摘要:
We studied the comparative effects of retrobulbar cyclosporin A (Cy-A) and topical prednisolone acetate 1% in prolonging graft survival in a single set,in vivorabbit model of corneal graft rejection. Forty animals received corneal allografts and were treated with either topical 1% prednisolone acetate, varying doses of retrobulbar Cy-A, or no immunosuppressives. Graft survival time was analyzed both from the day of operation as well as from the day of suture removal. Steroid-treated grafts vascularized more slowly than untreated grafts and grafts treated with Cy-A. When measured from the day of surgery, steroid-treated grafts survived longest. When compared, beginning with the day of suture removal, topical steroids and cyclosporin A (10 mg/kg) appeared equally effective in prolonging graft survival.

摘要:
We describe an experimental superficial traumaPseudomonaskeratitis model which is simply reproduced and mimics the human infection more closely than the intrastromal model. It is especially useful in studies of bacterial keratitis when therapy trials are undertaken and pharmacokinetics may be important, or where mechanisms of disease are being evaluated. The natural course of the untreated infection is described and correlated with light- and electron-microscopic observations.

摘要:
A 68-year-old man had shingles of the first and second divisions of the right trigeminal nerve. During the acute stage, the right cornea was affected by a localized and transient keratitis. However, excessive lacrimation has persisted for 2 years and appears to be due to corneal hyperesthesia.

We removed previously implanted convex hydrophilic lenses of 1/2–55% hydration from six eyes in six baboons 3–6 months following initial implantation, using standard keratophakia techniques. Subsequently, we replaced three of the lenticles with convex or concave lenticles. There were no surgical complications during lens removal or lens replacement. The implanted low-water-content lenticles were not tolerated by the corneas and had to be removed, whereas the hydrophilic lenticles which were reimplanted were well tolerated. The excised hydrophilic lenses were structurally intact; only the low-water-content lenses were coated with adherent keratocytes and collagenous material. Following lenticle removal, the recipient corneas returned to preoperative parameters. This study demonstrates the technical feasibility of intrastromal lens removal and/or exchange for the modification of refractive errors.

摘要:
The microkeratome designed by Jose Barraquer for use in keratomileusis and keratophakia was redesigned for use with the small non-human primate eye. This instrument gave accurate, reproducible lamellar sections that were within 0.03 mm of the predicted measurement. The ability to perform accurate anterior keratectomy is critical to the development of hydrogel keratophakia.

摘要:
Map topography of the cornea allows the surgeon to determine discrete areas of the corneal topography which contribute to astigmatism or refractive error. Microsurgical repair of these disparate corneal areas may be documented quantitatively. We present a method of corneal topography analysis using the corneascope and high resolution digitization analysis of the resultant photographs. Cold-plated steel balls of known radius of curvature are the calibration standards to develop the linear regression analysis presented. Data storage, acquisition, and comparison to subsequent photographs to determine minute topographical shifts is greatly facilitated by this new technique.

Suppurative keratitis due toStaphylococcus aureusoccurred in a myopic female with atopic dermatitis 10 days after radial keratotomy. A small perforation in the infected radial incision reopened during antibacterial therapy and was repaired with cyanoacrylate adhesive. Residual corneal scarring produced with-the-rule astigmatism, but uncorrected visual acuity improved to 20/25.

摘要:
A corneal button from a 17-year-old woman with clinically known herpes simplex stromal keratitis, which later perforated when she was off therapy, was examined. A viral culture from the corneal surface was negative. Light microscopy revealed both acute and chronic inflammation, with multinucleated giant cells at Des-cemet's membrane. Transmission electron microscopy revealed active lysosomal activity within the polymor-phonuclear leukocytes in areas of collagen degradation, and herpes simplex viral particles at various stages of development in keratocytes undergoing lysis. Immunoperoxidase staining showed HSV antigen present in epithelium adjacent to the ulceration. We believe that the pathogenesis of stromal edema and necrosis in HSV stromal keratitis is multifactorial and involves both active viral infection and the host response.
