摘要:
ABSTRACT.Fixed procedures for recording somatosensory evoked potentials (SEPs) have developed more slowly than for visual (VEP) or brainstem auditory evoked potentials (BAEPs). This fact leads to much confusion for the technologist. About the time he begins to feel comfortable with a procedure, a“new”peak is identified, or a different montage is suggested, or he hears that tech“X”in the next hospital does everything differently—and he passed the evoked potential (EP) board exam! SEPs are not difficult to do and anyone can“get by”by following a strict recipe or protocol, but, for the technologist who wants to really understand the procedure, be able to set up an EP lab, teach or supervise others, and/or be a valuable asset to the interpreter, such a“recipe”is not enough. In this paper, I will begin with some technical considerations ranging from signal averaging to stimulating techniques. Then I will discuss the related anatomy.

ABSTRACT.Epilepsy and EEG were studied in 421 and 515 Vietnam veterans who suffered from penetrating head injury. The incidence of posttraumatic epilepsy and EEG abnormality was 53% and 42% respectively after an average follow-up period of 15 years. Among wound characteristics, brain volume loss, presence of early hematoma, and retained metal fragments correlated significantly with posttraumatic epilepsy (p<0.05). Frequency of seizures during the first year and extent of brain volume loss were important determinants of seizure persistence. Among the patients with abnormal EEG, 33% showed focal slowing (FS) and 9% had epileptiform findings (EF). Both FS and EF correlated significantly with brain volume loss, aphasia, organic mental disorder, hemiparesis, prolonged loss of consciousness, and posttraumatic epilepsy. Comparison of EF and FS groups showed that posttraumatic epilepsy and volume loss correlated with EF better than FS (p<0.05). All patients with anterior temporal and central spikes in EEG experienced posttraumatic seizures. Total or near total recovery from original hemiparesis was noted in only 32% of the patients with focal slowing and in 70% of the patients with normal EEC's.

ABSTRACT.The equivalent of epileptic EEG discharges on the cellular level are paroxysmal depolarization shifts (PDS). This paper reviews mechanisms involved in the initiation of epileptic discharges, PDS generation, transition from interictal to ictal discharges, and ictal epileptic activity, focussing on the role of calcium ions in these different steps of epileptogenesis. Intrinsic and synaptically elicited transmembranous calcium currents play a critical role in the regulation of neuronal excitability; large intracellular calcium accumulations as found during intense epileptic activity may even lead to cell death. Different approaches to the development of new antiepileptic drugs based on the mechanisms of action of calcium ions in epileptogenesis are discussed.
