A method has been developed for the quantification of a projected organ area using a standard gamma camera with pinhole collimation and computer processing of two vertically displaced images. Formulae have been derived expressing the uniquely determined quadratic relationship between object area and aperture distance with respect to projective image areas. This technique is extremely simple and its successful application to thyroid imaging is described.

One hundred twenty-four sequential gated blood pool studies were examined by four observers in order to determine the impact on overall interpretation created by the addition of a routine left lateral view to supplement the standard LAO and RAO projections. Of these 124 studies, 114 studies in 87 patients were considered technically adequate. Forty-one of the 114 studies revealed differences in the perception of apex motion in the LAO as compared with the left lateral view. In seven of these 41 cases, the overall diagnostic impression was altered by the left lateral view. A 15-minute increase in technician time at an incremental cost of $3.00 is attributed to the added view. Approximately 6% of overall diagnostic impressions were altered because of this additional routine view.

Visualization of Ga-67 citrate in the kidneys at 48 hours and 72 hours post injection is usually interpreted as evidence of renal pathology. In reviewing approximately 200 consecutive patients referred for gallium scans, 40 patients who also underwent liver/spleen Tc-99m sulfur colloid (SC) studies within one month of the gallium study were identified. Fourteen of these patients showed advanced hepatocellular dysfunction on the Tc-99m SC liver/spleen images. Of these 14 patients, nine had persistent renal accumulation of gallium at 48 or 72 hours. Five of these nine patients had no evidence of primary renal disease by clinical or postmortem examination and subsequent clinical information indicated that two additional patients probably had no significant renal pathology. Therefore, bilateral symmetrically increased renal uptake of gallium in patients with advanced hepatocellular disease should not necessarily be interpreted as evidence of renal pathology.

One hundred sixty-nine children suspected of having malignant disease were examined by Ga-67 scintigraphy. In 99 children with untreated diseases at the time of examination, abnormal accumulation of Ga-67 was found in 51 patients, including 40 with malignant tumor. Forty-three negative results were obtained in children with benign disorders. Five false-negative results were obtained in patients with neuroblastoma of the adrenal gland. In 70 children with malignant diseases treated before the examination, abnormal accumulation of Ga-67 was seen in 40 patients, including 38 with malignant disease and two with no clinical evidence of recurrence. Normal results were obtained in 30 patients, including 11 still suffering from malignant disease.The results of Ga-67 scintigraphy in all of the children were evaluated qualitatively. For the final diagnosis of malignant disease, diagnostic specificity was 86% and diagnostic sensitivity 79%. The prevalence of malignant disease was 56%. It is concluded that Ga-67 scintigraphy should be used for primary visualization and control of malignant tumors in children.

Twenty-five reports in the medical literature of thyroid carcinomas which were detected after radioiodide therapy for hyperthyroidism were reviewed. These cases did not show a usual characteristic of radiation-associated tumors, namely a long latency period. That is, in 8/25 the latency period was under five years, and the mean latency was only 7.3 years. Further, there was no relationship between latency and age at treatment, or between latency and the dose of radioiodide employed. In 15/25 of the cases, there were known thyroid nodules. Three of the patients had thyroiditis (which itself has a correlation with thyroid carcinoma), and one individual had prior head and neck external radiation. There was no substantiating evidence that radioiodide treatment for hyperthyroidism was the cause of these thyroid carcinomas.

Two case reports of nonvisualization of the gallbladder by hepatobiliary scintigraphy in patients with carcinoma of the gallbladder are described. A hypothesis regarding the pathogenesis of gallbladder nonvisualization in cholecystic cancer is offered. The possible clinical utility of hepatobiliary scintigraphy in these patients is discussed.
