Since 1985 when parathyroid subtraction scintigraphy was presented in atlas form in this journal, the T1-201-Tc- 99m computer subtraction studies have become more and more popular. Although the study has been quite accurate in localizing abnormal parathyroid pathology, the rate of false positive studies has been somewhat excessive, usually due to thyroid pathology. This atlas is an update of that previous work with the goal of presenting a scheme of interpretation that lessens the incidence of false positive studies as well as presenting many of the pitfalls in diagnosis of parathyroid disease.

A 12-day-old girl was evaluated with a Meckel's scan (Pentagastrin 8 µg followed by 1.57 millicuries of Tc-99m pertechnetate) for bloody stools. Increased Tc-99m breast uptake was noted bilaterally. Increased uptake has been documented in lactating breasts as well as in hormonally stimulated non-lactiferous states from exogenous estrogen administration or amenorrheagalactorrhea. Uptake of Tc-99m in normal nonstimulated breasts does not seem to occur. Physiologic gynecomastia of the neonate appears responsible for neonatal breast uptake, a phenomenon that may cause confusion in imaging interpretation in patients of this age group.

A bone image of a hypercalcemic patient demonstrated multiple areas of soft tissue uptake. A follow-up image after medical treatment revealed a decrease in the degree of uptake when the serum calcium level returned to normal. Non-osseous uptake of bone-imaging agents occurs in a wide variety of benign and malignant disorders. The presumed mechanism is elevation of the product of the tissue calcium and phosphate ion concentrations, leading to calcium deposition. This case is of interest in that the reversibility of tracer uptake corresponding to the lower serum calcium level is demonstrated.

The uptake of lndium-111 labeled WBCs in the reticuloendothelial system and inflammatory sites is well established. A case is presented in which a peculiar-appearing inferior right hepatic lobe was believed to be a subhepatic abscess in a patient with the appropriate symptomatology. A hepatobiliary scan firmly resolved the issue.

Intravenous administration of contrast media and oral ingestion of iodide have been suggested as possible causes for sialadenites, which are thus sometimes referred to as “iodine mumps.” Ga-67 uptake has not been described in such cases. However, uptake has been reported in the lungs and lymph nodes following lymphangiography, probably resulting from the inflammatory reaction to the contrast material in the pulmonary parenchyma and the lymph nodes. A case is presented with unilateral Ga-67 uptake in a submandibular salivary gland five days following sialography. It is therefore recommended to perform Ga-67 imaging in the evaluation of salivary gland pathology prior to sialography in order to avoid false-positive increased Ga-67 uptake following the procedure.

This case illustrates peripheral enhancement in a hepatic hemangioma demonstrated in a Technetium-99m diethylenetriaminepentaacetic acid (Tc-99m DTPA) image. Peripheral enhancement in hemangiomas has previously been reported in Tc-99m labeled RBC studies, in computed tomography images, and in ultrasonography (1-4), but, to our knowledge, not in extrarenal uptake of Tc-99m DTPA (5). A 78-year-old, white woman was worked-up for hematuria in 1987. A renal image with Tc-99m DTPA showed uptake in a hepatic cavernous hemangioma, previously diagnosed in 1984 with CAT and Tc-99m labeled RBC images. The reason for the progressive accumulation of Tc-99m DTPA in the hepatic hemangioma is probably caused by the blood-pool of the lesion with probable extravasation secondary to altered capillary permeability (6).

Renal allograft survival in related non-identical recipients is similar to cadaveric graft survival. In order to increase the number of successful transplants among immunologically incompletely matched related recipients, donor blood transfusions are given to the recipients before renal transplantation. The graft survival results in the Donor-Specific Transfusion (DST) group is similar to those achieved by HLA (identical) siblings. A case of an unusual scan pattern of rejection in a patient receiving DST prior to the transplant is presented. This scan pattern is unlike that of conventional rejection.
