Radionuclide imaging with both Tc-99m sodium pertechnetate and TI-201 chloride was studied in 152 patients with thyroid nodules. Ultrasonography also was performed in 81 of those patients. Tc-99m sodium pertechnetate scans demonstrated nodules in 69.7% of 78 differentiated thyroid carcinomas (DC) and 72.2% of 54 thyroid adenomas (Ad). TI-201 chloride was accumulated in 73.7% of DC and 53.6% of Ad. By combining the Tc-99m sodium pertechnetate and TI-201 chloride scans, the detectability of the nodules was increased to 90.8% for DC and 88.9% for Ad, respectively. The Tc-99m sodium pertechnetate scans showed better visualization of cystic lesions than did the TI-201 chloride imaging. The TI-201 chloride images clearly demonstrated intrathoracic tumor invasions in six cases of carcinoma and two cases of Ad. The TI-201 chloride scan was also of value in detecting regional lymph node involvement and the recurrence and metastasis after thyroidectomy. The detectability of space-occupying lesions by ultrasonography was 96.3% in 81 patients with thyroid nodules. Ultrasonography differentiated well between solid and cystic lesions. The presence and extent of nodular lesions were detected with radionuclide imaging and ultrasonography in 98.8% of patients. Radionuclide imaging combined with ultrasonography provides a rapid, convenient, and useful method for the localization and visualization of thyroid tumors.

Scintigraphy using I-131 meta-iodobenzylguanidine has been introduced as an effective method to detect pheochromocytomas and neuroblastomas, and the radiopharmaceutical also is applied in therapy of these tumors. The authors present a case of a metastatic gastric carcinoid tumor, another neural crest tumor, concentrating I-131 MIBG, which was documented by conventional scintigraphy and SPECT in correlation with CT scans and colloid scintigrams of the liver. Two therapeutic attempts in this patient, using I-131 MIBG, are described. The metabolic basis of this phenomenon is discussed, and the importance of I-131 MIBG imaging in the detection of neural crest tumors is underlined.

ln-111 oxine WBC abdominal scanning is now a widely accepted technique for the detection of abdominal infectious processes. High sensitivity and specificity are achieved, ln-111 labeled leukocyte accumulation, however, does not always suggest the diagnosis of abscess. Higher specificity could be obtained by the knowledge of the ln-111 leukocyte distribution pattern in the abdomen in other pathologic states like 1) inflammation of abdominal wounds, stoma; 2) surgical complication without abscess formation; 3) inflammatory or ischemic bowel disease; or 4) swallowing leukocytes which subsequently are visualized in the intestinal lumen. One hundred fifty-two consecutive WBC scans performed over 18 months were reviewed and classified according to their pattern of uptake: 96 cases showed no abdominal uptake, and 56 had accumulation of leukocytes in the abdomen. Twenty-eight of these patients had proven abdominal abscess, and the remaining 26 were positive due to other causes. This report briefly discusses the distribution pattern of ln-111 labeled leukocytes in the latter patients and compares the results obtained using different diagnostic criteria.

A case of splenosis, or the autotransplantation of splenic tissue following trauma, was encountered unexpectedly on a routine liver scan performed in the workup of alcoholic liver disease. Confirmatory scintigraphic images using Tc-99m labeled, heat-damaged, autologous erythrocytes are presented with CT scans of the abdomen.

A case of a 23-year-old man with an encapsulated, anaerobic splenic abscess is reported. Both the ln-111 leukocyte and Tc-99m sulfur colloid scans demonstrated an intrasplenic defect. The Ga-67 citrate scan revealed uptake in the rim of the abscess only where the abscess cavity was relatively photon-deficient. The combined Tc-99m sulfur colloid/ln-111 leukocyte/Ga-67 scan appearance of a subacute splenic abscess has not been described previously. In cases suspected to be splenic abscesses the combined ln-111 leukocyte/ Tc-99m sulfur colloid imaging is the most useful.

Accurate count-based radionuclide estimates of left ventricular volume without the use of a blood sample have not been well described. Resting gated blood pool scans were obtained within 24 hours of catheterization in 31 patients (group 1), and simultaneously with thermodilution cardiac outputs in 29 other patients (group 2) at rest and during an induced-volume change (intervention). End-diastolic and end-systolic volumes were calculated from the single-plane angiogram in Group 1 and from the combination of thermodilution stroke volume and radionuclide ejection fraction in group 2. Excellent correlations were noted between scintigraphic counts and angiographic volumes (r= 0.964), between scintigraphic counts and thermodilution-derived volumes (r= 0.979), and between interventional scintigraphic counts and interventional thermodilutionderived volumes (r= 0.941). Thermodilution and scintigraphic volume changes during the intervention were well correlated (r= 0.85). Accurate count based estimates of left ventricular volume without a blood sample are feasible at rest and during an intervention.

Whole body Ga-67 scans revealed increased uptake in lymph nodes accessible for biopsy in three patients with the acquired immunodeficiency syndrome (AIDS) infected byMycobacterium avium-intracellulare(MAI). In diagnostically difficult cases where the usual methods for diagnosing MAI are not helpful, Ga-67 studies may be of value.

In appropriate cases, information from a dynamic series of nuclear images may be condensed into a single image with one spatial and one temporal dimension. A useful elaboration of the method consists of the masking out of undesired spatial regions. The versatility of such condensed dynamic images is illustrated by examples derived from gastroesophageal and pulmonary studies. Advantages of the method include obviating the need to examine and comprehend multiple images as well as enabling economical archiving of image data. Its diagnostic potential is particularly evident in esophageal transit and infant gastroesophageal reflux studies.
