The increasing use of topical cyclosporin A in the treatment of corneal allograft reaction and of external immune disease has made it necessary to investigate its possible corneal epithelial toxicity. We performed masked, controlled rabbit studies in 40 eyes to compare the effect of topical application of 2% cyclosporin A, its solvent (castor oil), and balanced salt solution on reepithelialization of iodine vapor-produced epithelial defects and on subsequent maintenance of an intact corneal surface. The rate of defect closure was measured by image analysis of standardized photographs. Statistical analysis revealed no difference between groups in healing rates and in the amount of superficial punctate epithelial erosion for up to 6 days after reepithelialization; superficial punctate epithelial erosion was extremely rare in all groups from 6 days up to 8 weeks. Light microscopy, scanning electron microscopy, and transmission electron microscopy examination 12 days following defect closure showed no difference in epithelial and stromal morphology. Thus we were unable to demonstrate any corneal epithelial toxicity of 2% cyclosporin A.

摘要:
An overview of the corneal toxicity of various ophthalmic medications is presented. Glaucoma medications. local anesthetics, ophthalmic preservatives, anti-virals, corticosteroids, and sodium hyaluronate are discussed from a clinical point of view. Ocular medications can adversely affect the cornea both directly and indirectly, structurally and metabolically.

Corneal involvement associated with epidemic keratoconjunctivitis (EKC) usually occurs in the form of punctate epithelial and subepithelial infiltrates. We recently treated a patient with EKC who developed punctate epithelial keratitis, followed shortly thereafter by bilateral disciform keratitis. This keratitis produced a significant a decrease in visual acuity, but responded rapidly and dramatically to topical corticosteroid therapy. This patient had a history of bilateral filtration surgery and had bilateral thin-walled blebs through which virus may have entered the anterior chamber. It appears that adenovirus may be an occasional cause of disciform keratitis.
