摘要:
ABSTRACT.The clinical significance of spikes and sharp waves on neonatal recordings remains controversial. Artifacts, as well as maturational patterns, may appear sharply contoured and require careful review before assigning electrographic importance. Sharp wave phenomena of either negative or positive polarity are unique in this young age group and require clinical correlation. Periodic discharges also can occur, usually in the context of electrographic seizures and have the same significance as PLEDS in older patients. Longitudinal studies are still needed to correlate these epileptiform features with clinical examination, brain imaging, and neuro-developmental assessment at older ages.

ABSTRACT.Subdural electrodes in a variety of configurations have been utilized by a number of epilepsy centers in this and other countries. They have the advantage of allowing the sampling of cortical activity over a large area of the brain when needed. Functional localization can be performed by means of cortical stimulation. We discuss the advantages and drawbacks of this technique.

ABSTRACT.Somatosensory evoked potentials (SEPs) are widely used in the assessment of multiple sclerosis. Since other methods have been introduced recently, it is necessary to reevaluate the diagnostic power of the SEP method to allow for an easier selection of the set of tests used in the assessment of multiple sclerosis. It is worthwhile to emphasize that upper and lower limb SEPs are reliable, inexpensive, and not time consuming. Discomfort and risk for the patient are minimal. There are a few criteria that deserve special comment. 1) The sensitivity of SEPs in detecting a lesion in multiple sclerosis patients depends on the method applied and is in general lower than of magnetic resonance imaging, visual evoked potentials, and CSF tests, but higher than using brainstem auditory evoked potentials. 2) There is no specificity of SEPs. 3) In many instances it is possible to localize spinal and supraspinal subcortical lesions. 4) By proving normal sensory conduction or detecting clinically silent lesions, SEPs confirm a new classification in the follow-up. 5) There is a poor correlation of SEPs and somatosensory clinical findings, suggesting a nontrivial relation of evoked potentials and sensory information processing.

ABSTRACT.The P300 (P3) event-related brain potential (ERP) is sensitive to the cognitive changes resulting from normal aging and the mental decline associated with dementia and other cognitive disorders. The relationship between the P3 ERP and the cognitive dysfunction found in dementing illness is reviewed to provide context for discussion of the theoretical meaning, appropriate methods, and measurement of the P3 component in clinical populations. The utility of the P3 with respect to extraneous factors that can contribute to its normal variability also is examined. It is concluded that the P3 ERP can provide useful information about cognitive function in a clinical setting, but that task and biological factors must be considered in order to obtain more precise and interpretable results.
