ABSTRACT.Telephone transmission of EEG was first demonstrated in 1971. The technique has proved to be a reliable way to obtain clinical information from rural hospitals. It must be recognized, however, that the validity of the data is only as good as the equipment and the personnel using it. Technologists at both the transmitting and the receiving sites must be well trained. This paper is a review of equipment, personnel, and philosophy involved in telephone transmission of EEGs.

ABSTRACT.The American EEG Society has published guidelines concerning clinical electroencephalography. Telephone EEGs should meet these guidelines, as well as the specifics devoted to the telephone transmission of EEG. At present, telephonic EEG is restricted by the frequency width on telephone lines. Usually only 8 channels of EEG can satisfactorily be transmitted.

ABSTRACT.Intensive monitoring (prolonged simultaneous EEG and video recording) has proved an effective means of establishing the diagnosis in patients with intractable epilepsy or enigmatic attacks and of localizing the epileptogenic focus in surgical candidates. Effective therapy depends on the correct seizure diagnosis. Numerous techniques are used to obtain data during long-term monitoring, which is now affordable and technically reliable. Intensive monitoring has greatly expanded our understanding of epilepsy and our ability to treat patients appropriately.

ABSTRACT.Visual stimuli of flickering geometric pattern and red flicker are generally very potent stimuli in triggering paroxysmal responses irrespective of using light stimulation with very low intensity, such as 10 nits. Application of these stimuli not only to the full field, but also to the center, periphery, and hemifield is described.

ABSTRACT.Although the conventional EEG has been unsatisfactory in the research of functional psychoses, it is widely used for detecting central nervous system organicity in psychiatric patients. The EEG is one of the popular laboratory tests in psychiatry, because it is safe and noninvasive, and considerably sensitive to cerebral organicity. Unfortunately, EEG reports are often ignored by psychiatrists who have ordered the EEG examinations.The knowledge of EEG changes caused by psychotropics is essential for the correct evaluation of EEGs because the majority of patients are receiving psychotropics. Several examples of EEG patterns seen in patients on psychotropics are shown. EEG studies of psychotropic complications (neuroleptic malignant syndrome, tardive dyskinesia and water intoxication) are reviewed. Recent advances of EEG study of mental disorder (e.g. suicide) and impacts of DSM-III on EEG research of mental disorder are discussed.

ABSTRACT.Maturation of the somatosensory nervous system comprises three interacting mechanisms: lengthening of the pathways, varying rates of myelination along the pathway, and increase in the number and efficacy of synapses. The shortening in latency as a result of increased conduction velocity is offset by the increase in length of the peripheral nerves and spinal cord. It is proposed that the latency changes seen for the various somatosensory evoked potential (SSEP) components reflect mainly the maturation of structures that are peripheral to their proposed generation sites. Corrections have been made for the effect of growth by calculating the latency per meter of arm length. The maturation of this normalized, effectively constant in size, somatosensory system is mainly the result of increased myelination (especially in peripheral nerves and spinal cord), increase in synaptic density (mainly in cortex), and in synaptic efficacy (spinal cord, brainstem, thalamocortical pathways). Each of these increases is assumed to result in an exponential decrease in latency of the evoked potentials toward the adult values but with different time constants (or specific maturation rates). These maturational processes proceed independently and thus in parallel. The resulting latency changes from all these maturational processes then can be described as the sum of decreasing exponentials. Two exponentials fit the data admirably; the time constants are in the range of Tm= 25−35 wk and T1around 5 yrs. A normative data set as a function of age for the median nerve N1, independent of the size of the subject and the type of equipment used, was constructed.
