A unilaterally aphakic patient wore extended wear hydrogel contact lenses for optical correction for 3 years with excellent visual results. Toward the end of this period, a “mulberry” deposit appeared in a lens. Several months later, the mulberry was noted to have increased in size and a defect was found in the underlying cornea. Pathological examination of the lens showed a full thickness hour-glass shaped deposit that was contaminated with colonies of micro-organisms. With the exception of a small corneal scar, the patient had no permanent visual disability.

Precise cutting and matching of the corneal donor button to the recipient bed is fundamental in reducing postgraft astigmatism. Since the donor button is usually quite circular and of the intended diameter, the main problem is the creation of a round recipient bed. By varying suture placement and suture tension in the placement of Flieringa rings on human eye bank eyes, we were able to induce significant degrees of corneal astigmatism. Corneal curvature was measured by keratometry and corneoscopy before and after placement of a Flieringa ring. Trephination of these distorted corneas resulted in oval recipient beds.

Corneal graft specimens were obtained from two patients with Type I Hurler/Scheie systemic mucopolysaccharidosis. Light and transmission electron microscopy were performed. Numerous vacuoles containing a predominance of fibrillogranular and some multimembranous materials were noted in corneal epithelial cells, histiocytes, keratocytes, and endothelial cells. The clinical presentation and enzyme deficiency responsible for the syndrome are reviewed. Case reports and possible genetic mechanisms are presented.

Pellucid marginal corneal degeneration is a bilateral disorder characterized by thinning of the cornea in the inferior periphery. The ectatic cornea remains clear and develops high, irregular astigmatism. We report the clinicopathologic features of three cases and review the clinical, histopathologic, and electron microscopic features of previously reported cases. Pellucid marginal corneal degeneration may represent an eccentric form of keratoconus and, in the setting of healed hydrops, may resemble closely Terrien's marginal degeneration clinically and histopathologically
