摘要:
Urinary incontinence is a major problem in older adults. Research suggests that the key to successful management of urinary incontinence is careful evaluation. In recent years, bladder ultrasonography has become widely used in rehabilitation and outpatient settings. A pilot study was initiated to evaluate the effectiveness of bladder ultrasound scanning in the assessment and treatment of urinary incontinence. The study took place in two rehabilitation units of a small orthopedic hospital. The study found that the bladder scanner enables nurses to do a complete assessment of bladder function without the need for invasive catheterization.

The purpose of this retrospective study was to compare the demographic, neurologic, and cognitive characteristics of patients with completed stroke who participated in a behavioral management program for urinary incontinence. The study sought to determine the differences between those patients who achieved continence by the time of discharge from the program and those who did not. A retrospective chart review that used the Functional Independence Measure (FIM) scores identified 75 subjects with urinary incontinence at the time of admission. Forty‐two study subjects achieved continence after participating in a rehabilitation and bladder management program. Thirty‐three study subjects remained incontinent despite participation in a rehabilitation and bladder management program. Incontinent subjects had more difficulty with orientation to time, memory, and problem solving. Nurses' perceptions of their professional role and their experience of role conflict were explored in a sample of certified general rehabilitation nurses. Nonnurse healthcare professionals who are on the same teams as the participating nurses also were assessed for their expectations of the nurse's role. The level of locus of control was identified among the various professions. The major finding of the study was that the rehabilitation nurses shared a consistent view of their professional role and of how they believed other professionals saw this role. The results showed that the nonnurse professionals were to some extent unaware of the expectations that the nurses held for the nursing role. There was no appreciable difference in the level of locus of control among team members, a situation which may have a leveling effect on the team dynamics. These findings have implications for professional role clarity, the promotion of locus of control, and interdisciplinary team dynamics.

Three methods of cleaning urinary catheters for reuse at home by clients on intermittent catheterization programs were studied. Reused catheters were collected from clients, autoclaved, and then incubated in a culture of Escherichia coli broth. Three different isolates of E. coli were used at concentrations ranging from 4.8 × 105to 1.0 × 108colony‐forming units per milliliter. The catheters were then rinsed with tap water for 1 minute and soaked in one of three cleaning solutions for 30 minutes. The three cleaning solutions studied were 0.6% hydrogen peroxide, bleach in a 1:4 solution with tap water, and betadine in a 1:2 solution with tap water. Many patients admitted to an inpatient rehabilitation setting experience alteration in urinary elimination as a result of neurological changes. Urinary retention can develop due to hypotonic bladder, increased sphincter tone, absent or decreased sensation of bladder fullness, or urethral blockage. Incomplete bladder emptying can lead to such complications as urinary infection and incontinence. Rehabilitation nurses must assess patients for alterations in bladder emptying. The use of bladder ultrasound is one method now available for noninvasive measurement of bladder volume. In this article, the author reviews the implementation and results of a bladder ultrasound program at one rehabilitation facility.

Ongoing competency evaluation of all nursing staff in rehabilitation settings is mandated by regulatory bodies. Rehabilitation nurse educators are challenged to design competency evaluation programs that meet regulatory standards, promote the professional development of rehabilitation nurses, and are cost‐effective. The competence of rehabilitation nurses is maintained through a combination of ongoing assessment and educational activities. Key components of program development include identifying critical nursing and rehabilitation nursing skills; developing an efficient documentation system; designing the evaluation, program promotion, and implementation activities; and evaluating the program. The strategies described meet administrative and educational need

The purpose of this study was to assess the impact of chronic illness on patients in relation to its physical, psychological, and social effects, and its effects on activities of daily living. A structured questionnaire was used to interview 177 patients. Chronic illness had the greatest effect on psychological functioning, followed by physical and social functioning. The least affected aspects of a patient's functioning were related to performing activities of daily living (ADLs). Results indicated that the diagnosis, duration of disease, and developmental stage of the patient's children were useful in predicting the impact of a chronic illness on a patient's physical functioning. Diagnosis, duration of disease, and economic status had a significant impact on a patient's psychological functioning. Diagnosis, duration of disease, age, sex, occupation, education, and marital and economic status were important factors relative to social functioning. Diagnosis, age, marital status, the developmental stage of the children, and the primary caregiver were the most important factors related to the effects of chronic illness on ADLs. The results of the study serve as a guide for a comprehensive assessment of chronically ill patients.
