SUMMARYThe sodium excretion in the urine was measured during the first 24 hours in patients subjected to cholecystectomy or hysterectomy. Group 1 received an infusion of 1500 ml glucose, group 2 1000 ml of 0.9% NaCl + 500 ml glucose and group 3 2000 ml 0.9% NaCl.The volume of urine produced was the same in all the groups. The sodium concentration in the urine and thus the sodium excretion increased with increasing administration of sodium.The serum sodium concentration decreased in the group that did not receive sodium. The venous haematocrit increased in the same group.If an amount of 150 or 300 mEq. of sodium was given on the day of operation, the serum sodium concentration was unchanged 24 hours after operation.Administration of 300 mEq. of sodium was followed by retention of approx. 200 mEq. in comparison with the patient group that did not receive sodium. This is a sodium retention of the same magnitude as was found in a group of non‐operated persons who were subjected to infusion of 300 mEq. sodium.It is concluded that there is no basis for the assumption that sodium cannot be excreted in the immediate postoperative period. In order to maintain an unchanged serum sodium concentration on the day of operation and in order to avoid an increase in the venous haematocrit, it is necessary to administer fluid containing sodium on the day of cholecystectomy or hysterectomy.The changes observed in the serum‐sodium concentration and haematocrit presumably result from a reduction in the active extracellular fluid volume in the patients who did not receive sodium ions.zusammenfassungDie Natriumausscheidung im Harn wurde während der ersten 24 Stunden bei Patienten gemessen, die einer Cholezystektomie oder Hysterektomie unterzogen worden waren. Gruppe 1 erhielt eine Infusion von 1500 ml Glukose, Gruppe 2 1000 ml 0,9% iges NaCl plus 500 ml Glukose und Gruppe 3 2000 ml 0,9 %iges NaC1.Das Harnausscheidungsvolumen war in allen Gruppen gleich. Die Natriumkonzentration im Harn und somit die Natriumausscheidung nahm mit vermehrter Verabrechung von Natrium zu.Die Serum‐Natriumkonzentration nahm in der Gruppe, die kein Natrium erhielt, ab. Der venose Hamatokrit nahm in derselben Gruppe zu.Wenn 150 oder 300 mEqu. Natrium am Tag der Operation verabreicht wurden, blieb die Serum‐Natriumkonzentration 24 Stunden nach der Operation unverändert.Der Verabreichung von 300 mEqu. Natrium folgte eine Retention von ungefähr 200 mEqu., verglichen mit jener Gruppe, die kein Natrium erhalten hatte. Das bedeutet eine Natriumretention der gleichen Grössenordnung, wie bei jener Gruppe, die nicht operiert wurde und Infusionen von 300 mEqu. erhalten hatte.Es wird daraus der Schluss gezogen, dass es keinen Grund für die Annahme gibt, dass Natrium in der unmittelbaren postoperativen Phase nicht ausgeschieden werden kann. Um eine unveränderte Serum‐Natriumkonzentration am Tag der Operation aufrecht zu erhalten und um ein Ansteigen des venösen Hämatokrits zu vermeiden, ist es notwendig natriumhäaltige Flüssigkeiten am Tag der Cholezystektomie oder Hysterektomie zu verabreichen.Die beobachteten Veränderungen der Serum‐Natriumkonzentration und des Hämatokrits ergeben sich wahrscheinlich aus der Verringerung des aktiven extrazellulären Flüssigkeitsvolumens bei Patienten, die keine Natr

SUMMARYFor the treatment of chronic “whiplash” injuries, total spinal blocking was performed by infusing a mixed solution containing mepivacaine with adrenaline (1: 200,000) (Carbocaine®) into the subarachnoidal space.
