Five cases of pyogenic vertebral osteomyelitis diagnosed clinically and with radioisotope studies are reported. All patients presented with symptoms of back pain, an elevated erythrocyte sedimentation rate, and a normal leukocyte count. Three had positive blood cultures and four had changes on plain radiographs. All five had positive phosphate bone and Ga-67 imaging, and two showed paravertebral uptake of gallium giving a “butterfly”-like appearance.

Twenty-four skeletal lesions were studied in seven patients with eosinophilic granuloma by radiographic skeletal surveys and radionuclide bone imaging. The radiographs detected 22 (92%) of these lesions and missed only two, whereas the scintiscans identified only 16 (67%) of these lesions, and missed eight. Radiographic skeletal survey and radionuclide bone imaging are complementary procedures in detecting bone lesions in bone marrow disorders, including eosinophilic granuloma. Use of either method alone is fraught with the danger of missing bone lesions of eosinophilic granuloma.

Osteosarcoma usually spreads via the blood stream, resulting in pulmonary and skeletal metastases. The value of bone imaging in the management of these patients is widely accepted. Less commonly, the disease spreads via the lymphatics. Such a case is reported to show that bone imaging will detect progressive involvement of the lymph nodes.

A 50-year-old man with hypercalcemia related to Cope syndrome was referred for bone scintigraphy. There was a clear accumulation of the technetium-phosphate complex in the stomach. The condition was reversible with treatment of the underlying cause.

Investigation of low back pain by quantitative sacroiliac joint scintigraphy (QSS) can detect the presence of sacroiliitis not apparent by roentgenography. Transient unilateral sacroiliitis was documented by QSS in a patient with Whipple's disease during tetracycline treatment, when the peripheral arthropathy was in remission. Thus, the pattern of inflammatory spinal disease, if associated with the intestinal disease, differs from that of the peripheral arthritis.

A case report of a 21-year-old man with Osgood-Schlatter disease is presented. Although the diagnosis was provided by the radiograph of the affected tibial tuberosity, the scan appears to have some diagnostic utility and may better correlate with clinical activity.
