Therapists who encounter life‐threatening behavior in their clinical practice are at risk in providing interventions which do not take into account the systemic effects of their interventions. Specifically, therapists are likely to engage in “more of the same” behavior with clients who present with suicidal or homicidal threats. This paper presents two case studies that employed therapeutic methods derived from the systemic therapy of the Palo Alto group. In these case studies, the author illustrates the importance of the therapist's ability to speak the client's language in formulating an intervention that has the potential for reducing the likelihood of actual violence. The first case illustrates the use of the client's world view about her situation to assist her in dealing with domestic violence. The second case presented describes the use of systemic interventions in the case of a young woman known to have made homicidal threats. The author discusses the appropriateness, as well as the ethical issues, of the techniques described. Also offered, is a discussion clarifying the theoretical underpinnings of the clinical work presented and an amplification of some of the ideas originally presented by the Palo Alto

Despite general trends for violence in marriage to escalate in frequency and intensity, there are some couples who, through their own efforts, stop being violent. Three such couples are described below, with particular attention given to spouses' reports on the cessation of their violence. Discussion focuses on the implications of these couples' experiences for the treatment of wife battering.

In addition to the emotional, physical, and economic issues that all abused women face, battered women from fundamentalist churches have religious issues that need to be addressed. While therapists usually leave discussion of such matters to the clergy, in the case of domestic violence, the fundamentalist clergy is likely to be unsupportive or to even unknowingly endanger battered women because of their legalistic attitudes about marriage and sex roles. It is, thus, imperative that the professional counselor understand these issues and be willing to discuss them.This article explains five teachings of fundamentalist churches, explores how they help perpetuate violent relationships, and offers suggestions to therapists on how to speak with clients about these issues.

Twenty‐six couples with alcoholic husbands, and 26 maritally conflicted and 26 nonconflicted couples without alcohol problems were compared. As predicted, the alcoholic and maritally conflicted couples did not differ, and both groups of problem couples showed greater relationship distress than nonconflicted couples on measures of marital stability, change desired, and positive communication behaviors. Contrary to predictions, alcoholic couples were not unique with respect to struggles for control, a pattern of wife dominance, a responsibility‐avoiding style of communication by the alcoholic husband, or impaired perceptual accuracy. This study assessed the convergent and discriminant validity of widely used various measures of family cohension and control using a multitrait, multi‐method approach. Forty‐two families, who had at least one adolescent and who were currently receiving family therapy, completed self‐report measures of cohesion and control and engaged in Reiss' Card Sort Procedure. Ratings of the families' cohesiveness were also obtained from the families and their therapists. We were only partially successful in demonstrating convergent validity and discriminant validity for the family trait of cohesiveness across measures and methods, and even less so for family control. There was a general lack of relationship between self‐report measures and either the therapist rating of family cohesion or the family's performance on the Card Sort Procedure. In recent training literature, a consistent recommendation emerges concerning the need for an emphasis on the development of a strong system theory base upon which future practitioners and researchers can build. This paper advocates the use of Buckley's hierarchical scheme as a teaching tool to provide an understandable, yet broadly applicable, theory base for graduate students in an introductory family therapy course. A method of teaching Buckley's scheme is outlined that not only incorporates the recommendations of the literature, but also provides an umbrella model of systems for future application and theory building.

Most marital and family therapists, both medically and nonmedically trained, regularly interact with physicians on a professional basis. Though some physicians receive specific training in marital and family therapy in their postgraduate residency programs, (notably those in psychiatry and family medicine), for many, the only academic exposure they receive to this field comes during a 6‐ to 10‐week clinical clerkship in psychiatry during their third or fourth year of medical school. No study could be located which examined marital and family therapy training in undergraduate medical education.
