To assess both ventricular function and myocardial perfusion, five normal volunteers, 19 patients with coronary artery disease, and two patients with cardiomyopathy and normal coronary arteries were injected with 20 mCi of Tc-99m isonitrile (either methoxyisobutyl isonitrile or carbomethoxyisopropyl isonitrile) at peak bicycle exercise and again at rest. A standard TI-201 stress test was performed in all patients at the same level of exercise within one month of the isonitrile study. In all normal subjects, myocardial perfusion was normal at stress levels and the left ventricular ejection fraction increased 5% or more with exercise. In the 19 patients with coronary artery disease, the ejection fraction response to exercise was abnormal in 14 patients. Perfusion images with the Tc-99m isonitriles correlated well with TI-201 images during exercise and at rest, with 89% concordance in areas of ischemia or infarction and 93% in normal segments. A simultaneous study of myocardial perfusion and ventricular function can be performed using a single Tc-99m labeled myocardial agent. Tc-99m isonitriles, particularly Tc-99m MIBI, result in sufficiently high photon flux that ventricular performance can be studied at peak exercise and again during rest using the first pass method.

Two cases of calf muscle arteriovenous malformation in young women, presenting clinically as deep venous thrombosis, were initially detected by Tc-99m erythrocyte venography and later confirmed by arteriography. The role of this technique in the investigation of venous thrombosis is discussed, emphasizing the importance of the lateral calf view.

A 72-year-old woman with Paget's disease underwent therapy with EHDP for 5 years. A Tc-99m MDP bone scan demonstrated an unusual image in the right femur. The upper one third of the femur was thickened, bowed, and had diffuse and mild uptake. This corresponded to radiographic findings of mixed or chronic Paget's disease. A long area of intense and homogenous uptake was found immediately distal, and with a sharp demarcation. Alterations typical of the lytic phase of Paget's disease were seen on radiographs. The overall lesion in this patient thus demonstrated two distinct patterns, both radiographically and on bone imaging. It is conjectural whether this was related to the chronic use of a diphosphonate in therapy.

One hundred adult sports medicine patients with clinical findings suggesting chondromalacia of the patella underwent bone imaging. Of these patients, 50 eventually underwent arthroscopic surgery and the remaining 50 were treated conservatively. The intensity of abnormally increased scintigraphic activity in the patella correlated closely with the grade of chondromalacia observed by the arthroscopist. Bone imaging also was of value in directing the arthroscopist to clinically unsuspected torn menisci. In conclusion, for sports medicine patients with clinical findings suggestive of chondromalacia of the patella, bone imaging contributes to an accurate diagnosis and appropriate surgical planning.

A 55-year-old man with an 11-year history of intermittent right pleural effusions had continued fevers and a large right transudative pleural effusion. Minimal ascites was noted by ultrasound examination. A communication between the thorax and intraperitoneal cavity was established by radionuclide examination of the chest and abdomen. Tc-99m sulfur colloid was injected into the peritoneal cavity, and imaging at 1 and 4.5 hours confirmed passage from the peritoneal to the pleural cavity. Repeated attempts to sclerose the pleural cavity failed to decrease the transudative accumulations. The patient was subsequently treated with the placement of a LeVeen shunt.

Brucellosis is an endemic disease in the Middle East. Its incidence in Kuwait has increased during the last 5 years. Bone and joint involvement causes major symptoms and disabilities. Radionuclide bone scans are more sensitive than radiographs in detecting these lesions. The aim of this study is to describe the abnormal patterns detected on bone imaging in acute and chronic brucellosis. Tc-99m MDP bone scans of 56 patients with established diagnosis of brucellosis (19 acute and 37 chronic) were retrospectively analyzed. Bone scans were positive in 8 of 19 patients (42%) with acute brucellosis and in 28 of 37 patients (76%) with chronic brucellosis. Six patterns were observed: involvement of an entire body of one or more vertebrae, especially at the lumbar region (50%); sacroiliitis (41%); focal high uptake at the junction of the upper and lateral margins of the vertebra “Caries sign” (27%); multiple costovertebral joints and costochondral junction involvement (19%); involvement of large joints similar to degenerative osteoarthritis (25%); and focal involvement of long bone (11%).

Forty-two patients with diffuse infiltrative lung diseases were imaged with Ga-67 citrate and Tc-99m glucoheptonate (GH). Twenty patients had sarcoidosis, six had fibrosis, six had tuberculosis, nine had lung infiltration, and one had pleural empyema. The main difference between Ga-67 and Tc-99m GH was the much greater uptake of Ga-67 in sarcoidosis than that of Tc-99m GH. Fifteen patients with sarcoidosis had positive Ga-67 scans but only six had positive Tc-99m GH scans. The results in other diffuse infiltrative lung diseases were almost equal with Ga-67 and Tc-99m GH. Although Tc-99m GH is less expensive and simpler to use, it is not an adequate substitute for Ga-67 in diffuse infiltrative lung diseases.

Sialography may cause greatly increased Ga-67 activity within otherwise normal salivary glands. Five patients are presented in whom sialography with oily contrast material injected 8–26 days before Ga-67 imaging produced strongly positive examinations. In each patient it appears that sialography rather than pre-existing pathologic conditions accounted for the increased activity. Sialography-induced subclinical sialadenitis may be the mechanism for the phenomenon.
