A case of unilateral microphthalmos with a sizeable retrobulbar cyst, totally dysplastic retina, and gross nerve head coloboma is reported in an otherwise normal pigmented rabbit. Despite the gross abnormality of the posterior part of the globe, an iris sectoral coloboma, and abnormal lentoid body, the cornea was complete but had moderate edema and a reduced diameter. Scanning electron microscopy and morphometric analysis of the corneal endothelium showed a fully tessellated mosaic of cells that displayed only modest pleomorphism and polymegethism.

We proposed poly(2-hydroxyethyl methacrylate) (PHEMA) hydrogel sponges as potential materials for the peripheral zone of a composite keratoprosthesis. It was previously shown that such sponges allowed cellular invasion when implanted s.c. in rabbits. To evaluate the reaction elicited by these materials in the corneal tissue, a PHEMA sponge, with pore size diameters of 10-30 µm, was produced, impregnated with collagen, and implanted in the rabbit cornea in both central and limbal regions of separate eyes. The eyes were examined by slit-lamp biomicroscopy up to 6 months postoperatively. Enucleation was performed at 3 weeks, 6 weeks, and 6 months, then excised implants were examined by light and transmission electron microscopy. Both clinical and histopathological examination indicated that sponges were well tolerated by the stromal and limbal tissues. No capsule was formed around any of the implants. Corneal fibroblasts and capillaries readily invaded the internal voids in the sponge to all depths. The invading cells remained viable and active up to the end of the follow-up period. Our findings suggest that hydrophilic PHEMA sponges may be successful as peripheral keratoprosthetic materials able to provide a permanent and tight fusion of the keratoprosthesis with the host tissue.

We documented acute changes in corneal topography and refraction after readministration of topical steroids in five patients with myopic regression after photorefractive keratectomy (PRK). One was a high myope (−10.00 sphere); the others were moderate myopes (preoperative spherical equivalents −4.25 to −6). All patients had been off steroids for 3 to 8 months before reinstitution. The changes in refractive spherical equivalents ranged from + 0.75 to +3.00 diopters, and were observed from 6 days to 4 weeks later. Refractive hyperopic shifts were accompanied by pronounced corneal flattening as measured by computer-assisted corneal topographic analysis. Two to seven months later, three patients have become even less myopic, and two patients have again become slightly more myopic. Topical steroid use can modulate refractive changes after excimer laser treatments in some patients.

We report three patients undergoing keratoepithelioplasty using donor limbal tissue. All three had significant vascularization of the recipient bed and all three had inflammatory reactions with apparent partial replacement of the donor epithelium with host epithelium. Limbal transplantation, which is likely to include significant numbers of donor Langerhans' cells, may well increase the risk of graft failure in keratoepithelioplasty
