摘要:
This paper presents an integration of object relations theory and family systems theory in the conceptualization of bulimia. It is based primarily on Winnicott's construct of the maternal “holding environment,” and Klein's formulation of ego deficits at the level of part‐object relations, as they apply to the family as a whole. We propose that in bulimic families there are transgenerational developmental deficits and adaptations which determine the level and quality of intrapsychic experience within individuals, as well as the interpersonal relationships and dynamic functioning of the larger family s

摘要:
Marital paradigms comprise the guiding images that serve as reference models for a couple's relationship. Such individualized images can be understood as variations on a small set of basic themes. An understanding of clients' marital paradigms can help foeus treatment on the most appropriategoals usingapproaches and techniques that are most effective for a particular couple. An unselected group of 103 couples seen in systems‐oriented marital therapy were reviewed, to identify outcome and treatment issues as a function of marital paradigm. Based on outcome criteria that took marital paradigm into account, 92% of 38 cases where both partners identified with the same basic paradigm were judged to be treatment successes, while only 60% of the cases where the partners identified with different paradigms could be considered successful outcomes. Clinical approaches based on experience with marital therapy conducted within a paradigmatic framework are presente

摘要:
This paper reexamines the Mental Research Institute (MRI) associates' intervention of refraining in marital therapy. The roots of MRI refraining are traced back to Milton Erickson. Ericksonian strategies of ratification and utilization are introduced to enhance the artistry of refraining construction and delivery. Through the use of these strategies, there is a heightened emphasis to four departures from the MRI position: (a) nonverbal language, (b) conjoint refraining,(c) unconscious “wisdom,” and (d) recursive therapy. Examples of ratification and utilization techniques illustrate how the marital therapist can have greater therapeutic impact in designing reframes based on the “raw material” of presenting couple be

摘要:
This paper examines how clients and therapists, together, create impasses or change. First, the general concept of a “therapeutic system” (therapist‐plus‐client) is briefly reviewed. Second, a model is described that synthesizes systemic and strategic ideas about therapeutic systems. Third, a series of questions based on this model is proposed for analyzing and resolving impasses. Finally, application of this framework is illustrated with

摘要:
This paper describes the computer‐based interpretive system for the Marital Satisfaction Inventory (MSI) and its application in initial phases of clinical assessment and treatment planning. The automated MSI interpretive report contains sections regarding profile validity and global marital affect, spousal communication, specific areas of interaction, concerns regarding children, role orientation, and family history of distress. A case study and sample narrative demonstrate the manner in which the MSI computerized report can be incorporated into initial clinical assessment and treatment planning. Clinical findings at intake are compared with MSI profiles for this couple obtained at termination and follow‐up. Both strengths and limitations of self‐report measures and computer‐based interpretive reports in marital therapy are con

摘要:
Today, family therapists should have an understanding of the development and maintenance of substance abusing patterns in families. In this paper, the authors describe a graduate course for teaching therapists about the contributions that have been made to the understanding and treatment of substance abuse by family researchers, theorists and clinicians. Course purpose, student selection, instructional methods and course content are delineated.

摘要:
In designing a course on the family treatment of alcohol and drug problems, a number of important issues and topics need to be addressed. A design by Heath and Atkinson (1988) provides an excellent, innovative model for such coverage. These authors, however, had to contend with certain constraints that have had a bearing on the course content and how it is taught. The present paper offers some revisions and alternatives to Heath and Atkinson's model in order to make it generalizable to a greater number of contexts. For clinicians who are less interested in teaching such a course, the two papers, combined, could serve as a guide for self‐study in this are

摘要:
Pre‐discharge psychoeducation workshops for relatives of psychiatric inpatients, conducted with and without the patient present, were compared to hospital‐tour and no‐contact control groups. Despite highly positive evaluations of the workshops, there were no group differences in patient relapse or family involvement in treatment after discharge; nor did the groups differ on questionnaire measures of relatives' illness attributions or criticism/rejection of the patient taken immediately after the workshops. Process analyses indicated that relatives made fewer personal and topic‐relevant comments over time in workshops with patients present, compared to those patients absent. Patients, however, maintained high (often disruptive) levels of partic
