The purpose of the study described here was to investigate whether vinyl urinary leg and bed bags used for a 4‐week period in the acute rehabilitation setting could be safely reused after having been decontaminated daily with a diluted bleach rinse. Rehabilitation patients who had bladder dysfunction and used vinyl urinary drainage bags(N=88)were randomly assigned to control (C) and experimental (E) groups; 54 patients completed the study. Group C's bags were replaced weekly; group E's were replaced after 4 weeks. The bags of patients in both groups were decontaminated in identical fashion each day. On the patient's admission to the study, a baseline assessment was completed, a urine culture was obtained, and sterile drainage equipment was provided. Each week for 4 weeks, urine and bag characteristics and signs and symptoms of urinary tract infection were noted. No statistically significant differences(p<.5)were found between the groups. There were no clinical signs of infection requiring treatment, no changes in bag appearance or integrity, and no infection outbreaks associated with a single urinary tract pathogen that would indicate cross‐contaminat

A clinical trial comparing two intermittent catheterization techniques used with spinal cord injured patients at Parkwood Hospital, a long‐term care facility in London, ON, Canada, was conducted to evaluate the techniques' effect on urinary tract infections (UTIs). Charts were reviewed retrospectively for 92 patients with traumatic spinal cord injury who were on intermittent catheterization between January 1985 and December 1988. Nearly 80% of these patients had more than one UTI per admission. A convenience sample of 18 patients with traumatic spinal cord injury participated in a prospective study using a no‐touch method of catheterization for 7 months. Preliminary findings at the completion of the study revealed that 44.4% of this experimental group had more than one UTI per admission—a 44.5% reduction. The no‐touch method using the O'Neil Sterile Field™ urinary catheter was successful in reducing the total number of infections and duration of infection for the experimental group. 摘要:
Urinary incontinence is a common issue in the treatment of adults with head injuries, but it is frequently neglected by healthcare professionals. Because the problem is treatable, early intervention is recommended. This article presents a systematic and comprehensive approach to bladder management. This management program includes three main interventions: fluid management, establishment of a micturition schedule, and use of a behavioral approach based on positive reinforcement. These interventions can be used independently or simultaneously, depending on the nursing assessment.

Monthly urine cultures were analyzed at the University of Alberta Department of Medical Microbiology and Infectious Diseases to determine whether single‐use sterile catheters and clean technique reduced the incidence of bacterial colonization in those using long‐term intermittent self‐catheterization. Thirty subjects with spina bifida, ages 3 years to 16 years, entered a crossover study with random assignment to 6 months of sterile single‐use catheters or clean reused catheters. Seventeen subjects were catheterized by a parent or caregiver; 13 were responsible for self‐catheterization and cleaning of the catheters. Six months of descriptive data were also collected at Alberta Children's Hospital from a similar group of subjects with spina bifida who used sterile catheters only. In the crossover group, 38% of all urine cultures were positive regardless of whether sterile single‐use or clean reused catheters were employed. The other group using only sterile catheters had a 36% positive culture rate. No difference in positive cultures was found between males and females or between children who catheterized themselves and children whose parents catheterized them. The rehabilitation nurse frequently encounters patients who display cognitive/perceptual impairments as a result of a head injury or cerebrovascular accident. Deficits in attention, perception, memory, sequencing, and initiation may prevent the patient from reaching goals set by nurses or other rehabilitation professionals. When nurses recognize cognitive or perceptual impairments and have techniques available that will compensate for such deficits, they can more readily help patients meet rehabilitation goals.

摘要:
The workers' compensation patient with low back pain has long been an enigma to the rehabilitation team. A patient with a seemingly minor injury may have a lengthy disability period, while another with serious pathology may be rehabilitated quickly. The purpose of the study this article describes was to determine which factors have the most significant impact on the length of disability. The results revealed that behavioral factors are the most significant in determining a patient's length of disability. A behavioral profile, as part of a nursing assessment, can tell the rehabilitation nurse who is at risk for a lengthy disability period, thus allowing early intervention.
