Seven cases of spindle cell hemangioendothelioma, a recently described vascular neoplasm, are presented. The lesions were multiple in five of the seven cases and primarily involved the upper extremities, particularly the fingers. On microscopical examination, the tumors were composed of two elements: cavernous spaces and spindled cells. In addition, clusters of histiocytoid endothelial cells, some with intracytoplasmic vacuoles, were present. The clinical behavior is marked by numerous local recurrences and slow but steady progression of the disease.

The clinical and pathological features of 29 cellular blue nevi (CBN) and one malignant cellular blue nevus from our hospital files were reviewed. Although the sacrococ-cygeal region/buttock was the commonest single site, the majority of CBN occurred on the limb peripheries. Two-thirds of patients were under the age of 40 years. Follow-up of a mean of 7 years did not reveal any evidence of malignancy. Four histological subtypes were recognized: mixed biphasic, alveolar, fascicular or neuronevoid, and atypical varieties. One case developed a benign nodal metastasis. In one case, malignancy arose within a CBN. The importance of recognizing the variety of patterns, the benign behavior of the atypical variety, and the criteria for malignancy are herein discussed.

Nine formalin-fixed paraffin sections of skin biopsy specimens of patients with clinically and histologically proven dermatomyositis were studied using the avidin-biotin immunoperoxidase technique. No evidence of dermoepi-dermal junction or perivascular IgG, IgA, or IgM deposits were noted. These findings may be related to loss of antigenicity during the fixation process or to the small amount of immunoglobulin deposit previously reported in dermatomyositis. The negative findings demonstrate an inability to use this technique to find immmunoglobulin deposition retrospectively in dermatomyositis.

We have reviewed the clinical and histological presentations of six cases of lipoid proteinosis. Deposits in the dermis were heterogeneous in nature and location. In the first stage of the disease's evolution, basement membrane components accumulate at the normal site of vascular, muscular, and epithelial basement membranes. Later on, lipids and amorphous material fill the superficial dermis, pushing apart the normal collagen bundles. Such deposits seem to be organized around preserved reticulin fibers. The amorphous material did not react with antibodies to collagen types I, III, IV, and V.

Two patients with Darier's disease—diagnosed clinically by the presence of characteristic keratotic papules in seborrheic areas—were biopsied for light- and electron-microscopic studies. Light microscopy revealed typical histologic features of Darier's disease. Ultrastructural studies showed the presence of cytoplasmic processes projecting from the basal keratinocytes (basal keratino-cyte herniations) into the underyling dermis through small defects in the basal lamina, which was intact in some areas but in other locations showed thickening, irregularity, and replication. Widening of the lamina lucida with partial or complete disappearance of hemidesmo-somes were noted in many areas.

Sebocrine adenoma is a benign cutaneous adnexal neoplasm differentiating in the direction of sebaceous and apocrine glands. The sebaceous differentiation is characterized by solitary instances or clusters of sebocytes and sebaceous ducts. The apocrine differentiation is characterized by eccrine poroma–like histology. Reported herein are three such cases.

This article reviews a group of skin disorders that have in common the presence of a cellular infiltrate with plasma cells upon histopathologic examination. These include inflammatory and neoplastic diseases and a group of miscellaneous disorders of varied etiology. The correlation between the presence of plasma cells in the cellular infiltrate and the diagnosis, prognosis, and pathogenesis of the disorders is emphasized.

We report the case of an ulcerated malignant cutaneous mixed tumor occurring on the nose of a 75-year-old woman. Malignant cutaneous mixed tumor (CMT) is a rare neoplasm. Of the 14 previously reported cases, 10 occurred on the extremities, three on the trunk, and one on the scalp. This contrasts with the benign CMT which is found mainly on the head and neck region, the nose being the most commonly affected site. Although eight of the cases of malignant CMT recurred and 10 metasta-sized, the primary lesion never ulcerated.

We describe an unusual malignant cutaneous neoplasm having a biphasic growth pattern and apparently arising in a mixed tumor. A lymph node metastasis was similar histologically to the primary lesion, exhibiting both carcinomatous differentiation and areas that appeared sarco-matous. Immunohistochemical stains showed selective staining for high molecular weight keratin in carcinomatous areas and for vimentin in areas having a sarco-matous appearance. The literature of cutaneous malignant mixed tumor is reviewed and possible explanations for the unusual morphology and immunohistochemical staining pattern are discussed.

A case of clinically appparent angiokeratoma corporis diffusum (Fabry's disease) in an adult female carrier is presented. The patient had biochemical evidence of the disease, and showed multiple cutaneous lesions, in the absence of other major organ involvement. Ultrastructural examination of tissue fragments obtained by skin biopsies demonstrated the presence of a few typical electron-dense lamellar structures in endothelial cells, but not in smooth muscle cells. Electron microscopy proved to be the only effective way of detecting the intracyto-plasmic inclusions, since light-microscopic histochemistry failed to reveal small amounts of the stored glycolipid. The exclusive involvement of endothelial cells suggests that they are more prone to store glycolipid than are all other types of cells usually involved in the disease. The abundance of intermediate filaments in the cytoplasm of endothelial cells is related exclusively to the high blood pressure in angiomatous arterovenous shunts.
