Fourteen patients with Lyme disease showed typical clinical features of erythema chronicum migrans. Eighteen biopsy specimens in all were obtained from the cutaneous lesions of these patients. The predominant histologic finding was a superficial and deep perivascular and interstitial infiltrate composed mostly of lymphocytes, or lymphocytes and either plasma cells and eosinophils or both. The plasma cells were found most frequently in biopsy specimens taken from the peripheries of the lesions, whereas eosinophils were found mostly in the centers of lesions. With the Warthin-Starry silver stain, spirochetes were found in six biopsy specimens from four patients in either the dermis, the epidermis, or both. The spirochetes were found mostly at the borders of the lesions and in biopsy specimens containing plasma cells. Spirochetes were subsequently cultured from a typical skin lesion of erythema chronicum migrans. These findings corroborate previous indirect evidence that a spirochete might be the cause of Lyme disease.

A lesion of erythema chronicum migrans was examined by light and electron microscopy. Spirochetes were identified by light microscopy with Steiner silver stain. Electron microscopy was performed on deparaffinized tissue of the biopsy specimen and a spirochete was identified in the tissue. This treponema-like spirochete has morphological characteristics similar to those described previously for spirochetes obtained from ticks in areas endemic for Lyme disease.

For the purpose of diagnosing and staging cutaneous T-cell lymphoma, the nuclear contour index and subsets of T-cells in the lesions were studied. Nuclear contour index measured in T-cells identified precisely with monoclonal pan-T antibody was found to be higher than nuclear contour index measured in T-cell-like cells with significant nuclear convolution. Nuclear contour index increased as the disease progressed from erythematous to tumor stage or to the Sezary syndrome. The relative ratio of supressor cytotoxic T-cells to helper T-cells was higher in the early rather than late stage of cutaneous T-cell lymphoma.

This is a report of the finding of a T-cell subpopulation bearing T-helper cells and la antigens in specimens of skin from patients with mycosis fungoides and the Sézary syndrome. Frozen sections of skin tissue from eight patients examined with monoclonal antibodies against mature T-cells, helper T-cells, suppressor T-cells, and la antigens exhibited similar staining patterns by a modified immunoperoxidase method. Antibodies against mature T-cells and helper T-cells stained 70–80% of the lymphocytes in the dermis. The antibody defining the phenotype of suppressor T-cells labelled 5–10% of the lymphocytes scattered throughout the lesions. Eighty to 90% of the lymphocytes took the stain for la antigen. Anti-thymocyte antibody (OKT6) stained cells in both the epidermis and dermis of the specimens. Of nonmalignant conditions examined, lesions from five cases of lichen planus exhibited a quantitatively different staining pattern than that of mycosis fungoides in that the number of T-helper cells was about equal to the number of T-suppressor cells. The findings reported are evidence for a homogeneous population of T-helper, la-positive lymphocytes in the cutaneous lesions of mycosis fungoides and the Sézary syndrome.

This is a 14-year follow-up of a patient with granulomatous mycosis fungoides. The patient has had nodular lesions of mycosis fungoides for more than 30 years, an unusually long survival for the nodular form of the disease that is often quickly fatal. The role of the granulomatous inflammatory response in his survival is speculated upon.

We report the clinical, histopathological, and ultrastructural features of a case of Woringer-Kolopp disease, review the literature, and discuss the relationship of the condition to mycosis fungoides. At the present time it is best to regard Woringer-Kolopp disease as a form of cutaneous T-cell lymphoma that may remain localized for many years or may eventually be associated with widespread mycosis fungoides.
