Rifampicin – a Leading Drug in the Therapy of Tuberculosis not Easily Amenable to Treatment |

Twenty-six patients suffering from pulmonary tuberculosis, not easily amenable to treatment, were treated with Rifampicin (RMP) and ethambutol (EMB); in some another effective drug was added. As not easily amenable to treatment were classified patients in whom intolerance to drugs and associated affections precluded the administration of a therapeutical effective combination of the currently available first and second line drugs. In all patients positive on smears and/or cultures it was possible to obtain within 6 months a conversion to negative findings, which persists in 15 patients for a mean period of 20 months. The treatment had to be interrupted in two cases. In one patient with liver disease because of the increasing impairment of hepatic functions and in another suffering from cirrhosis of the liver because of digestive disorders. In the remaining patients, including further 9 patients with hepatic disorders, RMP was well tolerated and did not reveal any side effects. The results obtained are compared with those observed in another group of patients with resistant tubercle bacilli who were treated with a triple combination of second line drugs. The occurrence of side-effects after the administration of RMP and after other antituberculous drugs is compared as well.

Controlled cooperative trials need certain methodical suppositions: (1) exact declaration of methods; (2) the least to be aimed for in a controlled cooperative study is indication of the effectiveness of treatment (single or combined therapy), as well as an indication of sideeffects; (3) prospective planning of the trial; (4) need of a control group of patients; (5) random allocation of patients; (6) pair matching of patients; (7) exact criteria for exclusion of patients; (8) quantified criteria of treatment results; (9) equal treatment of any patient in each group; (10) significancy tests of treatment results.
