The authors report a case in which in-111 platelet imaging provided uniquely useful information in the discovery of an abdominal aortic aneurysm. This case illustrates the potential usefulness of this procedure in the detection of unsuspected sites of thrombogenic vascular disease.

The authors have previously shown that the definition of ablation of thyroid tissue in patients treated with thyroidectomy and radioiodine (l-131) for thyroid carcinoma depends upon the dose of l-131 used to scan the patient. The therapeutic response to l-131 therapy was evaluated in a group of ten differentiated thyroid cancer patients who had a negative 2-mCi (-2 mCi) diagnostic study, but had a positive 10-mCi (+10 mCi) diagnostic study (group 1) during their follow-up evaluation. These results were compared to another group of ten differentiated thyroid cancer patients who received l-131 ablation therapy based on a positive 2-mCi (+2 mCi) l-131 scan (group 2). Six patients in group 1 and eight in group 2 had improvement or ablation of residual tissue based on the 10-mCi scan following therapy. The difference in response between the two groups was not statistically significant (P=0.63) by two-tailed Fisher's exact test, indicating that even patients with -2-mCi, but +10-mCi scans may respond to l-131 therapy. Whether the large dose therapy makes any impact on the clinical outcome has not been answered by this study.

The measurement of thyroidal uptake of radioiodine using a gamma camera and computer is described. In 12 children there was good agreement with simultaneous measurements using a probe, while in a further 14 patients the uptake values were consistent with the clinical diagnosis. The technique should be especially useful for infants and small children.

A case of massive hepatic necrosis has been found to have intense, diffuse accumulation of Tc-99m HMDP in the liver. A review of three similar cases revealed that diffuse intense uptake of bone-scanning agents is a grave prognostic sign.

With the recent advances in pediatric cardiology and cardiovascular surgery, assessment of ventricular function in single ventricle complexes is becoming increasingly important. The serial assessment of ventricular function helps our understanding of the natural and unnatural history in these patients. Equilibrium radionuclide ventriculography is safe, easy to perform, and well-suited to the serial assessment of ventricular function. Fifteen nuclear studies were performed in 15 children with single ventricle complex. Nuclear studies were imaged in both the anterior and left anterior oblique views in each patient. The ventricular ejection fraction calculated from the anterior view (the view with the best atrial-ventricular separation) closely approximated the cineangiographic ejection fraction (54.0 vs 59.1%). Equilibrium radionuclide ventriculography is a valid method to calculate ventricular ejection fraction in single ventricle. The anterior view should be used for region of interest selection and subsequent ejection fraction analyses.

Six preoperative and five postoperative hepatobiliary scans in children having cystic dilatation of the biliary tract were analyzed. The preoperative hepatobiliary scans revealed three distinct patterns. The type 1 pattern showed, in early scans, good hepatic uptake of radioactivity, and photon deficient area(s) around the porta hepatis or in the subhepatic region. These area(s) showed progressive accumulation of radio activity in the delayed scans. The type 2 pattern revealed poor hepatic uptake, nonvisualization of major biliary channels, and photon deficient area(s) in the porta hepatis or subhepatic region which persisted throughout the study. Preoperatively, it was possible to diagnose cystic dilatation of biliary tract in cases that showed type 1 and type 2 patterns. No preoperative diagnosis was possible when the scans snowed a type 3 pattern. Analysis of results revealed a correct pre-operative diagnostic rate of 83.3% for the cystic dilatation of biliary tract. It was also possible to provide correct functional information regarding the patency of the bile outflow tract in all 11 cases, both pre- and postoperatively, with a diagnostic rate of 100%. On the basis of the overall accuracy of results obtained in this series, hepatobiliary scintigraphy is strongly adovcated in suspected cases of cystic dilatation of the biliary tract for the initial diagnosis and for the assessment of the status of the bilioenteric bypass postoperatively.

A male infant, with bronchopulmonary dysplasia and ventilator dependence, had lung ventilation (Xe-133 gas) and perfusion (Tc-99m MAA) imaging performed. The examination revealed a region that was ventilated but not perfused (mismatch) and a separate area that was perfused but not ventilated (reverse mismatch). The basis of both abnormalities is suspected to be related to bronchopulmonary dysplasia, atelectasis and/or mechanical ventilation. Ventilation/perfusion mismatches and reverse mismatches can occur in the same patient.

Skeletal scintigraphy is useful for detecting primary renal carcinomas and associated osseous metastatic deposits. Usually, renal masses present as photondeficient foci, and osseous metastases, as foci of increased response. A case is presented with the unusual combination of focal increased Tc-99m MDP localization to the primary renal cell carcinoma and photopenic osseous metastatic foci. Proposed mechanisms are discussed.
