There is uncertainty regarding the role, if any, of oral contraceptive steroids in the development of focal nodular hyperplasia of the liver. A case of focal nodular hyperplasia of the liver diagnosed by scintigraphy in a 33-year-old woman is presented. The lesion regressed in a dramatic way after oral contraceptive discontinuation. This case report suggests that oral contraceptives may influence the natural history of focal nodular hyperplasia of the liver, and that a trophic effect on the liver lesion is quite possible.

The authors report the case of a 27-year-old woman with gamma probe-guided resection of a recurrent regional lymph node metastasis in papillary thyroid cancer. Reoperation is often impaired in cases with extensive scarring because of previous surgery, high-dose radioiodine therapy, and external irradiation. In this case with nodal metastasis, the high I-123 uptake allowed more rapid intraoperative detection and influenced the surgical approach. Evidence of complete removal of tumor tissue could be given intraoperatively by measuring the tumor bed, and this was confirmed by postoperative normalization of Tg levels and a negative I-131 scan. In other locations, and especially for small solitary lesions, gamma probe-guided surgery may be a promising alternative to high dose I-131 therapy.

A case of reflex sympathetic dystrophy (RSD) after total hip replacement (THR) is reported. It is a very unusual cause of pain after any arthroplasty procedure. Three-phase bone imaging diagnosed the condition and sympathetic blockade was used for confirming and treating the condition. The role of the bone scan in differentiating RSD from other causes of pain after THR is highlighted.

The authors studied 33 normal patients who ranged in age from 8 to 71 years to establish a normative data base for young-age, middle-age, and older-age subjects using a computer automated semi-quantitative cortical annular region of interest (ROI) method. The data were grouped to obtain a "young age-range" normative data base of mean age ± 1 S.D. = 13.1 ± 4.8 years (seven subjects with an age range of 6 to 20 years); a "middle age-range" normative data base of mean age ± 1 S.D. = 39 ± 2.7 years (12 subjects with an age range of 35 to 43 years); and an "older age-range" normative data base of mean age ± 1 S.D. = 59.7 ± 5.8 years (14 subjects with an age range of 55 to 71 years). Normative values were obtained for brain level parallel to and positioned at 3.5, 5.5, and 7.5 cm above the canthomeatal (CM) line. The results show that the average global rCBF indices (defined as cortical/cerebellar ratios) for the young age-range group were 0.98, 0.99, 1.07; middle age-range group were 0.84, 0.86 and 0.88; and older age-range group were 0.88, 0.87 and 0.87 for CM + 3.5 cm, CM + 5.5 cm, and CM + 7.5 cm, respectively. In routine clinical studies, on more than 2,000 Tc-99m HMPAO brain SPECT scans, the authors have employed this semi-quantitative cortical circumferential normalization method of analysis to accurately calculate indices representing cortical blood flow values. This method also allows efficient comparison of individual patient values to age-range matched normal control groups to assist in disease diagnosis.

The bone-seeking radiopharmaceutical Sr-89 has been used as a palliative treatment for patients with bone pain caused by bone metastases. The authors report the results of nine patients (three with prostate cancer, four with breast cancer, one with thyroid cancer, and one with lung cancer) who underwent therapy with Sr-89 chloride for painful bone metastases, and evaluate Sr-89 imaging with bremsstrahlung. Two levels of dosage (1.5 and 2.2 MBq/kg) were used. Sr-89 imaging was performed in seven patients 1 week after injection. Abnormal uptake was seen in all and was consistent with the results of Tc-99m HMDP imaging. Six patients were assessed at 3 months and three patients toward the time they were terminal; 78% (seven of nine) derived some benefit. Two patients had a favorable clinical response and showed improvement on Tc-99m HMDP imaging.

A 29-year-old man, with a history of von Recklinghausen's disease, presented with progressive dyspnea associated with a rapidly growing mass on the right chest wall. Plain radiograph and CT of the chest revealed a huge soft-tissue mass with central low-density area involving the right upper lung and chest wall. SPECT imaging with Ga-67 citrate, Tl-201 chloride, Tc-99m pentavalent DMSA (V-DMSA), and Tc-99m MIBI were performed to characterize the mass. The tumor concentrated Ga-67, Tl-201, and Tc-99m (V) DMSA, but not Tc-99m MIBI. Punch biopsy of the lesion revealed malignant transformation of a thoracic neuroma (neurofibrosarcoma). Subsequently, findings compatible with the presence of a multidrug resistance-1 (MDR1) gene in the tumor was documented, which may explain the poor uptake of Tc-99m MIBI. The patient did not respond to intensive chemotherapeutic regimens, and died 3 months later. This case demonstrates the potential use of combined radionuclide imaging for the detection of malignant transformation of neurofibroma, as well as for predicting tumor response to chemotherapy.

Tc-99m nofetumomab merpentan (Verluma), consisting of a Fab fragment of the pancarcinoma murine antibody NR-LU-10, has been previously evaluated as a diagnostic imaging agent in staging patients with lung cancer. The authors have taken advantage of the pancarcinoma reactivity of this antibody to select patients with a variety of carcinomas for radioimmunotherapy trials. These have included gastrointestinal, breast, ovary, pancreas, kidney, cervix, and bladder carcinoma. This article documents the range of tumor types and locations that can be identified by gamma camera imaging with this radioimmunoconjugate. Tumor was positively identified in 92% of 107 patients studied. In 15 patients, the images led to suspicion of previously unknown disease. The authors conclude that this radioimmunoconjugate is useful in assessing patients with advanced disease. Additional studies may be warranted to explore further the potential benefit of this diagnostic imaging agent in evaluating the extent of disease in patients with a variety of carcinomas.

The authors present the case of a patient with recurrent hyperparathyroidism who was shown on thallium-technetium subtraction scintigraphy, ultrasound, and magnetic resonance imaging to have recurrent parathyroid tissue in the right side of the neck. The thallium scan also showed intense uptake centrally in the chest, mimicking an intrathoracic adenoma. This was subsequently shown on plain films and a radionuclide bone scan to be due to Paget's disease of the sternum. This case illustrates the value of a combined imaging strategy in preventing an unnecessary thoracotomy after a false-positive nuclear medicine scan.

After the intravenous administration of a radiolabeled somatostatin analogue (octreotide), normal thyroid and neoplastic and nonneoplastic thyroid lesions can be visualized. The authors present the cases of two patients who underwent somatostatin receptor scintigraphy(SSRS) using In-111 pentetreotide: one for the study of suspected paraneoplastic ACTH hypersecretion, and the other for a restaging of breast carcinoma with neuroendocrine features. In both patients, SSRS revealed increased uptake in the thyroid, corresponding to "cold" nodules on Tc-99m pertechnetate imaging. Cytologic and histologic examinations showed the typical features of thyroid goiters without lymphocytic infiltration.

A poorly differentiated adenocarcinoma of the stomach showed an unusual type of diffuse wall thickening of the entire stomach, similar to that seen in malignant lymphoma. The pathologic specimen revealed a "lymphoepithelioma"-like pattern of undifferentiated carcinoma with intense lymphocyte and plasma cell infiltration.I-123 IMP scintigraphy incidentally demonstrated increased uptake in the entire stomach on early imaging and moderate washout on delayed studies. SPECT images demonstrated diffusely increased accumulation in the entire gastric wall.
