Separate imaging studies employing Tc-99m MDP, In-111 labeled leukocytes, and Tc-99m SC were performed in a patient receiving external beam radiation therapy to the mediastinum and left hemithorax. The leukocyte scan demonstrated greatly increased activity at the site of the radiation port. The bone marrow (sulfur colloid) scan was normal and the bone scan demonstrated only minimally increased activity in the irradiated region. The varying appearance of these scans may represent the difference in the early effect of radiation on bone and bone marrow elements.

The authors present three patients with degenerative disc disease (one asymptomatic patient, one febrile patient without spinal infection, and one patient with spinal infection) and cold vertebral lesions on In-111 WBC imaging. These cases illustrate the diagnostic dilemma degenerative disc disease may pose when interpreting an In-111 WBC study in suspected osteomyelitis.

A 43-year-old woman confined to bed and wheelchair due to multiple sclerosis (MS) presented with complaints of sharp, bilateral hip pain (left greater than right) that radiated to the thighs with point tenderness to trochanteric palpation. She had received several courses of high–dose intravenous corticosteroids for MS. Plain radiographs were normal. Three–phase Tc-99m MDP bone scan demonstrated focal increased tracer activity in the trochanteric regions of the hips on blood pool images only. MRI was performed using T1, fast spin echo T2, and STIR sequences in a 1.5T unit. The T1, fast spin echo T2, and a previous .5T standard spin echo examination all were normal. However, the STIR sequence demonstrated abnormal increased signal in the trochanteric bursae bilaterally. This is the first known report of trochanteric bursitis with plain film, scintigraphic, and MRI correlation.

Dynamic (continuous) and static imaging techniques were compared in the analysis of gastric emptying patterns in 16 consecutive adult patients with normal gastric half-emptying times assessed by dynamic imaging for 90 minutes under a dual head camera allowing simultaneous anterior and posterior imaging. Gastric t½ values were calculated for each patient based on four different data analysis methods: geometric mean corrected dynamic data (GD), anterior alone dynamic data (AD), geometric mean corrected simulated static data (GS), and anterior alone simulated static data (AS). The mean t½ values were: GD=63.8 minutes, AD=72.1 minutes, GS=65.3 minutes, and AS=73.0 minutes. Both the geometric mean corrected methods had a statistically different t½ than either of the anterior methods. Lag phase measurements, defined as a 2% decrease in gastric activity were: GD=7 minutes, AD=19 minutes, GS=6 minutes, and AS=16 minutes. Dynamic and static geometric mean methods provide similar gastric t½ calculations that are statistically different than anterior alone methods, and the lag phase is much shorter using either of the geometric mean methods.

Radionuclide gastric emptying studies, including solid and liquid phases, were used to evaluate the gastric motility of healthy Chinese. Forty-eight patients (18-76 years old) and 38 volunteers (27-79 years old) accepted solid or liquid food gastric emptying studies, respectively. The solid meal was two fried eggs mixed with 500 µCi Tc-99m phytate and two pieces of toast to form a sandwich. The liquid meal was 500 ml of water with 5% glucose and 500 µCi Tc-99m phytate. Studies were performed with the subjects in the supine position and a gamma camera in a left anterior oblique (LAO) position while data were collected for 90 minutes and 30 minutes after finishing the solid and liquid meal, respectively. The gastric emptying time was presented as a half-time (t½) of the food emptying from the stomach. The study results showed that the t½ is 89.0 ± 17.8 in older patients (>60 years old) and 88.0 ± 18.2 in younger patients (≤60 years old) for the solid phase studies (P>0.05), and that the t½ is 46.2 ± 11.1 in older patients (>60 years old) and 31.9 ± 11.3 in younger patients (≤60 years old) for the liquid phase studies (P<0.05). We concluded that the age effects of gastric emptying in healthy Chinese occurred only in the liquid phase.

Patients with Parkinson's disease often experience dysphagia, in which case food seems to be blocked in the throat. The patient must swallow over and over to get it down. A radionuclide solid phase esophageal motility study was conducted to evaluate esophageal function of patients with Parkinson's disease. Twenty-seven patients and 27 age-matched normal volunteers were studied. Each subject was placed in the supine position above a gamma camera linked to a computer and was given a 4 mL bolus of solid gelatin containing 75 MBq Tc-99m pertechnetate. Data were acquired in the list mode. A computer routine was used to calculate the total mean transit time, the residual fraction after the first swallow, and the retrograde index. The preliminary results suggest: 1) patients with Parkinson's disease display significantly slowed transit time when compared with normal age-matched controls, and 2) dysphagia of Parkinson's disease may improve with medication. A solid phase esophageal motility study may be used as a monitor of dysphagia in patients with Parkinson's disease in our future studies.

A case of carcinoid tumor of the thymus is presented in which TI-201 SPECT demonstrated intense accumulation in the lesion. Conventional chest radiograph and CT scan in a 66-year-old man who had been experiencing cough and pain at the anterior part of the chest revealed a tumor in the anterior mediastinum, suggesting a thymoma. The tumor was clearly delineated in both early and delayed SPECT studies. On the delayed scan, the tumor was more prominently visualized with an extremely high uptake ratio of the lesion compared to the contralateral background; therefore, the tumor was believed to possess very prolonged TI-201 retention. The pathological and immunohistochemical examinations confirmed that the area of intense TI-201 uptake was rich in viable cells.

Intense thyroidal Ga-67 accumulation was seen in a man with AIDS imaged for suspectedPneumocystis cariniipneumonia. Concurrent Tc-99m pertechnetate thyroid scanning demonstrated absent trapping, helping establish the diagnosis of painless thyroiditis. Occult hyperthyroidism, and not pulmonary infection, may have been responsible for the patient's original presenting symptoms.

ln-111 pentetreotide scintigraphy of 10 patients with residual or metastatic medullary thyroid carcinoma is described. Six patients had sporadic tumor and 4 had MEN IIB. Foci of increased tracer uptake were observed in 9 patients: in the thyroid bed (4 patients), the mediastinum (3 patients.), the shoulder area and left lower abdomen (1 patient), and the left upper abdomen (1 patient). The 10th patient had no abnormal uptake. CT confirmed 2 mediastinal lesions and 2 out of 3 thyroid masses, but did not detect the thyroid remnants or the lesions in the shoulder area and abdomen. Lung lesions ≤1 cm in diameter and ill-defined liver foci (2 patients) were seen on CT, but not on scintigraphy. Small liver metastases not demonstrated on CT or on scintigraphy were identified at surgery in a MEN IIB patient. Elevated urinary epinephrine was found in 2 out of 4 MEN IIB patients. In one, tracer uptake in the left adrenal corresponded to a mass on CT, to pathological uptake of MIBG and DMSA, and to a tumor removed at surgery. The second patient had peritoneal spread of malignant pheochromocytoma (at surgery), but negative CT and only a single focus in the left lower abdomen on scintigraphy. Somatostatin-receptor imaging is useful for the detection of residual and recurrent medullary thyroid carcinoma, and may identify pheochromocytoma in MEN IIB patients.

Enlarged cystic parathyroid glands, whether hyperfunctioning or nonfunctioning, are rare and their accurate preoperative diagnosis is a difficult task. The authors report two cases of patients with cystic neck masses studied by TI-201-Tc-99m pertechnetate subtraction scintigraphy, computerized tomography, and ultrasonography. The diagnostic consensus of preoperative studies was that these lesions were thyroid nodules or parathyroid adenomas. Surgical removal and histopathologic examination of these masses revealed a cystic parathyroid gland and a cystic parathyroid adenoma, respectively. When a fluid-filled lesion in the neck deep to the lower pole of the thyroid gland is encountered, the possibility of a parathyroid cyst should be considered so that further diagnostic tests can be properly performed.
