In five of 500 consecutive liver scans over a period of six months, isolated left lobe lesions were discovered by multiple imaging techniques. Three of the five cases had definite pathology, of the two remaining cases one was attributed to a normal anatomic variation, and the other was a false positive. The problems associated with and the causes for rarity of isolated lesions involving only the left lobe, and the clinical usefulness of multiple imaging techniques are discussed.

This report summarizes our experience using hepatosplenic scintigraphy to study 36 children with obstructive jaundice. Twenty-seven of these patients had intrahepatic obstruction with the remainder exhibiting obstruction of the biliary system outside the liver. All patients with extrahepatic obstruction had choledochal cysts, none of which concentrated rose bengal. Scans with radiocolloid failed to provide definitive information as to the site of obstruction.

Gray-scale ultrasonography and biliary scanning using111In-phenolphthalexon both demonstrated a dilated gallbladder in a vagotomized cirrhotic with progressive painless cholestasis, which proved to be due to a calculous choledochal obstruction. This case violated Courvoisier's law. Analysis of the111In excreted in a series of fecal specimen showed a wide variation from high-activity dark feces to low-activity pale ones, compatible with an intermittent choledochal patency suggestive of calculous obstruction.

An area of decreased perfusion on a lung scan was shown to correspond to an esophageal hiatus hernia by scanning after a99mTc-pertechnetate swallow. Hiatus hernia should be considered as one of the possible causes of pulmonary perfusion defects.

A case of a localized area of absent activity in the left lobe of the liver is presented. On physical examination there was a marked pectus excavatum of the chest present. In view of the negative workup it is believed that the defect in the liver is due to pectus excavatum.

The term functional asplenia has been used to refer to anatomic presence of the spleen without the ability to accumulate intravenously administered radiocolloid. Functional asplenia had previously been invariably associated with the presence of Howell-Jolly bodies in circulating erythrocytes. The first documented instance of functional asplenia, without the presence of these intraerythrocytic inclusions, is described. The patient suffered acute cellac artery thrombosis. The sudden onset of splenic dysfunction, and accompanying infection, probably accounted for the lack of Howell-Jolly bodies.

A patient with acute hypertrophic pulmonary osteoarthropathy (HPO) secondary to pulmonary metastases from a fibrosarcoma is presented for whom the radiographs of the bones were normal at the time the scan was classically abnormal. Following surgical removal of the pulmonary metastases, the clinical syndrome of HPO receded dramatically, but scan abnormalities persisted for several weeks. The pathology of HPO is discussed in relation to the sensitivity of the bone scan.

The localization of bone seeking radiopharmaceuticals in a variety of extraosseous lesions is well recognized. Specific accumulation in ischemic or infarcted tissue of the heart, brain and intestine has been reported. Two cases which demonstrate localization of99mTc-pyrophosphate in the soft tissues and bone in an arterial thrombosed leg are reported.

It is known that Warthin's tumor of the parotid gland shows increased uptake of99mTc-pertechnetate. The role played by vascularity of the tumor in the increased uptake is not clear. Two cases of histologically proven Warthin's tumor with increased vascularity and increased uptake are reported.

A case is reported of a 34-year-old, 4 month pregnant female, who aborted followed by completion with dilatation and evacuation a week after an appendectomy due to a ruptured appendix with peritonitis. A67Ga-citrate scan done six days post-abortion showed symmetrical accumulation in both breasts, which is believed to be a “normal variant.”
