Intense myocardial uptake from a Tc-99m-MDP bone scan is demonstrated in a patient with chronic renal failure and hypercalcemia. This case demonstrates the sensitivity of bone scanning in detecting microcalcification in the myocardium which is not apparent on the plain chest radiograph. The exact mechanism for myocardial uptake of the bone scanning agent is not known.

Extra–cardiac TI-201 accumulation was noted on myocardial imaging of a patient, and was subsequently shown to relate to normal uptake in a displaced stomach. It is important to recognize that–non–cardiac activity may relate to an unsuspected disease process, either benign or malignant.

A sixty–year–old man with acute myocardial infarction was incidentally found to have a hypertrophied anterolateral papillary muscle (ALPPM) of the left ventricle on gated blood pool (GBP) and TI-201 myocardial perfusion images. Hypertrophy of the ALPPM was visualized as a movable defect in the lateral basal area on GBP imaging throughout the cardiac cycle and on the TI-201 study as a radionuclide accumulating structure, consistent with the defect in the GBP. A combination of these findings may suggest the presence of a hypertrophied papillary muscle of the left ventricle.

Routine liver–spleen imaging is sometimes performed to search for residual splenic activity following splenectomy. A patient with previous splenectomy following trauma snowed no evidence of splenic tissue when the usual technique was used for colloidal liver–spleen imaging. A repeat study with an increased intensity setting demonstrated multiple sites of residual splenic tissue in the abdomen.

Visualization of amyloid deposition in breasts by bone scanning agent Tc-99m-MDP is presented. Marked deposition of amyloid in the intercellular spaces between fat cells is believed to be responsible for the appearance seen on the bone scan of this patient.

A case of supraceliac aortic occlusion diagnosed by a radionuclide angiogram and confirmed by contrast aortography is presented. The place of radionuclide angiograms in the evaluation of suspected aortic occlusions is discussed.
