The findings from immunofluorescent techniques have made a profound contribution to our knowledge of bullous discases. The techniques, once considered investigational, are now routinely employed in dermatologic diagnostic tests. Direct immunofluorescent methods applied to specimens of perilesional skin are extremely reliable to demonstrate characteristic deposition of various immunoreactants in pemphigus vulgaris, bullous pemphigoid, and dermatitis herpetiformis. Autoantibodies in serum revealed by indirect immunofluorescent techniques, are demonstrable in most cases of pemphigus vulgaris and bulious pemphigoid. The disappearance of these antibodies from the skin and serum of patients so affiicted appears to herald remission of their disceases. Thus, detection of these antibodies may be employed to regulate therapy for maximum effect.

A 68-year-old black woman with systemic lupus erythematosus being managed by systemic corticosteroids developed large ulcerated lesions in a dermatomal distribution on the flank and abdomen. Subsequently, large ulcers developed progressively in the perianal region, the buttocks, the perivaginal region, and the thighs. The clinical diagnosis was vasculitis of systemic lupus erythematosus. Biopsy from the margin of an ulcer showed changes in the epidermis that are specific for infections by herpesvirus. Electron microscopy revealed viral particles of herpesvirus. Cultures from the perianal lesions grewHerpesvirus hominis. This is the first report to our knowledge of “chronic cutaneous herpes infection‘’ in a patient with systemic lupus erythematosus.

A case is reported of deep cutancous mycosis in an immunosuppressed patient who, after heart transplantation, developed a solitary blue-black skin nodule that was excised. No cultures were taken. The nodule, in the dermis, was a granuloma with microabscesses containing a few pale yellowish-brown fungal spores that were found after much searching in hematoxylin- and eosinstained sectios. Stains for melanin showed numerous spores and septate branching hyphac that also stained with P.A.S. Mclanin staining indicated that the fungi were dematiaceous and that the lesion was a phaeomycotic cyst. Staining of stock cultures of fungi for melanin showed good correlation between the amount of pigment in cell walls of fungi and the accentuation of staining when a melanin stain was incorporated.

Monoclonal antibodies have been used to characterize subpopulations of lymphocytes in a variety of dermatologic conditions by immunofluorescence and immunoperoxidase staining. Various clinical presentations of cutaneous T-cell lymphoma have been shown to be neoplastic expansions of helper T-lymphocytes. Immunoperoxidase studies using subset-specific monoclonal antibodies have allowed the differentiation of patients with cutaneous T-cell lymphoma from lymphocytoma cutis and B-cell lymphomas secondarily involving the epidermis.Increased numbers of OKT-6 positive Langerhans' cells in the dermal infiltrates of patients with cutancous T-cell lymphoma may be another immunocytological marker of this disease.Studies done on circulating lymphocytes of patients with psoriasis and pemphigus vulgaris have shown normal ratios of T-cell subsets in patients with psoriasis and an expansion of the helper T-cell subset in an untreated patient with pemphigus.Immunofluorescence and immunoperoxidase teachniques used in conjunction with highly specific monoclonal antibodies have been shown to be valuable for histopathologic study of dermatologic discases.
