Studies of the skin of two siblings with the Sjögren-Larsson syndrome by electron microscopy revealed within keratinocytes prominent Golgi apparatus and increased numbers of mitochondria. Laminated membranous figures which resembled cementsomes were present in cornified cells. These changes indicate that cutaneous lesions in Sjögren-Larsson syndrome may have distinct electron-microscopic changes.

In most of the few reports about the histopathology of acrodermatitis enteropathica, the changes have been considered to be nonspecific. We undertook a prospective study of 12 cases of acrodermatitis enteropathica in order to assess better the clinical and histologic features of the cutancous lesions as thev evolved over time.Early lesions of acrodermatitis enteropathica are characterized by loss of the granular layer, replacement of this layer by clear cells, and focal parakeratosis. As the condition progresses, the epidermis becomes increasingly psoriasiform, the parakeratosis becomes more confluent, and the pallor of the upper part of the epidermis becomes more prominent. Still later, the pallor disappears, but the psoriasiform hyperpiasia persists. As the lesions involute, only parakeratosis remains. Dilation and tortuosity of the vessels in the papillary dermis, reminiscent of those in psoriasis, is present throughout most of the course of the process.A histologic diagnosis of acrodermatitis enteropathica (and other diseases of zine deficiency) may be made on the basis of the histopathological changes.

Despite its common use for centuries, the term eczema has never been defined in a repeatable way. Although there have been innumerable attempts to do that, the word eczema still lacks specific meaning. Dermatology and dermatopathology can come of age only when the word eczema is no longer used.
