ABSTRACT.During the past two years, we have recorded 45 neonates during extracorporeal membrane oxygenation therapy (ECMO). We have developed methods of minimizing artifacts and obtaining useful recordings in over 90% of cases. Adequate EEG recording of neonates during ECMO therapy is often difficult because of abundant artifacts secondary to the extensive supportive machinery and monitoring systems required for these critically ill patients. The integrity of the central nervous system (CNS) of these infants is often uncertain because of the sedative, paralyzing, and analgesic medications required. The EEG can provide a clue to CNS function. Our methods involve specific electrode application techniques, use of a modified placement and montage, and interruption of unessential electrical fuses and other equipment frequently present in ICU settings. These methods have enabled us to obtain records lengthy enough to include both active and quiet sleep and to eliminate or diminish most artifacts, including ECMO pump artifact.

ABSTRACT.Extracorporeal membrane oxygenation (ECMO) is a modified form of heart/lung bypass therapy utilized in the full-term infant with life-threatening respiratory failure. The risks of ECMO, including intracranial hemorrhages and their subsequent effects, make it important to check the condition of brain function before, during, and after ECMO therapy. Therefore, serial EEG recordings are necessary for these patients. Knowledge of EEG recording techniques in this population as well as neonatal respiratory physiology is a requirement for the neurodiagnostic technologist faced with recording EEGs from these infants.

ABSTRACT.The Rett syndrome is presumed to be a neurodegenerative disorder of childhood and adolescence occurring solely in girls. The EEG is almost invariably abnormal and frequently demonstrates prominent paroxysmal discharges. This finding corresponds with frequent occurrence of epileptic seizures. Spike activity is quite often accentuated over the central region and may be attenuated or blocked with passive hand movements. Slow spike-waves may also occur. Starting with the end of the first decade, the Rett syndrome tends to lose its progression and comes to a standstill. Epileptic seizures tend to abate while residual neurological damage (along with EEG slowing) persists. Sleep recordings also show unusual features with decline of physiological patterns and burst-like anterior delta waves.
