Donor granulocytes, obtained by filtration leukophoresis, were labeled with ln-111-oxine and injected into seven severely neutropenic patients. All patients were febrile, and presumed to have infection, although infection was eventually proved in only five. Excellent demonstration of infection was achieved in two patients, and fair localization in one. One negative scan became positive when repeated with sedimentation-derived cells from a different donor. This method may be useful in detecting infections in neutropenic patients.

Gallium-67-citrate scanning is often used in the diagnosis of inflammatory processes in hemodialysis patients. The question arises whether the dialysis process will adversely affect the quality of the gallium scan. To answer this question, nine dialysis patients were studied, eight on hollow-fiber dialysis units and one on ultrafiltration, in an attempt to quantitate the loss of gallium through dialysis. Blood sampling during dialysis failed to demonstrate changes in gallium concentration attributable to removal by dialysis. Sampling of dialysate demonstrated levels of activity significantly greater than background; however, quantitation was not reliable. Ultrafiltration with total collection of extracted fluids yielded 0.5% of injected dose removed from the blood. Whole-body scans at 24 hours postinjection showed good quality and total counts unaltered by dialysis. The time of injection prior to dialysis also showed no effect on the quality of the final image.

A 9-year-old uremic girl who showed intense parotid uptake when scanned with gallium-67-citrate is discussed. None of the more frequently described predisposing factors was present. A review of 22 other children scanned for suspected inflammatory disease revealed minimal parotid and lacrimal activity in three, with lacrimal activity only in another four. It is suggested that a metabolic cause be considered when there is abnormal parotid activity with Ga-67 scintigraphy.

A patient with chronic myelogenous leukemia, treated for two years with busulfan, presented with increasing dyspnea of several months' duration. Despite a normal chest radiograph, there was a markedly reduced carbon monoxide (CO) diffusing capacity (41% of predicted normal) and a restrictive ventilatory pattern on pulmonary function testing. Gallium-67 scanning revealed diffuse uptake in both lungs. The busulfan was discontinued and therapy was changed to hydroxyurea. Three months later the patient was without symptoms, the CO diffusing capacity had risen to 64% (of predicted), and the Ga-67 scan had returned to normal. The chest radiograph remained normal. Despite the lack of biopsy proof, we believe Ga-67 scanning was an aid in the early detection of cytotoxic-induced lung disease in a reversible stage. Gallium-67 scanning may be useful in the early diagnosis of pulmonary injury from cytotoxic agents.

A technique is described using l-131-sodium iodine and Tc-99m-sodium pertechnetate for localizing functioning thyroid tissue in the neck following total thyroidectomy. The procedure consists of obtaining a 1-131 thyroid scan. If functioning thyroid tissue is found, it can be localized by means of skin markers as well as a swallow using Tc-99m-sodium pertechnetate. The procedure is safe, simple, and cost efficient and provides a means of localizing functioning thyroid that may influence subsequent therapy.
