摘要:
ABSTRACT.Because of the EEG's sensitivity to brain dysfunction, it lends itself to the examination of the following aspects of coma: the severity and course (improvement or worsening with time) of brain dysfunction, the diagnosis of certain conditions, and the prognosis (especially in conditions which produce permanent structural damage and death of cortical neurons). It is very important to perform serial EEGs to confirm the stability of the pattern and to note evolutionary changes. Although limited, the“anesthesia model”helps conceptually in dealing with a large variety of conditions which produce altered consciousness.

ABSTRACT.During the past ten years, computer technology has made its way into many facets of our lives. Until recently, however, computer technology had managed to elude the practice of clinical EEG. As instrument manufacturers increasingly use computerized systems and modular software package designs in their new product lines, the specialty of EEG is going to experience many benefits not offered by conventional EEG instruments. This paper addresses the basic concepts of digital EEG technology and the differences between digital and conventional instrumentation.

ABSTRACT.A simple introduction to the concept of fractal dimensions and their use in EEG long-term video-EEG monitoring is discussed. This method detects primary and secondary generalized seizures and distinguishes them from pseudoseizures and complex partial seizures without secondary generalization and purely artifactual activity. The fractal values of physiological artifacts, e.g., chewing, laughing, are different from those obtained from raw EEG but similar to seizure activity. However, when electrodes became dislodged during a seizure a high fractal dimension in excess of that used to trigger the seizure alarm occurred. Inspection of the video allows ready separation of artifactual activity in a nonseizure situation from that created by generalized seizures.

ABSTRACT.We investigated EEG phase-plane trajectories associated with the transition from the interictal state to clinical seizures. Our recordings were taken from scalp and invasive electrodes placed in epilepsy patients undergoing prolonged video/EEG monitoring. We systematically examined trajectories from 60 seizures in an attempt to determine criteria which may be used for classifying interictal and ictal patterns. These phase-plane trajectories emphasize the presence of repetitive waves, and clearly show some phase relationships between EEG channels. It is possible to follow the progress of seizure onset using these trajectories, and to define the time-course of the activity. We have found that phase-plane representations provide a different set of criteria for classifying and grouping EEG signals.

ABSTRACT.There are numerous factors in the operating room that can cause changes and/or artifacts while monitoring somatosensory evoked responses (Chiappa 1983, Nuwer 1986, York 1987). Drugs, anesthetic agents, irrigation, manipulation of the spinal column, changes in the patient's blood pressure and numerous other electrical equipment in the area can all cause changes in the evoked potentials. It is the responsibility of the EP technologist to determine the possible cause of changes when present. The technologist must know what factors can cause changes in the EPs and how to determine the source of artifact. The surgeon must be notified of any critical changes. Intraoperative monitoring can be less stressful and easier for the technologists involved if they are familiar with the surgical procedures, equipment, and medications.
