Coma rehabilitation programs are proliferating in the United States. Their aim is to stimulate arousal of comatose patients and to rehabilitate them to a higher level of functioning. The International Coma Recovery Institute team evaluates comatose patients and designs individualized stimulation programs that are carried out by patients' families. Family support groups are provided. A case study illustrates the functioning of the program.

A pediatric rehabilitation facility has been treating children with head trauma for many years. Through direct observation it was concluded that early stage intensive therapy favorably influenced outcome. A comprehensive data collection instrument was retrospectively employed to compare and contrast outcomes of 160 children with a diagnosis of head trauma. The average age was 12.7, and there were twice as many boys as girls. Seventy‐five percent of the injuries resulted from vehicular accidents. Sixty‐nine percent of the children sustained both head and associated injuries. Of those requiring rehabilitation, 48 percent showed good recovery and 24 percent had moderate disability. Of the 68 percent comatose on admission, only 16 percent remained in a persistent vegetative state at discharge. This pilot study assessed the predictive ability of the prognostic expectations of 19 myocardial infarction (MI) patients relative to their functional health status (as measured by the McMaster Health Status Index) eight weeks post‐MI. The relationship of patient expectations, medical severity of the MI and functional health status at two days prior to hospital discharge and eight weeks post‐MI were also described. In spite of the constancy of low medical severity of the MI, there was no significant improvement in overall functional health status scores for patients between the time of hospital discharge and eight weeks post‐MI. The raw score comparison of the physical, emotional and social components of functional health status outcome indicated general improvement in physical functioning and a downward trend in emotional and social components over time. As the majority of patients had good expectations and fair outcome scores, it could be concluded that neither medical severity of the MI nor prognostic expectations defined good or poor functional health status at eight weeks post‐MI. The main purpose of the Diagnostic Related Group (DRG) prospective payment system is decreased hospital utilization. Shortened lengths of stay in acute care hospitals may result in increased acuity levels of patients entering rehabilitation facilities. Since there are no reports of this phenomenon in the literature to date, this topic will be reviewed from the experience of one rehabilitation hospital. Data are presented from the nursing patient acuity system for the years 1984–1986. These are then compared to the functional rating scales of patients on admission to determine if the patients are sicker or more functionally disabled. Findings reveal that patients required more nursing care for all rehabilitation diagnostic categories in 1985 than in 1984, but less care was required in 1986 than in 1985 (with the exception of amputee patients). In five out of eight patient categories, patients had better functional abilities on admission in 1986 than in 1985.
