A case of traumatic myositis of the intrinsic muscles of the hand is presented. This is believed to be the first report in which the diagnosis of this type of soft tissue injury was made based upon phases I and II of a threephase bone scan. An incorrect diagnosis of primary skeletal injury could have been made if delayed, 3-4 hour (phase III) imaging alone had been performed because there was augmented uptake of tracer in the normal bone adjacent to the area of soft tissue hyperemia. This report emphasizes the need to perform three-phase scintigraphy to diagnose nonskeletal injury in patients with suspected bone trauma or pain of unknown etiology.

CT scanning, MRI, and ultrasonography are the imaging procedures of choice for the study of rectus sheath hematomas. A rectus sheath hematoma was evaluated scintigraphically after the intravenous administration of Tc-99m labeled RBC, which confirmed the hematoma, demonstrated the sites of bleeding, and revealed continued bleeding.

Pyomyositis is a Staphylococcus aureus infection of muscles reported primarily in tropical countries but seen with increasing frequency in temperate climates. Because of its rarity in the United States, the diagnosis may not be considered immediately, and involved muscles may break down and become abscessed. Diagnosis is most reliably made by the recovery of pus from a muscle aspirate. Ga-67 scan findings of an interesting case of pyomyositis involving many muscle groups in both upper and lower extremities is reported. Despite arthralgias and complicating adult respiratory distress syndrome, Ga-67 correctly localized the infection to muscles in the shoulders, upper thorax, buttocks, and thighs. A bone scan was negative, excluding the possibility of extension of the infection to bone.

Radionuclide imaging in obstructive uropathy has concentrated on disorders of the pelviureteric junction and lower urinary tract. There is a paucity of information, however, available on the scintigraphic manifestations of disorders of the calyces and infundibulum. These conditions may be responsible for similar symptoms, and thus recognition of abnormal retention of activity within one or more calyces offers important diagnostic information. One of these disorders is vascular obstruction of the superior infundibulum, and this report describes the radionuclide appearance of this surgically curable condition.

A case of constritive pericarditis was demonstrated by a positive thallium SPECT stress test for myocardial ischemia. After pericardiectomy, the repeat thallium stress test was normal. The disappearance of the criteria for a positive test suggests that constrictive pericarditis can cause myocardial ischemia, which can be demonstrated by thallium SPECT stress testing.

Radionuclide ventriculography has become the standard method for serially evaluating left ventricular ejection fraction (EF). The "gold standard" for evaluating EF is the contrast ventriculogram, which uses mathematical models to arrive at the volumes used to calculate EF. These models are subject to possible error. This paper reports the standardization of volume measurements of a digital angiographic camera system using a series of cardiac phantoms and the correlation of measurement of the EF of a series of patients whose EF was determined by digital angiography and radionuclide ventriculography.

A method to assess ventricular functional reserve in infants and children unable to perform dynamic exercise was evaluated. Left ventricular ejection fraction was measured by radionuclide angiocardiography at rest and during infusion of dobutamine in 5-15 µg/kg/min dosages. The only side effects noted were arrhythmias in two patients, who had similar ectopy documented previously. Group left ventricular ejection fraction increased from 0.40 ± 0.21 to a maximum of 0.49 ± 0.24 (P<0.001). Left ventricular ejection fraction at 5 µg/kg/min (0.44 ± 0.23) was not statistically different from that measured during infusion of dobutamine at 10 µg/kg/min (0.48 ± 0.24). The six patients receiving anthracyclines, who had relatively low function at rest and increased function during dobutamine administration, were continued on their anticancer regimen without deteriorating in condition. Absolute values of left ventricular ejection fraction correlated best with the prognosis in patients with idiopathic dilated cardiomyopathy or structural abnormalities; six out of ten patients whose left ventricular ejection fraction never rose above 0.40 have either died or are in transplant protocols. Thus, study of left ventricular function can be performed easily during dobutamine infusion, generating valuable prognostic information.

An unusual posterior extension of the right lobe of the liver mimicked a retroperitoneal mass on CT scanning. Tc-99m SC SPECT imaging documented normal liver uptake within the suspected mass, proving that it was an anomalous extension of the right lobe of the liver rather than a pathologic lesion. This unusual liver variant is not believed to have been reported previously.

Xenon-133 washout phase imaging is often used to help determine whether the etiology of a perfusion defect is embolic or due to pulmonary parenchymal pathology, such as chronic obstructive pulmonary disease. This study was designed to evaluate the pulmonary blood flow patterns associated with isolated defects on xenon washout images. Scintigraphic lung studies were reviewed until 100 cases with abnormal ventilation results were obtained. Ventilation abnormalities were compared with the corresponding perfusion scan results at the same anatomic site. Of the 208 individual lung regions with xenon abnormalities, 111 showed isolated washout defects (that is, with normal washin). Ninety-four of these 111 sites showed either normal perfusion or a small, nonsegmental corresponding perfusion defect. Three segmental perfusion defects were noted in association with isolated xenon retention. In each of these cases, however, the patient was felt actually to have pulmonary embolism. Thus, it is recommended that, for interpretation of scintigraphic images in the assessment of pulmonary embolism, lung pathology associated with isolated xenon retention not be considered a potential cause for large or segmental perfusion defects.

Spontaneous pneumothorax is a serious though infrequently reported pulmonary complication of AIDS. An unsuspected lung collapse was discovered via gallium scintigraphy for the study of Pneumocystis carinii pneumonia. Neither the pneumonia nor the pneumothorax were apparent on the most recent chest roentgenogram. In evaluating gallium images during the work-up of AIDS patients with associated pulmonary pathology, the possible complication of lung collapse should be considered. If pneumothorax js suspected on gallium imaging, a chest roentgenogram in expiration must be obtained for prompt delineation of this serious, yet correctable, condition.
