude of labor-induced leukocytosis in 44 Yemenite Jewish women, by comparing their hematological values during labor with those of 27 non-Yemenite women. A statistically significant difference was found in the absolute counts of the white blood cells (WBC) during delivery between the two groups, the values being lower among Yemenite Jews [10,291 ± 422] vs. [11,759 ± 630] × 109/1) and similar findings were detected among their infants. The low WBC counts in the Yemenite group at parturition were also associated with significantly lower plasma cortisol levels (Yemenite group: 33.7 ± 3.5 μg/dl; controls: 49.3 × 2.9 μg/dl). A correlation was found between serum cortisol levels and the magnitude of leukocytosis in both groups of women and their infants. These results suggest that a low basal corticosteroid output may contribute to the low white blood cell counts detected in some ethnic groups like the Yemenit

Data on 372 patients with thrombocytosis (TC), defined as a platelet count over 500 × 109/1, were accumulated in a non-biased fashion. Surgery, per se, was a common cause of TC and malignancy, if uncomplicated by infection, bleeding or surgery, was a less common cause of TC than suggested in previous surveys. The reasons for these apparent differences are discussed. Postoperative TC resolved within 30 days of surgery in the absence of complications. Transient reactive TC due to a variety of causes was accompanied by a significant fall in platelet volume in contrast to the megathrom-bocytosis found in postsplenectomy and myeloproliferative TC

During acute Plasmodium falciparum infection in man, plasma concentrations of platelet-specific proteins, β-thromboglobulin (βTG) and platelet factor 4 (PF4) were significantly elevated. For βTG, the mean concentration was 136.24 ± 71.58 ng/ml in patients, and 50.53 ± 25.42 ng/ml in control subjects (t = 5.3794; p = 0.0001), while for PF4 mean values were, respectively, 75.35 ± 23.09 and 18.64 ± 13.42 ng/ml (t = -6.0897; p < 0.0001). Platelet LDH loss in vitro in response to stimulation with 0.5 U of thrombin was 57.0 ± 29.5% in patient samples and 24.8 ± 16.9% (control); the values being significantly different from each other (t = 2.888; p < 0.025). With serotonin (5HT) uptake and release however, the values were essentially normal, in spite of marginal difference observed in the uptake value of patients compared to control. The data indicate that there was in vivo platelet activation during the infection, with the haemostatic balance tilted towards hypercoagulability. There was also associated easy platelet lysis with stimulation. It is also suggested that the latter finding may be one of the mechanisms of reduced circulating platelet numbers observed in patients in the acute

55 patients with iron deficiency anemia (IDA) and 55 age- and sex-matched control subjects were given exercise test on a treadmill to observe the effect on ST segment of the electrocardiogram. 14 IDA patients showed significant ST segment depression compared to only 1 in the control group, the difference being highly significant (p < 0.001). Test was repeated 2–3 days later in 12 IDA cases and it showed significant ST depression as on previous occasions showing the reproducibility of the results. 11 of these patients received total dose i.v. iron-dextran and the exercise test was repeated 2–3 days later before any significant rise in the hemoglobin level; in 10 cases there was no significant ST segment depression. Response to iron therapy was highly significant (p < 0.003). Erythrocytes containing abnormal haemoglobins with high affinity for red cell membrane are subjected to enhanced oxidant stress. Since HbS is known to have high affinity for red cell membrane and sickle cells are particularly susceptible to membrane lipid peroxidation, the behaviour of erythrocyte antioxidant system has been evaluated in 20 subjects, heterozygous for sickle cell anaemia. These subjects have shown normal levels of reduced glutathione, increased superoxide dismutase and glutathione peroxidase activities and low catalase activity. These data suggest that such an unbalanced antioxidant system can not prevent damage by the enhanced production of oxygen free radicals by membrane-bound HbS molecules.

A case of complete spontaneous remission of CLL in a 76-year-old woman is reported. Despite the complete remission, the study of the peripheral blood T lymphocytic subsets by the aid of monoclonal antibodies has showed, however, that the ratio between helper-induced and suppressor cytotoxic T cells is still impaired (0.81) as we can see in overt B-CLL. The cause of the remission and its possible connection with the imbalanced distribution of the T lymphocytic subsets is discussed.
