Rehabilitation programs for myocardial infarction (MI) survivors are designed to alter survivors' self‐care patterns and to improve long‐term physical and psychological outcomes. The purpose of this study was to examine the relationship between participation in cardiac rehabilitation and health state; days of reduced activity; anxiety; depression; self‐esteem; quality of life; and performance of exercise, diet, medication, stress‐modification, and smoking‐reduction self‐care behaviors after MI. Interviews were conducted with 197 women and men 1 to 2 years after their initial MI to measure health state, mood, self‐esteem, quality of life, and relevant self‐care behaviors. Rehabilitation center records were reviewed to determine participation in rehabilitation programs. Rehabilitation participation was significantly associated with health state; days of reduced activity; self‐esteem; quality of life; and performance of exercise, diet, and medication self‐care. One hundred patients with coronary artery disease (CAD) were studied to determine which factors affect their baseline knowledge and retention of knowledge about CAD and whether CAD classes are an effective teaching tool. All patients were given a pretest about CAD. Patients then attended a 1‐hour rehabilitation class about CAD. One month later, they were given an identical posttest. Pretest scores had a negative correlation with patient age and positive correlations with the number of years of formal education, the number of previous myocardial infarctions, and the number of previous CAD classes attended. Posttest scores had a positive correlation with the number of previous CAD classes attended. There was a significant relationship between learning and marital status, with married patients learning better than unmarried patients. Comparison of patients' pretest and posttest scores revealed a highly significant improvement. Based on the results of this study, the CAD rehabilitation class is an effective tool for rehabilitation of the cardiac patient; additional teaching effort should be focused on unmarried patients. This article details a quantitative, descriptive study that explored the effects of participation in two types of cardiac rehabilitation programs on psychosocial functioning and life satisfaction. Forty‐seven clients with coronary artery disease (CAD) were included in the study sample. Data were collected during a home visit made 6 to 8 weeks after the coronary event. All subjects completed two data collection tools: the Sickness Impact Profile (SIP), which measured clients' psychosocial functioning, and the Quality of Life Index (QLI), which measured clients' life satisfaction. No significant differences were found between the two treatment groups in psychosocial functioning or life satisfaction. Psychosocial functioning and life satisfaction were found to be related strongly. This article describes a specialized rehabilitation education program, based on adult learning theory, that was developed to promote retention of rehabilitation nurses at MetroHealth Center for Rehabilitation in Cleveland. The curriculum encompasses the skills required to care for rehabilitation patients needing both advanced rehabilitation nursing and critical care nursing. Critical care and rehabilitation nurses collaborated in the presentation of topics and were guided by the course coordinators. 摘要:
Some facilities are adopting a new organizational model to deliver rehabilitation services: the programmatic model. The premise of this model is the delivery of completely integrated services. The programmatic model differs from the traditional service model in many aspects, including philosophy, governance, clinical practice standards, and identification of patient care goals. There are five phases involved in moving from a pure service model to a programmatic model: commitment, enmeshment, product orientation, integration, and maintenance and growth. This article describes the clinical nurse specialist role and experiences in implementing a programmatic model in a rehabilitation hospital.

Progressive neurologic disease has a significant impact on family members, particularly those living with and caring for a homebound adult. Focus groups were conducted with women who were the primary caregiver for their disabled husband. The women described themselves as being restricted to the home by caregiving requirements. Multiple sclerosis was a critical personal hardship for them, involving persistent struggle that often resulted in anger and frustration. Because of the social isolation created by their caregiving responsibilities, these women depended on their disabled husband for support. Yet, never having time away from the husband strained the marital relationship and threatened the supportive nature of the relationship. The women in this study found it necessary to have personal time and space away from their husband. They created space for themselves by setting apart a place in the home that was theirs or by declaring time out for themselves. These strategies helped them avoid or reduce negative outcomes of the social support received from their husband.
