This is a report of a patient who underwent high-resolution brain perfusion SPECT studies during and after an episode of transient global amnesia. During the attack, SPECT imaging showed increased perfusion in the left medial temporal region involving the amygdala and hippocampus. After recovery from amnesia, a follow-up SPECT examination showed slightly decreased perfusion in this region. These findings support the hypothesis that transient global amnesia is associated with transient hyperperfusion in the medial temporal brain structures, and confirm the utility of high-resolution SPECT imaging for the evaluation of the fine details of functional brain anatomy.

Previous reports on the application of bone scintigraphy to evaluate and assess the degree of vascular and soft tissue ingrowth into coralline ocular implants after operation are limited and SPECT imaging has not been reported. Two cases are described in which planar and SPECT imaging of Tc-99m MDP uptake in ocular implants were performed at approximately 3 and 15 months after surgery.

Familial dysalbuminemic hyperthyroxinemia, a syndrome involving an abnormal affinity of albumin for thyroxine, results in elevated total thyroxine and free thyroxine index levels but normal triiodothyronine resin uptake and thyroid-stimulating hormone levels. Danazol is a synthetic androgen that increases triiodothyronine resin uptake and decreases total thyroxine, secondary to a decrease in thyroid-binding globulin levels. A 35-year-old woman with familial dysalbuminemic hyperthyroxinemia who was given danazol, in whom nervousness, insomnia, and weight gain developed, is described. Thyroid tests performed after initiation of danazol therapy revealed an increase in triiodothyronine resin uptake, with persistently elevated total thyroxine and free thyroxine index levels, and normal thyroid-stimulating hormone levels. Once the danazol was withdrawn, the symptoms resolved, the triiodothyronine resin uptake returned to normal, and the thyroid-stimulating hormone remained normal. The effects of danazol on a patient with familial dysalbuminemic hyperthyroxinemia correlate well with the effects on normals, and the ultrasensitive thyroid-stimulating hormone was the most useful test in separating hyperthyroxinemia from hyperthyroidism.

Carbohydrate antigen 50 (CA 50) is a tumor marker that increases in many malignancies, especially in carcinoma of the digestive tract. False-positive results occur in benign liver disease. The behavior of CA 50 in 86 cirrhotic patients was studied, with thorough clinical and laboratory evaluations. There were abnormal values in 75.6% of the patients without significant differences among the different Child's grades. Significant correlations with some liver tests were found, especially transaminases, and at lower degrees with cholestatic parameters. Despite the previously reported relation with cholestasis, especially in biliary diseases, the CA 50 serum levels of the authors' cirrhotic patients appeared to be more closely related to cytolysis, according to the results of several statistical tests, including multivariate analysis. Because of the percentage and the levels of the abnormal results, this antigen cannot be used as a tumor marker in cirrhotic patients. Cytolysis seems to have a pathogenetic role in the increase of CA 50, at least in cirrhosis.

The intestinal phase of cholescintigraphy may be used to study the state of small bowel dysfunction. A patient with transient afferent limb obstruction following a Billroth II gastrojejunostomy was diagnosed by cholescintigraphy. The study also was instrumental in recommending conservative management of the patient, thereby avoiding major surgical intervention.

The authors discuss a rare case in which bone metastases from a malignant glucagonoma mimicked avascular necrosis of the right femoral head initially on both bone scintigraphy and MRI. Repeated imaging 6 months later showed an additional L1 vertebral abnormality, indicating that the lesions were actually bone metastases. Retrospective analysis of the bone scan abnormality in the proximal femur revealed that the abnormal tracer uptake extended into the femoral neck, helping the radiologist to distinguish avascular necrosis from metastasis.

After resection of a medulloblastoma, a 2-year-old boy had a bone scan to rule out metastasis. An abnormally intense concentration of Tc-99m MDP was noted within the left kidney. A renal scan performed on the previous day showed no obstruction. Further renal evaluation with sonography revealed calcifications of the left medullary pyramid tips and renal sinus. The right kidney was normal. The findings are consistent with the clinical diagnosis of a unilateral medullary sponge kidney.

A 72-year-old woman, who had been treated with intravesical instillation of formalin for hemorrhagic cystitis (secondary to cyclophosphamide therapy), underwent ln-111-labeled leukocyte scintigraphy for a suspected intra-abdominal abscess. Although no focus of infection was identified, intense bladder wall uptake of labeled leukocytes, presumably the result of chemical cystitis, was observed.

Tc-99m tagged RBC SPECT imaging has been proven a sensitive means of detecting portosystemic venous collaterals in patients with cirrhosis and portal hypertension. Presented is a case of a 54-year-old man with a history of long-term alcohol abuse, cirrhosis with portal hypertension, and other complications. A Tc-99m–tagged RBC SPECT examination, performed to evaluate the liver for a cavernous hemangioma, demonstrated a tortuous, dilated umbilical vein and multiple dilated superficial paraumbilical veins: caput medusae.
