The care of the chronically ill traditionally has focused on the individual who has the illness. Nursing has always emphasized including the family in care, but very few protocols dealing with chronic illness focus on the family as the unit of care. In this article, the author presents a new approach to the care of families affected by chronic illness, applying concepts of the family health system (Anderson&Tomlinson, 1992) to chronic illness. The protocol described, which includes nursing assessments and interventions, groups areas of family experience into five types of processes: interactive, developmental, coping, integrity, and health processes. In this article, the authors present a protocol study developed to evaluate the usefulness of the nursing diagnosis “high risk for ineffective management of therapeutic regimen” among patients with congestive heart failure (CHF). Nursing interventions for increasing the CHF patient's abilities to perform therapeutic self‐care for the promotion and maintenance of a prescribed medication regimen were evaluated based on predicted outcomes. Orem's self‐care deficit theory of nursing provided the theoretical framework used in the protocol. Orem's supportive‐educative nursing system was the approach used. Knowledge deficit was noted to be a major obstacle to maintaining the prescribed medication regimen among CHF patients. Comprehensive teaching of the patient, supported by a therapeutic nurse‐patient relationship, was found to be the most important factor in the promotion and maintenance of a prescribed medication regimen among patien

The ultimate goal of any rehabilitation program is to discharge patients into the community with the preparation they need to meet their healthcare requirements as independently as possible (White&Holloway, 1990). Rehabilitation patients actively participate in physical therapy, occupational therapy, and, if needed, speech therapy. They should also be actively involved in their medication programs. When patients leave the hospital setting, they will be expected to be independent in their medication management. A program in which patients receive instruction on medications and are permitted to self‐administer their medications would better prepare them for this role after discharge. The purpose of the study described in this article was to examine the experience of chronic pain. Using a phenomenological approach, 15 individuals who were being evaluated for admission to, or were being treated in, a pain management center were interviewed and asked to describe their life with chronic low back pain. Data were gathered from the subjects in interviews and then analyzed. The phrase experiencing thechronic pain phenomenondescribes a process that individuals with chronic low back pain undergo, starting with the initial injury. They must deal with the uncertainty of the cause of pain; eventually, some recognize the inevitable nature of the pain. The findings indicate that chronic low back pain is a multidimensional problem that can affect every aspect of an individual's life.
