摘要:
Psychotherapy is a value‐laden process because the very notions of therapeutic', ‘cure’, and ‘health’ involve patients and therapists in making value choices. Two hierarchies of health values as they apply to therapy are reviewed. The first guides the therapist in setting limits on pathological behavior and constitutes the so‐called 6 D's of pathology: destructiveness, dysphoria, disorganization, deviancy, dependency, and disillusionment. The second illuminates the therapist's role‐modeling or ideal‐setting function and takes the form of a developmental sequence of positive health values, as derived from Kohlberg's work on moral judgment. These health values, from the lowest to the highest forms, are fit‐survival, pleasure‐seeking, social‐belonging, structural duty, adaptive flexibility, and universal commitment. this hierarchy has important implications for understanding the nature of therapeutic changeand the appropriate matching of

The model of family functioning being presented is the product of over twenty years of research in clinical work with family units. A method of family therapy has been developed based upon this model and will be described in a future issue. The model utilizes a general systems theory approach in an attempt to describe the structure, organization, and transactional patterns of the family unit. It allows examination of families along the total spectrum ranging from healthy to severely pathological in their functioning. With an aim toward encouraging more precise identification and treatment of familial dysfunction, the authors suggest combining the principles of first and second order change (Watzlawick, Weakland, and Fisch, 1974) with the concept of the family life cycle (Duvall, 1971). A theoretical discussion is followed by clinical case examples.

The present study investigated a simulation game designed to improve relationship communication. Eight couples were treated for relationship difficulties by the use of the game. Before and after the game's use, the couples' communication behavior in the counseling sessions was sampled by audio recordings that were later rated by independent blind raters. The couples' out‐of‐session communication behavior was sampled by daily independent charting for a month before and after the game's use. A three‐month follow‐up of self‐reported general relationship happiness and relationship communication satisfaction was included. Results indicated that in the counseling session sample, from pre‐ to post‐game treatment, the couples' communication improved. The couples also recorded significant out‐of‐session communication improvements. This paper presents an empirical approach to supervision and training of marriage and family therapists. Advantages from the use of the empirical approach include a systematic investigation of the skills and competencies of the therapist and establishing the basis for the scientific study of supervision. A Counseling Problems Worksheet is presented to help in developing multiple hypothesis related to counselor skill deficiencies. Two case studies are given to demonstrate the use of the empirical approach.

The family therapy field is more sophisticated developmentally in its therapeutic methodologies than in the areas of training and supervision. Publications and research efforts on these latter topics have not kept pace with the progress of the clinical aspects of the field. Family therapy training and supervision literature were reviewed and found to be fragmented and disorganized. There is a need for a comprehensive source which would synthesize this literature and compare the existing viewpoints. This paper works toward this end by reviewing, contrasting, and categorizing the available publications according to a series of content areas frequently appearing in the literature. These topic areas are: (1) Goals of Training and Supervision and Skills of the Supervisor; (2) Training and Supervision Techniques; (3) Supervisor‐Supervisee Relationship; (4) Personal Therapy for Trainees; (5) Politics of Family Therapy Training; and (6) Evaluation of Training. A comprehensive table is provided to quickly access references in the content area. Recommendations are offered for future efforts in this are

This paper describes and evaluates the Marriage Encounter movement from theoretical and clinical perspectives. While acknowledging that Marriage Encounter responds to a need among many couples for greater marital closeness, the authors raise concerns about potentially destructive and illusory effects of the Marriage Encounter experience. The paper calls for more dialogue between professionals and leaders of the Marriage Encounter movement as well as for empirical evaluations of the program's effects.
