A patient with progressively worsening thyrotoxicosis, refractory to medical therapy, is described. Repeated measurements of thyroidal RAI uptake over a 13 month period were low consistently and could not be explained by iodine ingestion, thyroiditis, or administration of exogenous thyroid hormone. An I-131 scan ultimately revealed striking activity at the base of the skull, reflecting ectopic excessive production of thyroid hormone by a solitary functioning metastatic thyroid carcinoma. The thyrotoxic state resolved after large doses of therapeutic I-131. Typical features of this rare cause of hyperthyroidism are discussed.

Fourteen patients with slipped capital femoral epiphysis (SCFE) who had high-resolution and magnification bone scintigraphy during treatment were studied. By demonstrating the vascular status of the femoral head and physiologic condition of the growth plate, scintigraphy was found to assist the clinician in the management of patients with this disorder.

Residual urine volume in the bladder was measured using a gamma camera and Tc-99m DTPA labeling of the urine and was compared to urethral catheterization in 21 male patients. The results of the radionuclide method did not differ significantly from the catheterization (P<0.001) and a good correlation was found between the two methods (r = 0.98). The results suggest that the simple, safe radionuclide method may be used instead of catheterization to measure residual urine volume in the bladder.

A rare case of benign cardiac fibroma causing a focal defect on TI-201 imaging in an adult patient is presented. The abnormalities on the TI-201 scan are correlated with other studies performed on the same patient. The usefulness of the TI-201 myocardial perfusion scan as an early, noninvasive test in cardiac workup is stressed.
