Back pain is a significant cause of functional impairment in elderly women with vertebral fractures. To determine the degree of functional impairment due to painful osteoporotic vertebral fractures, investigators compared 75 white women with a mean age of 77 (±7.2) years who had a mean number of 2.8 (±0.87) vertebral fractures with 83 white women with a mean age of 74 (±6.5) years who had no vertebral fractures. These patients were seen within a 2‐year period. The Katz Activities of Daily Living scale was used to assess the patients' functional status. All patients underwent a comprehensive evaluation that included a detailed history and a physical examination. Bone density was evaluated in most of the patients and the evaluation revealed osteoporosis in all of the patients with vertebral fractures. The women with vertebral fractures were found to be more significantly dependent in the following activities of daily living: bathing, toileting, dressing, transfer, and continence. The number of elderly people in America is growing, and falls among the elderly population are a significant problem. Injuries and their complications can have serious consequences requiring medical attention, hospitalization, and nursing home admission. Seventy percent of the fall‐related deaths in the United States occur among elderly people. Risks for falls are categorized as extrinsic factors involving the environment, intrinsic or host factors, those associated with nonbipedal falls, risks related to nonclassified falls, and agent, or iatrogenic, risk factors. Lawsuits related to falls and restraints are increasing. Fall prevention programs, assessment, evaluation, and lobbying for legislation to assist elderly people who fall are some of the responses made by the healthcare system. Rehabilitation nurses must be aware of the incidence and consequences of falls. It is imperative that nurses become involved in coordinating fall prevention and fall reduction programs, monitoring risks for falls, and implementing corrective measure

As one of the leading causes of death in the United States, coronary atherosclerotic heart disease (CAHD) has been shown to have a reduced incidence if the principles of primary prevention are instituted to reduce cardiac risk factors. In people who already suffer from CAHD, employing secondary prevention such as controlling hypertension, reducing serum lipid levels, eliminating nicotine intake, reducing stress, losing weight, and increasing exercise habits can decrease recurrence of problems related to CAHD. Cardiac rehabilitation programs are a way to help CAHD patients restore optimal medical, physiological, psychological, social, and vocational performance following an acute cardiac event. A major problem in cardiac rehabilitation programs is patient dropout. For any cardiac rehabilitation program to be a success, the CAHD patient must become invested in the maintenance of health and, therefore, committed to participation in the program. The author reviews the current literature, including discussions of the history of cardiac rehabilitation, current trends in cardiac rehabilitation programs, factors that influence patients' investment in these programs, and ways in which the cardiac rehabilitation nurse might intervene to make an impact on patients' participation.

This study compared gender differences in outcomes of patients who had had coronary bypass surgery and either attended structured cardiac rehabilitation programs or participated in home programs. The principal advantages related to patients' attending a structured hospital‐based outpatient rehabilitation program, compared to a home program, included increased exercise adherence for males and females, increased knowledge about the condition for males, and increased stress control for females. Return to work, self‐efficacy, and aspects of self‐care such as adherence to a medication regimen and smoking cessation were not significantly different, regardless of type of program, for either sex. In general, females at home fared the worst regarding lifestyle changes, showing decreased exercise adherence, decreased ability to control stress, and no significant increase in knowledge about their medical cond

Chronic obstructive pulmonary disease (COPD) is the second most significant illness in those who are in their middle‐to‐late adult years. Pulmonary rehabilitation is one means of working with individuals with COPD to improve their quality of life. Nurses are often the facilitators and coordinators of pulmonary rehabilitation. Evaluation of the effects of pulmonary rehabilitation is an essential aspect of providing quality care. The purpose of this study was to evaluate the relationship of pulmonary rehabilitation to individuals' use of healthcare services and to their psychosocial adjustment to COPD. A convenience sample was obtained from an outpatient pulmonary rehabilitation program associated with a rural regional medical center in the southwestern United States. The sample consisted of 13 individuals who had no formal pulmonary rehabilitation and 17 individuals who had completed a formal pulmonary rehabilitation program. Psychosocial adjustment was measured by the Psychosocial Adjustment to Medical Illness Scale‐Self Reporting (PAIS‐SR).
