Thirty-eight children who had septic arthritis, juxtaarticular osteomyelitis, cellulitis or a combination thereof were studied on consecutive days with99mTc-methylene diphosphonate (99mTc-MDP) and67Ga-citrate (67Ga) imaging, respectively, in an attempt to make a definitive diagnosis. The99mTc-MDP uptake reflects the bone response to the septic process, whereas67Ga seems to be concentrated at the site of infection by the inflammatory components. By utilizing the information from both modalities the septic focus was disclosed in all cases, and in the majority of patients the precise location of the infection, i.e., bone, synovium, or adjacent soft tissue was possible. The relatively poor resolution of the gamma camera vis a vis radiography was the limiting factor, particularly with infants, in determining the exact site of the septic process. Early in the evolution of the disease radionuclide imaging is considerably more sensitive than radiography.

The use of gallium imaging in the diagnosis of intraabdominal abscess has resulted in false-positive scans from the accumulation of gallium in the gastrointestinal tract. This is particularly true in patients with a blind loop or bowel obstruction. Orally administered99mTc can be useful in reducing the number of falsepositive scans and may confirm the intraluminal accumulation of gallium in these cases. A case study is presented which illustrates the efficacy of orally administered99mTc for this purpose.

Post-traumatic autotransplantation of splenic tissue in the pleural or peritoneal cavities may be diagnosed by99mTc-sulfur colloid scanning. This may be useful in the evaluation of abdominal pain or pleural mass in patients after splenectomy performed for trauma.

Three cases of autonomous hot nodules of the thyroid (toxic adenomas) coexisting with thyroid cancer are reported, and the scintigraphic findings presented. In the first case the carcinoma appeared as a cold area within the hot nodule: in the second the hot nodule was on the right lobe, whereas the carcinoma involved the left lobe and the isthmus; and in the third case there were two benign autonomous hot nodules together with a malignant cold nodule.

Extraosseous accumulation of bone-seeking radiopharmaceuticals has been reported in various benign and neoplastic conditions. This report concerns extraosseous uptake in mucinous adenocarcinoma of the stomach. Punctate calcification was demonstrated in the lesion. To our knowledge this has not been previously reported. Additionally, focal hepatic activity was present, presumably in the hepatic metastases from the gastric carcinoma.

A new technique for bone imaging based on a single axis diverging collimator is presented. Whole body bone images are performed in a single pass in standard or minified size with appropriate reduction in imaging times. The advantages of the method are discussed.

Previous pharmacological studies have suggested that diphosphonates are superior bone scanning agents than pyrophosphate. The bone images obtained with both99mTc-hydroxyethylidene diphosphonate and99mTc-pyrophosphate were compared in a clinical study of 11 patients with bone metastases. The images obtained with H.E.D.P. were subjectively superior, and the total number of lesions detected was greater than with PYRO. In addition, higher tumour:normal bone ratios were obtained with H.E.D.P. The superior pharmacological properties of H.E.D.P. appear to give this agent a clinically important advantage in lesion detection when compared with PYRO.

The value of the67Ga scan has been established in a variety of malignancies. However, in a newly described entity, angioimmunoblastic lymphadenopathy (AILD), the value of this procedure has not been defined. We have described here a case of AILD in which67Ga scan was helpful in indicating disease activity in some areas not readily appreciated on physical examination.
