A total of 2,455 conjunctival lesions in adults (over 15 years old) obtained during a 61-year period were reviewed and histopathologically classified as congenital, acquired epithelial, acquired subepithelial, pigmented, inflammatory/degenerative, and miscellaneous. The most common lesions in decreasing order of frequency were: pterygium, nevus, dysplasia, nonspecific nongranulo- matous inflammation, and epithelial inclusion cyst. The most common conjunctival malignancy in adults was squamous cell carcinoma, followed by melanoma, and pagetoid change associated with sebaceous gland carcinoma. Squamous cell carcinomas arise from progressive stages of mild, moderate, and severe dysplasia. These tumors have a low malignant potential but may behave more aggressively in the spindle cell and mucoepider- moid variants. Melanomas arise de novo, from nevi or, most commonly, from acquired melanosis and have an associated 14–32% mortality rate by metastatic spread. Sebaceous carcinomas are often initially clinically mis-diagnosed, involve the conjunctiva by pagetoid spread, and can have a low rate of metastasis and death if diagnosed early and excised with frozen-section monitoring of the surgical margins.

Rabbit eyes submitted to alkali burns (in 1NNaOH, 7 mm, for 2 min) were evaluated as to the effect of contact lenses on the corneal reepithelialization. Forty eyes were used, divided into two groups: 3 weeks and those observed for 5 weeks. In each group, 10 eyes received a contact lens while 10 served as controls. From the daily evaluation of the corneal surface as well as the microscopic examinations at the end of the two periods of observation, we concluded that hydrophilic contact lenses provide a beneficial effect on alkali burns of the cornea, reducing the incidence of epithelial defects to statistically significant levels.

Thermal cauterization of the center of the rat cornea results in emigration of neutrophils into the extravascular limbal tissue and blood vessel growth into the cornea. In this study, 1.0Msodium salicylate, 1.0Msodium chloride, and ointment vehicle were administered to normal and cauterized rat corneas for periods of 6, 48, and 144 h. When applied to the normal cornea, salicylate resulted in a marked increase in neutrophils in the limbal tissue at 6 h, but an inhibition at 48 h. Similarly, for the cauterized corneas, administration of salicylate increased the extent of neutrophil emigration at 6 h, but this effect was not sustained at 48 h. Neither vehicle nor sodium chloride had any effect on the extravascular neutrophil population. After 6 days, administration of the vehicle resulted in a slight increase in vascular growth into the cornea, whereas sodium salicylate caused a decrease. These findings indicate that hypertonic (1M) sodium salicylate does not inhibit the emigration of neutrophils from limbal vessels of cauterized rat corneas, but does appear to have a cytotoxic effect on the tissues and on blood vessel endothelial cells.

Six eyes with persistent corneal epithelial defects of various etiologies were treated in a randomized, prospective, double-masked study to evaluate the effect of fibronectin on corneal epithelial wound healing. Study eyes received homologous human fibronectin (300 μg/ml), mixed with gentamicin and artificial tears, 4 times a day. Control eyes received the mixture of gentamicin and artificial tears. If no healing occurred after 2 weeks, the drug was unmasked and where control drops had been given, the mixture containing fibronectin was administered in open-label fashion. All 4 eyes in diabetic patients had successful epithelial healing when treated with the fibronectin solution. The 2 remaining corneas, which had defects secondary to a thermal injury and a presumed infectious etiology, did not heal with fibronectin.

A 20-year-old man with bilateral keratoconus underwent an epikeratoplasty in the left eye. The postoperative course was uneventful. Five years postepikeratoplasty he developed progressive myopia in the operated eye. A contact lens would not remain centered and the spectacle correction in a trial frame was not tolerated. A successful penetrating keratoplasty was performed six years after the epikeratoplasty procedure. Light and electron microscopic studies of the corneal button showed hypocellularity and irregular distribution of the keratocytes in the lenticule and the accumulation of a periodic acid-Schiff-positive, electron-dense fibrillogranular material at the interface between the lenticule and the recipient Bowman's layer.

A 3-year-old child with posttraumatic aphakia and a corneal scar was treated with a rotational penetrating ipsilateral autokeratoplasty followed by epikeratophakia 2 months later. His final best corrected visual acuity was 20/30, with 1.37D of corneal astigmatism. There were no significant operative or postoperative complications. We present a patient with a corneal ulcer due toListeria monocytogenes, which has not previously been considered to be a feature of human listeriosis. The ulcer responded to topical and subconjunctival gentamicin and cephaloridine. Subsequent management was complicated by the development of a fibrinous pupillary membrane leading to pupillary block requiring iridotomy and later vitrectomy with trabeculectomy.Listeria monocytogenesmay be confused with diphtheroid contaminants seen in corneal ulcer scrapings and is probably underreported as a cause for microbial keratitis.
