SummaryForty patients treated with removable partial dentures (RPDs) at the Institute of Dentistry, University of Oulu, were selected for this study. The type of occlusion, horizontal and vertical overlap, the slide between retruded contact position and intercuspal position, and the occlusal interferences were investigated as occlusal parameters. Helkimo's anamnestic and clinical indices were used to measure the craniomandibular function and the clinical index score was used to compare the differences between different groups.The RPD‐generated occlusal interferences seemed to cause most harmful effects in point centric, interference free occlusions, when the damage was directed at the temporomandibular joint. These interferences were most frequently found in the retruded contact position and caused by the RPD teet

SummaryAn electromyographic study of the masseter and anterior temporal muscles together with questioning patients regarding subjective symptoms was performed on 15 patients with myofascial pain dysfunction (MPD) syndrome prior to and during bite plate therapy.The electromyographic study was evaluated by the slopes of the voltage tension (V/T) curves (calculated as the value of the relative inclination between the integrated electromyram and the biting force). The improvement of subjective symptoms was recognized earlier than the relief of objective symptoms by bite plate therapy. The results confirm that judgement of level of improvement following treatment of MPD syndrome is more appropriate through objective evaluation. Follow‐up recording of the voltage tension can be considered to be an objective indicator for patients with MPD syndrom

SummaryThe aims of the present study were: (i) to identify the association of patterns of congenitally missing teeth with combinations of ectodermal symptoms occurring in patients with oligodontia; and (ii) to propose a diagnostic scheme for the general practitioner.For this study 167 patients with oligodontia, both isolated and as part of a syndrome, and 135 healthy controls were interviewed and documented. Chi‐square tests, logistic regression and correspondence analysis were used to evaluate and test differences between the groups and associations between the congenitally missing teeth and ectodermal symptoms.No significant differences were found between the control group and the patients with isolated oligodontia with exception of the skin. It could be concluded from the present study that there were no clear associations between congenitally missing teeth, either individually or patterns, and the ectodermal symptoms or combinations of ectodermal symptoms.However, it could be concluded that if the most stable teeth are missing, or if the number of missing teeth is large the patient should be examined carefully for symptoms of ectodermal dysplasia.Using logistic regression a patient could be classified as having isolated oligodontia or oligodontia as part of a syndrome with a specificity and sensitivity of 88.2

SummaryIn order to clarify the basic histopathological effect of a denture prosthesis on the denture supporting tissues, a new experimental appliance was developed. The denture base was designed to make contact with palatal mucosa of the experimental animal without any mechanical pressure and transmit no occlusal force to the mucosa. The objectives were: (i) non‐pressure contact of denture base with palatal mucosa; and (ii) lack of physiological stimulation for palatal tissue by tongue and/or food.The results of the study revealed that covering the palatal mucosa with a denture base without any mechanical pressure and with the maintenance of a clean palate and denture base, did not cause any inflammatory change and bone resorption in the denture supporting tissues, except for a slight transient proliferative reaction in the epithelial ridge

SummaryInterexaminer reliability is defined as the degree of consistency among examiners when making observations of the same clinical variable. In the present study, the interexaminer reliability of six orthopaedic tests was determined in a group of 79 patients with signs and/or symptoms of craniomandibular disorders (CMD), subdivided into three subgroups of patients with a mainly myogenous, a mainly arthrogenous, and a combined myogenous and arthrogenous disorder. Multi‐test scores were composed for each test and combinations of tests for the three main symptoms of CMD, viz. pain, joint noises and restriction of movement. Although the orthopaedic tests showed different reliability scores, overall reliability of the determination of these three main symptoms of CMD was satisfactory.In the subgroups, arthrogenous signs and symptoms could be determined reliably with the set of six tests, whereas the reliability of the tests in determining pain and joint noises in the myogenous group was rather low. It may be concluded that the tests are well suited to evaluate arthrogenous signs and symptoms, but that the clinician should be aware of erroneous results of the tests in evaluating pain of a myogenous origi

SummaryAn investigation was made of the reflex responses evoked in the human masseter by periodontal mechanoreceptors. Weak taps were applied to the labial surface of a central incisor tooth by an electromechanical stimulator with a flexible probe (von Frey hair). Forces as low as 0.2N evoked inhibitory reflex responses in the surface electromyograms of both masseter muscles. These reflexes were modulated to markedly different extents in different subjects by auditory white noise, which always reduced the amplitude of the inhibition. The reflexes were abolished when local anaesthesia was infiltrated around the stimulated tooth and white noise was played into the ears. Evidence is presented that the sound of the tap on the tooth, transmitted through the air, is in itself sufficient to evoke an inhibitory reflex in masseter which is qualitatively similar to that from the periodontal receptors. Thus, in the absence of auditory masking, the total reflex evoked by tooth taps is the result of the summation of the inputs from both periodontal mechanoreceptors and auditory receptors.

SummaryTotal discectomy was performed experimentally in the rabbit temporomandibular joint (TMJ). In order to evaluate the injury and repair of the articular cartilage of the mandibular condyle after discectomy, both histologic and immunohistochemical observations were made. Immunohistochemical observation by using bromodeoxyuridine (BrdU) and its monoclonal antibodyin vivolabelling method was used.The present study demonstrated that the cartilage on the articular surface of the condyle disappeared at 1 week post‐operatively. This defect in the injured articular cartilage was not repaired by the cartilage, but was covered by a fibrous connective tissue at 6 weeks post‐operatively, where the condylar recontouring by the adjacent articular cartilage, and flattening and hyperplasia of the condyle were observed. SummaryThe aim of this investigation was to study the characteristics of hyperalgesia and inflammatory change in the craniofacial area following the production of an acute arthritis of the temporomandibular joint (TMJ) in the rabbit. Unilateral arthritis was produced by the injection of monosodium urate (MSU) into the TMJ of the rabbits. Hyperalgesia was evaluated by the reduction in the threshold of the head‐withdrawal response to pressure stimulation, and inflammatory change was determined by the quantitative Evans‐blue test and light‐microscopic examination of histological sections. Several hours after an injection of MSU (0.15, 0.5, 1.5 mg), hyperalgesic changes were observed over not only the MSU‐administered condyle, but also over the ipsilateral masseter muscle and temporal muscle in a dose dependent manner, and lasted for more than 24 h before gradual recovery. The other craniofacial region showed no significant threshold‐reduction. Although a very intense inflammatory sign was found in the region of the MSU‐treated TMJ, a decreased or negative sign was found in the masseter muscle with a very low threshold. SummaryThe aim of the present study was to determine changes in function of the masticatory system in children with articulatory speech disorders and in the controls by re‐examining a sample of 287 previously studied children from 7 to 10 years of age. Articulatory speech disorders were diagnosed by the same speech therapist during spontaneous speech and test words using the Remes Articulation Test. Mandibular movement capacity, occlusal interferences, slide between retruded and intercuspal positions, and signs and symptoms of the craniomandibular disorders (CMD) were recorded by a dentist.The follow‐up results showed that mandibular movement capacity increased in almost all individuals, decreasing only in about 10% of cases. However, among children with speech disorders lateral jaw movements decreased in about 30% of subjects. During growth the prevalence of CMD increased while single signs and symptoms fluctuated. Differences between the groups for expression of CMD, mandibular movement capacity and occlusal recordings mainly levelled off. Higher constancy of subjective symptoms and retrusive interferences in children with speech disorders than in the controls supports our previous findings that, in growing individuals, different orofacial dysfunctions seem to be associated with each ot

SummaryThe activity of jaw elevator muscles (masseter and anterior temporal muscle) was studied by surface electromyography in 17 patients wearing complete dentures with anterior‐canine guidance.Baseline electromyography was recorded with the dentures and compared to recordings obtained with two different splints adapted to the upper denture, one providing anterior guidance leading to immediate disclusion of the posterior teeth during any jaw movement, the other providing posterior guidance buccally on the working side and lingually on the non‐working side (bilaterally balanced occlusion).Recordings were obtained during a sequence of different jaw movements: postural position, maximal tooth contact in intercuspal position, protrusive movement of the mandible (under tooth contact) and during lateral excursions, also carried out under tooth contact.Muscle activity was not significantly different as recorded with dentures only versus with the splints providing anterior guidance.
