Blepharospasm is a chronic, progressive, involuntary spasmodic closure of the eyelids associated with abnormal facial and oromandibular movements. It is a neurologic disorder whose cause is unknown and whose pathophysiology is poorly understood. Without appropriate treatment, it can result in functional blindness and other disabilities. In the last decade, botulinum toxin has been found to be effective therapy for most individuals. The drug, which is given by local injection, has a denervation effect. It relieves symptoms for several months, allowing patients to resume their former lifestyles between treatments. This new therapy modality challenges rehabilitation nurses to bridge the gap between disabled persons in the community and this new technology. Casefinding, referrals, and patient education are among the interventions that can help meet this challenge. The major purpose of this article is to inform rehabilitation nurses about how to recognize the symptoms of neurologic blepharospasm and how to intervene to prevent disabilities that could result.

This study explored the relationship between health locus of control and the trajectory of multiple sclerosis. A random sample of 100 respondents with multiple sclerosis was selected from the membership list of a state multiple sclerosis support group. The subjects were interviewed in their homes and completed study instruments. Respondents with an internal health locus of control orientation had more knowledge of their disease, practiced more self‐care, and had a more benign course of multiple sclerosis than those with an external locus of control. The purpose of the research reported in this article was to describe the experiences of caregivers who were providing care to an individual who recently had been discharged from a physical rehabilitation program. The research was aimed at answering the question, “What is the meaning ascribed to the caregiver role?” A convenience sample of 10 caregivers was interviewed. Most of the caregivers (80%) were female; all were relatives of the individual requiring care. These family members believed that caregiving was their responsibility by virtue of relationship, or that no one else was available to assume the caregiving role. Many clinical nurse specialists (CNSs) are involved in providing group teaching programs for individuals with chronic obstructive pulmonary disease (COPD). Although knowledge levels often are enhanced by these programs, what is less well known is whether such programs also can affect the coping methods used by those with this disease. The exploratory study described here was part of a larger research project aimed at determining patients' knowledge, specific psychological parameters (anxiety, hostility, depression), and coping strategies. Thirty people agreed to participate in the study. The Jalowiec Coping Scale (JCS) (Jalowiec&Powers, 1981) was used so that participants could rate each coping method according to the degree of use and to determine whether the methods employed were affective or problem‐solving approaches. There was very little change in the pre‐ and posttest scores on the JCS, indicating that there was little change in coping strategies used. There also were no significant correlations between the JCS scores and the demographic characteristics of age, educational level, and the length of time since diagnosis of COPD. The results of this study seem to indicate that pulmonary rehabilitation programs should place more emphasis on appropriate coping strategies that the COPD patient can incorporate into his or her lifest

To effectively prevent and manage pressure sores, healthcare professionals need an accurate assessment and precise communication of the status of skin and soft tissue. A study by French et al. (1983) found inconsistency in staff nurses' descriptions of pressure sores. Consistency in documentation was significantly improved immediately after a standardized teaching session, but these improvements were much less evident after 6 months. Based on these findings, a pressure sore flow sheet was developed at the authors' institution, the Rehabilitation Institute of Chicago, as a result of the study. The purpose of this form is to assist the nurse in conducting a more thorough, objective, and consistent evaluation of pressure sores. The form contains assessment parameters, key descriptor words, and a diagram with axes on which to draw the sore and label significant areas such as necrotic tissue. A grading key also is included on the form to ensure consistency in grading of pressure sores.
